After the storming of the Capitol on Jan. 6, calls for the impeachment and removal of President Trump have reverberated throughout the Democratic Party, including from House Speaker Nancy Pelosi and incoming Senate Majority Leader Chuck Schumer. But with less than two weeks left in Trump’s term, many pundits and politicians have acknowledged that impeachment proceedings may not be logistically or politically feasible.

Believe it or not, Trump’s exit from office on Jan. 20 is not the end of the impeachment conversation. Post-presidency impeachment may not only constitutionally be possible, but politically feasible given the bipartisan support for Trump’s impeachment today.

Trump Administration Partners with Chain and Independent Community Pharmacies to Increase Access to Future COVID-19 Vaccines, November 12, 2020

To maximize access to COVID-19 vaccines for all Americans, the U.S. Department of Health and Human Services (HHS) today announced the U.S. government’s partnerships with large chain pharmacies and networks that represent independent pharmacies and regional chains. Through the partnership with pharmacy chains, this program covers approximately 60 percent of pharmacies throughout the 50 states, the District of Columbia, Puerto Rico, and the U.S. Virgin Islands. Through the partnerships with network administrators, independent pharmacies and regional chains will also be part of the federal pharmacy program, further increasing access to vaccine across the country—particularly in traditionally underserved areas.

“Ensuring access and affordability of the COVID-19 vaccine for all Americans is a top priority for the Trump Administration,” said HHS Secretary Alex Azar. “We are leveraging the existing private sector infrastructure to get safe and effective vaccines supported by Operation Warp Speed into communities and into arms as quickly as possible with no out-of-pocket costs. The vast majority of Americans live within five miles of a pharmacy, and our new agreement with pharmacy partners across America is a critical step toward making sure all Americans have access to safe and effective COVID-19 vaccines when they are available.”

It’s time to consider delaying the second dose of coronavirus vaccine

Until recently, we supported the strict vaccine regimen, requiring two doses within a month. Both the Pfizer and Moderna clinical trials were carried out this way, and the much-touted 95 percent efficacy rates were drawn from those trials.

But as Mike Tyson famously said, “Everybody has a plan until they’ve been punched in the mouth.” When it comes to covid-19, we’re being punched in the mouth over and over again. It’s time to change the plan; namely, we should give people a single vaccination now and defer their second shot until more doses of vaccine become available.

What changed? First, there’s simply a supply constraint. We hoped that additional vaccines would be available by now. But only the Pfizer and Moderna vaccines have been authorized, and they’re being produced more slowly than hoped. Even more worrisome are the distribution bottlenecks that are making it difficult to vaccinate people as quickly as possible.

As of Jan. 1, the United States has given first vaccine doses to about 1 percent of our population. Contrast this with Israel, which has vaccinated 10 percent of its population, including more than 40 percent of people older than 60. The benefits of the immunity these shots produce accrue not just to those getting vaccinated, but to broader society as well by slowing the virus’s spread. Until a lot more people are vaccinated, this latter benefit will be minimal.

Another factor is the state of the pandemic: Hundreds of thousands of people are being infected and thousands are dying every day. Any high-risk person who isn’t vaccinated today could be infected tomorrow and die in a month. But we are holding back half of the doses in reserve for people to get their second shots. That slows things down.

Now comes another punch to the mouth: the new variant of the coronavirus, which is likely much more contagious and is spreading rapidly in multiple U.S. communities. Epidemiological models and Britain’s experience indicate that, while only a few cases of the variant have been identified in the United States, it will likely become our dominant strain within a few months. Although the new variant doesn’t seem deadlier at the individual level, a more infectious virus means more cases, which means more hospitalizations and deaths. We need to vaccinate as many people as quickly as possible to save the most lives.

Whether the bottleneck is due to vaccine production or distribution, giving people a single shot now and waiting on the second until the shortage abates will get more people vaccinated sooner. But will it work? Here, the clinical trial results for the Pfizer and Moderna vaccines are reassuring. While they were designed to test the effectiveness of two shots given about a month apart, both showed that the first shot had a substantial benefit beginning around 10 days afterward. In both trials, by the time of the second shot, the first was already 80 to 90 percent effective in preventing covid-19 cases.

Across the United States, limited funding has undermined end-of-year efforts to hire coronavirus vaccine staff, create vaccination centers and raise awareness. We are not arguing against people receiving two shots. The second shot adds a measure of effectiveness and likely makes vaccination-derived immunity more durable. That’s why we’re advocating that it be delayed, not dropped altogether. And an argument can be made that for groups at highest risk for exposure (such as health-care workers) and for death (such as Americans over age 75), sticking with the early second-shot strategy is reasonable. But for everyone else, the deferral strategy makes sense.

Is there any potential downside to a delay? Yes. The strategy hasn’t been studied, so we don’t know if and when the immunity from the first shot begins to wane. But data from Moderna, for example, is reassuring. It shows robust immune response four weeks after the first shot, and most experts believe it is extremely unlikely immunity would somehow plummet by week eight or even week 12 following a single shot. Another concern is that with a delay, some people may forget to get their second shot. It will require effort to track people and remind them to receive it.

A more general risk is that this would be yet another change in our pandemic strategy at a time when people have grown weary of our ever-changing response regarding opening schools, touching surfaces and wearing masks. If this change causes them to become exasperated and choose not to be vaccinated at all, that would be a problem. But we believe the advantages of rapidly vaccinating twice as many people would outweigh these potential disadvantages.

In a perfect world, there would be no trade-offs. But if 2020 taught us anything, it’s that we don’t live in a perfect world. Between 50,000 and 100,000 Americans will die of covid-19 this month alone. Giving 100 million people — particularly those at high risk — a single shot that is 80 to 90 percent effective will save far more lives than giving 50 million people two shots that are 95 percent effective. It’s what we should do.

“I don’t think it’s a conspiracy theory. I think it’s a legitimate question that needs to be investigated and answered. To understand exactly how this originated is critical knowledge for preventing this from happening in the future.” Xiao Qiang, a research scientist at the School of Information at the University of California at Berkeley.

It is the mission of the International Center for Technology Assessment to assess the impacts of technology and technological systems on society. As such we have undertaken to better understand the possible role of genetic engineering and other laboratory techniques in the creation of the COVID-19 virus. After considerable research, including a thorough review of the selected research materials included below and the citations thereto, and discussions with experts did not evolve naturally but rather is the product of one of the high-security bio-medical laboratories in Wuhan, China.

We believe that there is a preponderance of circumstantial and scientific evidence demonstrating that the “laboratory virus” hypothesis is not only possible but probable.

By contrast, recent refutation of the hypothesis that the virus originated at a Wuhan “wet market” and new findings that the virus has not been found in nature despite significant effort to do so, makes the view that the virus evolved naturally unlikely. (ICTA will be publishing a full Report on its Findings and Conclusion on the origin of the COVID-19 virus in September 2020.

No dispositive finding on the virus’ origin can be made without a full review of the records and logs of the Wuhan high security laboratories involved, which the current stance of the government of China makes improbable. Nevertheless, in coming to a conclusion as to the probability of its laboratory origin, ICTA understands that it is critical that any analysis of the origin of this catastrophic contagion be apolitical and constructive. ICTA’s work in this area is not intended to blame individual scientists or any country, but rather to help provide the insight, and encourage the action needed to spare humanity from a series of future man-made pandemics that could surpass the current one in transmissibility and lethality.

It may seem that engaging in exploration of the origins of the COVID-19 virus should wait until the current pandemic has been addressed and mass suffering alleviated. However, consideration of the “laboratory virus” theory matters because it highlights the most important preventive strategy that humanity can take to avoid repeats of the tragic pandemic we are all living through. This involves halting dangerous ongoing biomedical research on potential pandemic viruses. This important and necessary strategy has been completely absent in media coverage of COVID-19s origin. As a result its importance remains opaque to the general public, even though ignoring this issue threatens our individual and collective health security every minute, every hour, and every day.

Moreover, with billions earmarked for coronavirus research in the aftermath of the pandemic, there is the real possibility that a significant portion of these funds will be spent on this dangerous research which would increase the risk of future pandemics rather than lessen that threat.

Reinstate the US government moratorium on funding “Gain of Function/Gain of Threat” research on potential pandemic viruses.

The major international bio-security threat made more apparent by the probable origin of the current pandemic goes under the innocuous term “gain of function” research, often shortened to GoF. This term refers to research that uses and combines laboratory techniques including, but not limited to, genetic engineering, animal experimentation and cell and tissue culture research to increase the transmissibility and/or lethality of pathogens, including the most dangerous pandemic viruses in the world. ICTA believes that a more apt term for this research is “Gain of Threat (GoT)” rather than “Gain of Function (GoF).” To make that point we will refer to them together.

Some researchers insist that there are benefits to taking potential pandemic viruses and engineering them to create novel viruses even more catastrophically transmissible and deadly. We have not seen any evidence of such benefits in our research. We agree with Thomas Inglesby, director of Center for Health Security at Johns Hopkins, that this gain of function/gain of threat (GoF/GoT) research has little or no value for vaccine development or other intervention strategies and that there is no evidence “that the information people are pursuing could be put into widespread use in the field.”
 
We also agree with Rutgers University molecular biologist Richard H. Ebright that this research threatens our health security and deflects funds from vaccine or anti-viral drug research. He notes that “after the SARS virus, US funding agencies spent hundreds of millions of on global virome and gain-of function research that increases the risk of outbreaks and provided no information-absolutely no information-useful for preventing of combating outbreaks.”

While the benefits of GoF/GoT research are debatable, the risk of this research is well established. Concerns about this research came to a head almost a decade ago when two research teams, one in the United States and the other in the Netherlands were funded by the NIH to do GoF/GoT research on the avian flu virus H5N1. This virus causes a 60% mortality rate among those infected. Fortunately it is not easily transmissible so it has never reached pandemic proportions. To increase the transmissibility of the virus these two research teams genetically engineered it and used animal research to allow airborne transmission of this deadly flu. If accidentally released or stolen from one of these laboratories, this novel and enhanced virus could lead to a pandemic killing 1.6 billion people.

The publication of this research resulted in hundreds of eminent scientists PUBLICALLY urging that this research be halted.In 2014, in response to this unique the Obama Administration wisely declared a moratorium on this research. The moratorium announced by the Office of Science and Technology Policy (OSTP) stated that:

The U.S. Government will institute a pause on funding for any new studies that include certain gain-of-function experiments involving influenza, SARS, and MERS viruses. Specifically, the funding pause will apply to gain-of-function research projects that may be reasonably anticipated to confer attributes to influenza, MERS, or SARS viruses such that the virus would have enhanced pathogenicity and/or transmissibility in mammals via the respiratory route.

Unfortunately, under pressure from a group of powerful scientists- many in leadership positions at the NIH- this moratorium was lifted in December 2017. Soon after, both of the H5N1 experiments were refunded without public disclosure, and are ongoing, subjecting the world’s population to the continuing threat of accidental release of this novel virus or its theft and use in bioterrorism.

Gain of Function/Gain of Threat research to create novel pandemic viruses is now being practiced in laboratories around the globe mostly in secret, and no doubt, some of this research involves bioweapons research. This research now includes synthetic biology, synthetic virology, gene editing and other cutting edge technologies. It is difficult to fully and accurately assess the risk to public health from this research. It is well established that accidental releases of pathogens from high security laboratories are all too common. It is also well established that biosecurity in labs around the world is weak. Given these variables, Mark Lipsitch, Professor of epidemiology at Harvard University, has attempted to come up with a numerical assessment of that risk. He posits that for every year that a researcher works on increasing the transmissibility and/or lethality of a potential pandemic virus, there is a 1/1000 to 1/10,000 chance of an accidental release.

Hundreds of researchers are now doing such work which increases the risk significantly.

ICTA’s upcoming Report on the origins of the COVID-19 virus will discuss in-depth the GoF/GoT research most relevant to the origins of the current pandemic. This involves grants from the US National Institutes of Health and USAID over the last 5 years to researchers at the Wuhan Institute for Virology (WIV). This funding was closest to the COVID-19 virus to make them more easily transmissible and lethal.

However, and we believe this is of key importance, the urgent need to reinstate the moratorium on GoF/GoT research does not rest on proving that it was definitely the cause of COVID-19 but only that it was a possible cause. In other words the burden on those who want to continue this highly hazardous research in the wake of the pandemic is to demonstrate that it could not have created the pandemic. This they cannot do. The WIV’s own publication of its research and the testimony of well-respected scientists and observers have made it evident that virus research conducted at one or both of the Wuhan high security labs was apossible source of the virus. This includes Shi Zhengli the controversial Chinese researcher at the head of GoF/GoT research with bat coronaviruses at the WIV who admitted having many sleepless nights worrying that the COVID-19 virus could have originated from her laboratory.

To ensure against future pandemics, there needs to be an urgent public education campaign so the American people, who have been kept in the dark about this research, can better understand its profound potential impacts on their lives. This needs to be accompanied by a renewed call by hundreds of responsible members of the scientific research community, similar to that which occurred in 2014, urging the reinstitution of the moratorium on all federal funding of GoF/GoT research, and a strong recommendation that private laboratories do the same or lose potential future government funding. This moratorium should be for a minimum of 10 years or longer until the scientific community better understands the origins of the current pandemic, there is a robust public debate about the threat of such research to US health security and significant, measurable improvement in the biosecurity at US and international high security laboratories.

ACTION STEPS:
A) ICTA will be working to achieve bipartisan support in Congress for a halt of all appropriations to the NIH or any other government entity for GoF/GoT research.

 B) ICTA will work to help organize an open letter to the White House in support of reinstating the 2014 moratorium on GoF/GoT research which will include provisions mandating no exceptions and a minimum 10 year period.​

“After considerable research, including a thorough review of the selected research materials and discussions with experts in the field, we have come to agree with the view that the virus causing COVID-19 did not evolve naturally but rather is the product of one of the high-security bio-medical laboratories in Wuhan, China,” the group said in a statement issued last month.  “We believe that there is a preponderance of circumstantial and scientific evidence demonstrating that the ‘laboratory virus’ hypothesis is not only possible but probable. By contrast, recent refutation of the hypothesis that the virus originated at a Wuhan wet market and new findings that the virus has not been found in nature despite significant effort to do so, makes the view that the virus evolved naturally unlikely.” 

“No dispositive finding on the virus’ origin can be made without a full review of the records and logs of the Wuhan high security laboratories involved, which the current stance of the government of China makes improbable. Nevertheless, in coming to a conclusion as to the probability of its laboratory origin, ICTA understands that it is critical that any analysis of the origin of this catastrophic contagion be apolitical and constructive. ICTA’s work in this area is not intended to blame individual scientists or any country,  but rather to help provide the insight, and encourage the action needed to spare humanity from a series of future man-made pandemics that could surpass the current one in transmissibility and lethality.”

Andrew Kimbrell is executive director of the International Center for Technology Assessment. 

“Let’s start with the probability – more likely than not – that the COVID-19 virus is a lab created virus – from one of the two labs in Wuhan China,” Kimbrell told Corporate Crime Reporter in an interview last month. 

“Let’s take a look at the virus itself.” 

“Is there anything about the virus that would indicate one way or another? The other four categories are more circumstantial. Circumstantial evidence is fine in a court of law.” 

“One is – location. Where did it happen?”

“Two – precedent. Has anything like this ever happened before?”

“Three – warnings. Did anybody warn that this might happen?”

“And four – cover-up. Did the labs and the Chinese government try to cover it up?”

“Those are the five categories that I would ask your friends and skeptics to go through carefully before they use words like conspiracy or baloney. And later on I will go through why some of them are using those terms. We will get into the corporate support for these people and why you are getting this misinformation.”

“Let’s go through it. It is undisputed that this is a chimeric virus that has never been seen before. It’s a hybrid virus “

“The bat coronaviruses that are closest to COVID-19 are lacking two incredibly important things that COVID-19 has that make it so dangerous. One is the proteins that spike the cell – the spike proteins. The spike proteins that are on COVID-19 are completely different than those on the bat coronaviruses that are closest to it otherwise. Then there is the furin cleavage site. This is something that allows the virus to get inside the cell and have the cell mechanism reproduce it. That does not exist in this group of bat coronaviruses.” 

“You have a basic bat coronavirus and you have two things that have been added to it. The spike protein is closest to an animal called the pangolin. We do know that somehow this bat virus was infected by at least two other animals and then went into a human host. And for that virus to be the way it is, it had to happen simultaneously.”

“We have a hybrid virus never seen before in nature, it had to have been infected simultaneously with these other elements that make it more dangerous – make it more infective and more transmissible.”

“There is no theory about how they got in there. They used to think it was the wet market. That has been completely debunked, including by the Chinese government. No one believes that anymore. That explanation was a smoke screen put up by the Chinese and Americans who want to support that idea.”

What are the chances it happened naturally?

“Someone will have to come up with a scenario. It sounds almost like a joke. A horseshoe bat, a pangolin and some other creature met in a bar in Wuhan and somehow simultaneously infected them.”  

“I haven’t seen any scenario of how that happened or where that happened. But we know that had to happen. It happened somewhere. It either happened in nature or it happened in the Wuhan Institute of Virology or it happened at the CDC lab in Wuhan.” 

“That is undisputed. Then at the end of May, Nickolai Petrovsky and his team in Australia said – let’s see if we can find a creature that might have an affinity for this. That way we might find the animals that might have come together to create this virus. Their conclusion was that they could not find it anywhere else in nature. These are objective researchers. They are not Trump supporters. That study made it even more difficult to accept the natural theory.”

“Meanwhile, we know that this was exactly the kind of work that was going on at one or both of the Wuhan labs. They call it gain of function research. I call it gain of threat research. They were taking NIH money, through the EcoHealth Alliance to do exactly this. And they did exactly this. They added different kinds of protein spikes. They mixed and matched various viruses. They genetically engineered them. They infected a number of animals. They put them into human cell cultures to increase the threat.”

Why were they doing this research?

“The point of the research was to collect all of these bat viruses from 1,000 miles away from Wuhan and bring them back into their labs. The bat coronavirus was also the basis for the first SARS outbreak. They collected the bat viruses and brought them back to the labs. And then we are going to see what it would take for them to become really dangerous. What would it take? The idea was – if we can show what it takes in a laboratory for them to become incredibly dangerous then maybe we can predict that happening in nature. And then maybe we could have vaccines or interventions and be ready for the next pandemic.”

It was a way to develop vaccines?

“No. It was a way to develop a potential pandemic virus that might have occurred in nature at some point in the future. By having it, they would be able to think about what intervention strategies might work against this virus, which is now only in the lab, not in nature.”

“They would say – we’re trying to not have the next pandemic. And there are a couple of problems with that argument. I sent you an article by Marc Lipsitch at Harvard and Tom Inglesby at Johns Hopkins. They pretty much demolished this argument. They say – there are hundreds of combinations of coronaviruses that could happen in nature. The idea that you can pick one or two and that is going to be the one that nature comes up with is like winning the lottery. And then to create a vaccine for a non existent virus – except in your laboratory – no one is going to do that. They are going to wait to see what happens in nature.” 

“This whole gain of threat research, there are many reputable scientists now saying – it gives you no information, it’s not useful for vaccines, it’s not useful for anything except for the curiosity and interest of this small group of scientists who do this research.”

“Meanwhile they are creating novel pandemic viruses.”

“Let’s get back to the list.”

“Location. Why did this happen in Wuhan? Of all the cities in China. Of all the areas where bats are – and they are nowhere near Wuhan, they are 1,000 miles away. Of all of the cities it could have happened in, of all the small towns it could have happened in, why did it happen in Wuhan? What are the odds of this happening in Wuhan naturally versus happening in Wuhan because researchers there were doing exactly the kind of research that would create it? What are the odds of that? If I was in court, I would say that’s a very strong indicator that it happened in the labs. And in the interview with Shi Zhengli, she was so surprised. Why would this happen in Wuhan? And that’s why she got so nervous. Check that in favor of the lab theory.”

“Two is precedent. Was there any precedent? Yes. In 2003 and 2004, the original SARS virus was leaked four times from Chinese laboratories. It was reported in Science magazine. So, we’ve already had a leak of SARS 1. And a couple of people who worked in that laboratory died in 2004. We have a precedent with the SARS virus.” 

“What about warnings? There were numerous warnings. UPMC Center for Health Security looked at ten nations including China. In 2016, they found inadequate training and inadequate safety personnel in China to secure biosecurity.”

“In 2017, there is an article in Nature where scientists say they are very concerned about a biosafety level 4 laboratory in China doing all of this controversial research. We don’t feel they have the experience or the expertise to do that.”

“In 2018, we have the cables from the U.S. State Department saying – we are in this lab in China and we are very concerned that they are not taking appropriate precautions. And we are hoping that the United States government is coming to help them because this could be a very bad result. That was reported on by Josh Rogin in the Washington Post. You can read these cables.”

“In 2019, the Global Health Security Index for the very first time looks at biosecurity for 195 nations. No one has ever done anything that comprehensive. They found that China was not even in the top fifty of the most biosecure countries.” 

“NBC reported that in October 2019 there was cell phone silence at the Wuhan lab. They were concerned that might have had something to do with an accident.”

“You had all of these warnings. You had precedent. Then you have a massive cover-up. Milton Leitenberg in the Bulletin of the Atomic Scientists goes over that cover-up in great detail in an article in June titled “Did the SARS-CoV-2 virus arise from a bat coronavirus research program in a Chinese laboratory? Very possibly.”

“Leitenberg goes over the cover-up in detail. China orders the virus destroyed. They punish those who were publishing stories about it. They refused to make any records from the labs available. They put out disinformation that it came from a U.S. military lab.” 

What about the so called batwoman?

“The Chinese virologist Shi Zhengli. She works at the Wuhan Institute of Virology. She says she didn’t sleep a wink for days, fearful that the virus came from her lab. But now she assures us that it didn’t come from her lab. She may be right or she may be wrong. I don’t know. It may have come from the other lab or from someone else working there. But she herself was so frightened about the possibility that her research had created this pandemic that she didn’t sleep a wink for days. That’s enough to say to me – that research should never happen.”

What you call gain of threat research was banned for a while, correct?

“That’s correct. Gain of function research is used for different kinds of research. If you were to be working with a plant and were trying to get the plant to fixate nitrogen better, that would be gain of function for that plant. There is nothing wrong with gain of function research. But to use the term as they do is dishonest. The term gain of function sounds innocuous. Gain of function – that doesn’t sound bad.”

You don’t want to ban gain of function research.

“I don’t want to ban gain of function research. I’m going to take away the double speak and call it what it is – gain of threat research on potential pandemic viruses. That’s what I want to ban. No one in the world should be doing gain of threat research on potentially pandemic viruses. It’s the definition of insanity.”

“In 2014, the Obama administration declared a moratorium on any federal funding of gain of threat research. The reason they did this was because two researchers – Ron Fouchier in the Netherlands and Yoshihiro Kawaoka in Wisconsin – were working on the H5N1 bird flu, which had a 60 percent mortality rate, but was not transmissible through the air. It killed a few hundred people, but because it was not transmissible, it didn’t go very far. But they decided they were going to try and turn it into a transmissible virus and publish their results.”

“With a 60 percent mortality rate, if that virus escaped, you have a potential 1.6 billion casualties.” 

Did they actually turn it into a transmissible virus?

“According to them, they did yes.”

What are the ethics of turning that into a transmissible virus?

“Marc Lipsitch, professor of epidemiology and director of the Center for Communicable Disease Dynamics at the Harvard School of Public Health said this ‘We have accepted principles, embodied in the Nuremberg Code, that say that biomedical experiments posing a risk to human subjects should only be undertaken if they provide benefits that sufficiently offset the risks – and if there are no other means of obtaining those benefits. Although these experiments don’t involve people directly, they do put human life and well-being at risk.’”

When Tedros Adhanom Ghebreyesus, the director-general of the World Health Organization (WHO), declared the Wuhan coronavirus a public health emergency of international concern on Thursday, he praised China for taking “unprecedented” steps to control the deadly virus. “I have never seen for myself this kind of mobilization,” he noted. “China is actually setting a new standard for outbreak response.”

The epidemic control efforts unfolding today in China—including placing some 100 million citizens on lockdown, shutting down a national holiday, building enormous quarantine hospitals in days’ time, and ramping up 24-hour manufacturing of medical equipment—are indeed gargantuan. It’s impossible to watch them without wondering, “What would we do? How would my government respond if this virus spread across my country?”

For the United States, the answers are especially worrying because the government has intentionally rendered itself incapable. In 2018, the Trump administration fired the government’s entire pandemic response chain of command, including the White House management infrastructure. In numerous phone calls and emails with key agencies across the U.S. government, the only consistent response I encountered was distressed confusion.

If the United States still has a clear chain of command for pandemic response, the White House urgently needs to clarify what it is
—not just for the public but for the government itself, which largely finds itself in the dark.

When Ebola broke out in West Africa in 2014, President Barack Obama recognized that responding to the outbreak overseas, while also protecting Americans at home, involved multiple U.S. government departments and agencies, none of which were speaking to one another. Basically, the U.S. pandemic infrastructure was an enormous orchestra full of talented, egotistical players, each jockeying for solos and fame, refusing to rehearse, and demanding higher salaries—all without a conductor. To bring order and harmony to the chaos, rein in the agency egos, and create a coherent multiagency response overseas and on the homefront, Obama anointed a former vice presidential staffer, Ronald Klain, as a sort of “epidemic czar” inside the White House, clearly stipulated the roles and budgets of various agencies, and placed incident commanders in charge in each Ebola-hit country and inside the United States. The orchestra may have still had its off-key instruments, but it played the same tune.

Building on the Ebola experience, the Obama administration set up a permanent epidemic monitoring and command group inside the White House National Security Council (NSC) and another in the Department of Homeland Security (DHS)—both of which followed the scientific and public health leads of the National Institutes of Health (NIH) and the Centers for Disease Control and Prevention (CDC) and the diplomatic advice of the State Department.

On the domestic front, the real business of assuring public health and safety is a local matter, executed by state, county, and city departments that operate under a mosaic of laws and regulations that vary jurisdiction by jurisdiction. Some massive cities, such as New York City or Boston, have large budgets, clear regulations, and epidemic experiences that have left deep benches of medical and public health talent. But much of the United States is less fortunate on the local level, struggling with underfunded agencies, understaffing, and no genuine epidemic experience. Large and small, America’s localities rely in times of public health crisis on the federal government.

Bureaucracy matters. Without it, there’s nothing to coherently manage an alphabet soup of agencies housed in departments ranging from Defense to Commerce, Homeland Security to Health and Human Services (HHS).

But that’s all gone now.

In the spring of 2018, the White House pushed Congress to cut funding for Obama-era disease security programs, proposing to eliminate $252 million in previously committed resources for rebuilding health systems in Ebola-ravaged Liberia, Sierra Leone, and Guinea. Under fire from both sides of the aisle, President Donald Trump dropped the proposal to eliminate Ebola funds a month later. But other White House efforts included reducing $15 billion in national health spending and cutting the global disease-fighting operational budgets of the CDC, NSC, DHS, and HHS. And the government’s $30 million Complex Crises Fund was eliminated.

In May 2018, Trump ordered the NSC’s entire global health security unit shut down, calling for reassignment of Rear Adm. Timothy Ziemer and dissolution of his team inside the agency. The month before, then-White House National Security Advisor John Bolton pressuredZiemer’s DHS counterpart, Tom Bossert, to resign along with his team. Neither the NSC nor DHS epidemic teams have been replaced. The global health section of the CDC was so drastically cut in 2018 that much of its staff was laid off and the number of countries it was working in was reduced from 49 to merely 10. Meanwhile, throughout 2018, the U.S. Agency for International Development and its director, Mark Green, came repeatedly under fire from both the White House and Secretary of State Mike Pompeo. And though Congress has so far managed to block Trump administration plans to cut the U.S. Public Health Service Commissioned Corps by 40 percent, the disease-fighting cadres have steadily eroded as retiring officers go unreplaced.

Public health advocates have been ringing alarm bells to no avail.

 Klain has been warning for two years that the United States was in grave danger should a pandemic emerge. In 2017 and 2018, the philanthropist billionaire Bill Gates met repeatedly with Bolton and his predecessor, H.R. McMaster, warning that ongoing cuts to the global health disease infrastructure would render the United States vulnerable to, as he put it, the “significant probability of a large and lethal modern-day pandemic occurring in our lifetimes.” And an independent, bipartisan panel formed by the Center for Strategic and International Studies concluded that lack of preparedness was so acute in the Trump administration that the “United States must either pay now and gain protection and security or wait for the next epidemic and pay a much greater price in human and economic costs.”

The next epidemic is now here; we’ll soon know the costs imposed by the Trump administration’s early negligence and present panic. On Jan. 29, Trump announced the creation of the President’s Coronavirus Task Force, an all-male group of a dozen advisors, five from the White House staff. Chaired by Secretary of Health and Human Services Alex Azar, the task force includes men from the CDC, State Department, DHS, the Office of Management and Budget, and the Transportation Department. It’s not clear how this task force will function or when it will even convene.

In the absence of a formal structure, the government has resorted to improvisation. In practical terms, the U.S. government’s public health effort is led by Daniel Jernigan, the incident commander for the Wuhan coronavirus response at the CDC. Jernigan is responsible for convening meetings of the nation’s state health commissioners and briefing CDC Director Robert Redfield and his boss, Azar. Meanwhile, state-level health leaders told me that they have been sharing information with one another and deciding how best to prepare their medical and public health workers without waiting for instructions from federal leadership. The most important federal program for  local medical worker and hospital epidemic training, however, will run out of money in May, as Congress has failed to vote on its funding. The HHS Office of the Assistant Secretary for Preparedness and Response (ASPR) is the bulwark between hospitals and health departments versus pandemic threats; last year HHS requested $2.58 billion, but Congress did not act.

On Thursday, the CDC confirmed the first human-to-human spread of the Wuhan coronavirus inside the United States, between a husband and wife in Chicago. While the wife acquired her infection traveling in China, she passed the virus to her husband on return to the United States. Though only six Wuhan coronavirus cases have been confirmed in the United States, with no deaths, Nancy Messonnier of the CDC told reporters on Thursday: “Moving forward, we can expect to see more cases, and more cases mean the potential for more person-to-person spread.”

As the number of coronavirus cases increases, Americans are growing more fearful, which is creating new problems that the government is leaving unaddressed.

Surveying the largest drug store chains in New York City on Wednesday, I found that all were sold out of medical face masks and latex gloves, as is Amazon. Searching online for protective masks reveals that dozens of products intended for use to block dust and particles far larger than viruses are garnering brisk sales—and none available that can actually prevent viral exposure. The surge in mask and glove sales to worried citizens all over the world needs refereeing. Bona fide anti-viral masks should be prioritized to front-line medical and public health staff, and the populace shouldn’t be misled into purchasing and wearing products that offer no genuine protection.

Countering misinformation, conspiracy theories, rumormongering, and discriminatory behavior against people believed to be disease spreaders requires thoughtful communication from leadership at the highest levels of government. None is in evidence. Instead, Commerce Secretary Wilbur Ross appeared on Fox Business on Thursday to fan the flames of fear for the sake of hypothetical business opportunities. “It does give businesses yet another thing to consider when they go through their review of their supply chain,” Ross said. “It’s another risk factor that people need to take into account. So, I think it will help accelerate the return of jobs to North America, some to the U.S., probably some to Mexico as well.” Meanwhile, Trump, asked at the recent World Economic Forum gathering in Davos, Switzerland how he intended to respond to the epidemic, said the situation was under control and a world away from the United States.

In a statement released this week, Pompeo sought to calm Americans, saying, “People should know that there are enormous efforts underway by the United States government to make sure that we do everything we can to protect the American people and to reduce the risk all around the globe.” But late Thursday night, the secretary—in clear defiance of WHO’s admonishment against restricting travel to and from China--issued an advisory saying, “Those currently in China should consider departing.”

In recent days, a handful of policy leaders have been shifted from government positions focused on weapons of mass destruction and bioterrorism to the slowly emerging epidemic response infrastructure, such as Matthew Pottinger, Philip Ferro, and David Wade on the NSC and the bioterrorism expert Anthony Ruggiero. It’s not at all clear how they would handle an explosion of coronavirus cases, were such a dreadful thing to occur in the United States. “The full weight of the US Government is working on this,” a senior administration official toldCNN on Tuesday. “As with any interagency effort of this scale, the National Security Council works closely with the whole of government to ensure a coordinated and unified effort.”

The last time the U.S. government and its many local and state counterparts faced an explosive pandemic on American soil was 2009, with the spread of H1N1, or swine flu. The then-new Obama administration was still filling key positions across the executive branch when the epidemic emerged that spring, and it struggled to set the proper tone in reaction to what turned out to be an exceptionally contagious, but not unusually virulent, form of influenza. The challenge revealed enormous gaps in America’s ability to swiftly manufacture vaccines, stock-outs of face masks and vital hospital supplies, and serious difficulty in keeping ahead of outright lies, conspiracy theories, and rumormongering on cable TV and social media. The much more deadly pandemic test came in 1981, with the arrival of HIV: It did not go well, as history has well established, because homophobia was so pervasive in the country and within government that gay men, rather than the virus killing them, were treated as a national scourge.

Since the great influenza pandemic of 1918, the United States has been spared terrifying epidemics. Americans now are epidemic voyeurs. They watch YouTube videos of China’s struggles. They see the government attack its epidemic by building a 1,000-bed quarantine hospital in a single week, lock down cities larger than New York or Los Angeles, ramp up 24/7 manufacture of face masks and protective gear, deploy its armed forces medical corps to treat ailing citizens, send enormous convoys of food and supplies to anxious citizens of Wuhan, and release terrifying, growing tallies daily of its swelling patient populations. They look in horror at panicked lines of masked people waiting to learn if their fevers are caused by the deadly disease, at bodies lying on cold floors in overcrowded hospitals, and at people crying out from behind their masks for help. And they ask, “What would the United States do? What would the White House do?” The answers are not reassuring.

Before Virus Outbreak, a Cascade of Warnings Went Unheeded

​Government exercises, including one last year, made clear that the U.S. was not ready for a pandemic like the coronavirus. But little was done.

​WASHINGTON — The outbreak of the respiratory virus began in China and was quickly spread around the world by air travelers, who ran high fevers. In the United States, it was first detected in Chicago, and 47 days later, the World Health Organization declared a pandemic. By then it was too late: 110 million Americans were expected to become ill, leading to 7.7 million hospitalized and 586,000 dead.

That scenario, code-named “Crimson Contagion” and imagining an influenza pandemic, was simulated by the Trump administration’s Department of Health and Human Services in a series of exercises that ran from last January to August.

The simulation’s sobering results — contained in a draft report dated October 2019 that has not previously been reported — drove home just how underfunded, underprepared and uncoordinated the federal government would be for a life-or-death battle with a virus for which no treatment existed.

The draft report, marked “not to be disclosed,” laid out in stark detail repeated cases of “confusion” in the exercise. Federal agencies jockeyed over who was in charge. State officials and hospitals struggled to figure out what kind of equipment was stockpiled or available. Cities and states went their own ways on school closings.

Many of the potentially deadly consequences of a failure to address the shortcomings are now playing out in all-too-real fashion across the country. And it was hardly the first warning for the nation’s leaders. Three times over the past four years the U.S. government, across two administrations, had grappled in depth with what a pandemic would look like, identifying likely shortcomings and in some cases recommending specific action.

In 2016, the Obama administration produced a comprehensive report on the lessons learned by the government from battling Ebola. In January 2017, outgoing Obama administration officials ran an extensive exercise on responding to a pandemic for incoming senior officials of the Trump administration.

The full story of the Trump administration’s response to the coronavirus is still playing out. Government officials, health professionals, journalists and historians will spend years looking back on the muddled messages and missed opportunities of the past three months, as President Trump moved from dismissing the coronavirus as a few cases that would soon be “under control” to his revisionist announcement on Monday that he had known all along that a pandemic was on the way.

What the scenario makes clear, however, is that his own administration had already modeled a similar pandemic and understood its potential trajectory.

The White House defended its record, saying it responded to the 2019 exercise with an executive order to improve the availability and quality of flu vaccines, and that it moved early this year to increase funding for the Department of Health and Human Services’ program that focuses on global pandemic threats.

“Any suggestion that President Trump did not take the threat of COVID-19 seriously is false,” said Judd Deere, a White House spokesman.

But officials have declined to say why the administration was so slow to roll out broad testing or to move faster, as the simulations all indicated it should, to urge social distancing and school closings.

Asked at his news briefing on Thursday about the government’s preparedness, Mr. Trump responded: “Nobody knew there would be a pandemic or epidemic of this proportion. Nobody has ever seen anything like this before.”

The work done over the past five years, however, demonstrates that the government had considerable knowledge about the risks of a pandemic and accurately predicted the very types of problems Mr. Trump is now scrambling belatedly to address.

Crimson Contagion, the exercise conducted last year in Washington and 12 states including New York and Illinois, showed that federal agencies under Mr. Trump continued the Obama-era effort to think ahead about a pandemic.

But the planning and thinking happened many layers down in the bureaucracy. The knowledge and sense of urgency about the peril appear never to have gotten sufficient attention at the highest level of the executive branch or from Congress, leaving the nation with funding shortfalls, equipment shortages and disorganization within and among various branches and levels of government.

The October 2019 report in particular documents that officials at the Departments of Homeland Security and Health and Human Services, and even at the White House’s National Security Council, were aware of the potential for a respiratory virus outbreak originating in China to spread quickly to the United States and overwhelm the nation.

“Nobody ever thought of numbers like this,’’ Mr. Trump said on Wednesday, at a news conference.

In fact, they had.

As early as the George W. Bush administration, homeland security and health officials focused on big gaps in the American response to a biological attacks and the growing risk of pandemics. The first test came in April 2009, just a few months after the start of President Barack Obama’s first term. A 10-year-old California girl was diagnosed with a contagious disease that would be called swine flu or H1N1, the first flu pandemic in more than 40 years.

The Centers for Disease Control and Prevention estimates that ultimately there were about 60.8 million cases in the United States, along with 274,304 hospitalizations and 12,469 deaths associated with H1N1.

The virus turned out to be less deadly than first expected. But it was a warning shot that officials in the Obama administration said they took seriously, kicking off a planning effort that escalated in early 2014, with the outbreak of Ebola in West Africa and ensuing fear that it could spread to the United States.

Ebola was less contagious than the flu, but far more deadly. It killed 11,000 people in Africa. But it could have been far worse. The United States sent nearly 3,000 troops to Africa to help keep the disease from spreading. While the containment effort was considered a success, inside the White House, officials sensed that the United States had gotten lucky — and that the response had revealed gaps in preparedness.

Christopher Kirchhoff, a national security aide who moved from the Pentagon to the White House to deal with the Ebola crisis, was given the job of putting together a “lessons learned” report, with input from across the government.

The weaknesses Mr. Kirchhoff identified were early warning signals of what has unfolded in the past three months.

His report concluded that the United States assumed more ability on the part of the World Health Organization than the agency actually had.

The United States had its own issues. There was no airplane in the U.S. fleet capable of evacuating an American doctor who was infected while treating patients in Liberia. The Pentagon was largely unprepared for the intervention that Mr. Obama ordered.

While the United States rapidly developed a way to screen air passengers coming into the country — borrowing from intelligence tools developed after the Sept. 11, 2001, attacks to track possible terrorists — Mr. Kirchhoff found deficiencies in even measuring how fast the virus was spreading.

On the plus side, the Obama White House had created an Ebola Task Force, run by Ron Klain, Vice President Joseph R. Biden Jr.’s former chief of staff, before a single case emerged in the United States. Congress allocated $5.4 billion in emergency funding to pay for Ebola treatment and prevention efforts in the United States and West Africa.

The money helped fund a little-known agency inside the Department of Health and Human Services in charge of preparing for future contagious disease outbreaks, the same office that in 2019 ran the Crimson Contagion exercise and other similar events in the years since.

After a man named Thomas Duncan, a Liberian citizen, became the first person given a diagnosis of Ebola on American territory in September 2014, errors resulted in the infection of two nurses and fear of a wider spread in the United States. (Mr. Duncan died, but the two nurses recovered.)

What is striking in reading Mr. Kirchhoff’s account today, however, is how few of the major faults he found in the American response resulted in action — even though the report was filled with department-by-department recommendations.

There were deficiencies “in personal protective equipment use, disinfection” and “social services for those placed under quarantine.”

There was confusion over travel restrictions, and the need “for a smoother sliding scale of escalation of government response, from local authorities acting on their own to local authorities acting with some federal assistance” to the full activation of the federal government.

The report concluded that “a minimum planning benchmark might be an epidemic an order of magnitude or two more difficult than that presented by the outbreak of Ebola in West Africa, with much more significant domestic spread.”

But one big change did come out of the study: The creation of a dedicated office at the National Security Council to coordinate responses and raise the alarm early.

“What I learned most is that we had to stand up a global biosecurity and health directorate, and get it enshrined for the next administration,” said Lisa Monaco, Mr. Obama’s homeland security adviser.

After Mr. Trump’s election, Ms. Monaco arranged an extensive exercise for high-level incoming officials — including Rex W. Tillerson, the nominee for secretary of state; John F. Kelly, designated to become homeland security secretary; and Rick Perry, who would become energy secretary — gaming out the response to a deadly flu outbreak.

She asked Tom Bossert, who was preparing to come in as Mr. Trump’s homeland security adviser, to run the event alongside her.

“We modeled a new strain of flu in the exercise precisely because it’s so communicable,” Ms. Monaco said. “There is no vaccine, and you would get issues like nursing homes being particularly vulnerable, shortages of ventilators.”

Ms. Monaco was impressed by how seriously Mr. Bossert, her successor, appeared to take the threat, as did many of the 30 or so Trump team members who participated in the exercise, details of which were reported by Politico.

But by the time the current crisis hit, almost all of the leaders at the table — Mr. Tillerson, Mr. Kelly and Mr. Perry among them — had been fired or moved on.

In 2018, Mr. Trump’s national security adviser at the time, John R. Bolton, ousted Mr. Bossert and eliminated the National Security Council directorate, folding it into an office dedicated to weapons of mass destruction in what Trump officials called a logical consolidation.

Asked about that shift on March 13, Mr. Trump told a reporter that it was “a nasty question,” before adding: “I don’t know anything about it.” Writing on Twitter the next day, Mr. Bolton lashed out at critics who said the shift had reflected disregard for pandemic threats.

“Claims that streamlining NSC structures impaired our nation’s bio defense are false,” Mr. Bolton tweeted. “Global health remained a top NSC priority.”

In a statement, the National Security Council said it “has directors and staff whose full-time job it is to monitor, plan for, and respond to pandemics, including an infectious disease epidemiologist and a virologist.”

But in testimony to Congress last week, Dr. Anthony S. Fauci, the director of the National Institute of Allergy and Infectious Diseases, suggested that ending the stand-alone directorate was ill-advised. “It would be nice if the office was still there,” he said.

On Feb. 10, nearly three weeks after the first coronavirus case was diagnosed in the United States, Mr. Trump submitted a 2021 budget proposal that called for a $693.3 million reduction in funding for the C.D.C., or about 9 percent, although there was a modest increase for the division that combats global pandemics.

The Crimson Contagion planning exercise run last year by the Department of Health and Human Services involved officials from 12 states and at least a dozen federal agencies. They included the Pentagon, the Department of Veterans Affairs and the National Security Council. Groups like the American Red Cross and American Nurses Association were invited to join, as were health insurance companies and major hospitals like the Mayo Clinic.

The war game-like exercise was overseen by Robert P. Kadlec, a former Air Force physician who has spent decades focused on biodefense issues. After stints on the Bush administration’s Homeland Security Council and the staff of the Senate Intelligence Committee, he was appointed assistant secretary of Health and Human Services for Preparedness and Response.

“He recognized early that we have a big problem and we needed much bigger budgets to prepare,” said Richard Danzig, the secretary of the Navy in the Clinton administration, who had worked with Mr. Kadlec.

The exercise played out in four separate stages, starting in January 2019.

The events were supposedly unspooling in real time — with the worst-case scenario underway as of Aug. 13, 2019 — when, according to the script, 12,100 cases had already been reported in the United States, with the largest number in Chicago, which had 1,400.

The fictional outbreak involved a pandemic flu, which the Department of Health and Human Services says was “very different than the novel coronavirus.” The staged outbreak had started when a group of 35 tourists visiting China were infected and then flew home to Australia, Kuwait, Malaysia, Thailand, Britain and Spain, as well as to the United States, with some developing respiratory symptoms and fevers en route.

A 52-year-old man from Chicago, who was on the tour, had “low energy and a dry cough” upon his return home. His 17-year-old son on that same day went out to a large public event in Chicago, and the chain of illnesses in the United States started.

Many of the moments during the tabletop exercise are now chillingly familiar.

In the fictional pandemic, as the virus spread quickly across the United States, the C.D.C. issued guidelines for social distancing, and many employees were told to work from home.

But federal and state officials struggled to identify which employees were essential and what equipment was needed to effectively work from home.

There also was confusion over how to handle school children. The C.D.C. recommended that states delay school openings — the exercise took place toward the end of the summer. But some school districts decided to go ahead with the start of school while others followed the federal advice, causing the same types of confusion and discrepancies that have marked the response to the coronavirus.

The exercise from last year then went on to predict how the situation on the ground in the United States would worsen as the weeks passed.

Confusion emerged as state governments began to turn in large numbers to Washington for help to address shortages of antiviral medications, personal protective equipment and ventilators. Then states started to submit requests to different branches of the federal government, leading to bureaucratic chaos.

Friction also emerged between the Federal Emergency Management Agency, which is traditionally in charge of disaster response, and the Department of Health and Human Services, another scenario playing out now.

But the problems were larger than bureaucratic snags. The United States, the organizers realized, did not have the means to quickly manufacture more essential medical equipment, supplies or medicines, including antiviral medications, needles, syringes, N95 respirators and ventilators, the agency concluded.

Congress was briefed in December on some of these findings, including the inability to quickly replenish certain medical supplies, given that much of the product comes from overseas.

These concerns turned more urgent at a hearing last Thursday on Capitol Hill, as lawmakers peppered officials with the Department of Health and Human Services with questions that sounded almost as if they had read the script from the fictional exercise, reflecting the shortage of respirators and protective gear.

Senator Mitt Romney, Republican of Utah, said last week that he blamed Congress and prior administrations for not increasing stockpiles of this type of equipment.

“That is an area we ought to consider making an investment in,” he added, making a point, apparently unknown to him, that the administration’s own simulation had made clear five months earlier.

The 2019 pandemic exercise pointed to areas of concern for state and federal officials

In Chicago, back in August 2019, federal agencies conducted a mock drill to see how the nation would handle a pandemic, specifically a deadly global outbreak with no known cure. The exercise pointed to a number of national shortcomings, including an insufficient amount of medical supplies.

It is called the 'Crimson Contagion 2019 Functional Exercise' and is marked not for distribution.  The New York Times was the first to report and publish it.

This was an exercise about the flu, not the coronavirus, but the document points to specific problematic areas for a hypothetical outbreak that, prophetically, begins in China and lands in Chicago.

On August 13, in Illinois and 11 states from Arizona to Connecticut, federal, state and local officials began the four-day exercise.

The Scenario: 

A large-scale outbreak of novel influenza begins in China and quickly spreads, first detected in Chicago in the U.S. and grows to pandemic proportion by human-to-human contact.  
Stockpiled vaccines, per the exercise, are not a direct match to contain the virus.

Involved in the national test were:

• 19 federal agencies

• 12 states

• 74 local health departments

• 87 hospitals

​According to the report, officials at the National Security Council in the White House were briefed during the exercise.

Among the key findings:

• Insufficient federal funding sources for a severe influenza pandemic

• Confusion on how to apply the Defense Production Act

• The current medical supply chain and production capacity could not meet the demand

• Global manufacturing would be unable to meet the domestic demand for personal protective equipment and ancillary supplies

Dr. Allison Arwady was intimately involved in the exercise and ratcheted up Chicago’s preparedness as a result. She had no comment on whether the feds took the same actions the city did. 

Mayor Lori Lightfoot was blunt in a telephone briefing with reporters.

“It is clear to me the federal government will not help us,” she said. “They are not the cavalry.”
The Chicago Department of Public Health, the Office of Emergency Management and Communications, as well as the state Health Department and Emergency Management Agency, took part in the exercise.

Though both the report and Dr. Arwady commend many federal agencies for working together and devising response strategies, the pandemic exercise predicted dire consequences--110 million illnesses, 7.7 million hospitalizations and 586,000 deaths, all in the U.S. in the absence of a coordinated national response.

Exclusive: U.S. axed CDC expert job in China months before virus outbreak

WASHINGTON (Reuters) - Several months before the coronavirus pandemic began, the Trump administration eliminated a key American public health position in Beijing intended to help detect disease outbreaks in China, Reuters has learned.

The American disease expert, a medical epidemiologist embedded in China’s disease control agency, left her post in July, according to four sources with knowledge of the issue. The first cases of the new coronavirus may have emerged as early as November, and as cases exploded, the Trump administration in February chastised China for censoring information about the outbreak and keeping U.S. experts from entering the country to help. 

“It was heartbreaking to watch,” said Bao-Ping Zhu, a Chinese American who served in that role, which was funded by the U.S. Centers for Disease Control and Prevention, between 2007 and 2011. “If someone had been there, public health officials and governments across the world could have moved much faster.” 

Zhu and the other sources said the American expert, Dr. Linda Quick, was a trainer of Chinese field epidemiologists who were deployed to the epicenter of outbreaks to help track, investigate and contain diseases. 

As an American CDC employee, they said, Quick was in an ideal position to be the eyes and ears on the ground for the United States and other countries on the coronavirus outbreak, and might have alerted them to the growing threat weeks earlier. 

No other foreign disease experts were embedded to lead the program after Quick left in July, according to the sources. Zhu said an embedded expert can often get word of outbreaks early, after forming close relationships with Chinese counterparts.

At the same time, the sheer breadth of complications linked to COVID-19 is mind-boggling. In late April, Akrami collaborated with Body Politic, a group of COVID-19 survivors, to survey more than 600 who still had symptoms after 2 weeks. She logged 62 different symptoms and is now readying the findings for publication and developing a second survey to capture longer term ailments. “Even though it’s one virus, it can cause all different kinds of diseases in people,” says Akiko Iwasaki, an immunologist at Yale University who is studying lingering effects on the immune system.

Opinion: The ‘excess deaths’ tally in the U.S. is 204,691 in 7 months — so COVID-19 deaths might be undercounted

The number of excess deaths exceeds the number attributable to COVID-19 by more than 16,000 people

The number of deaths in the United States through July 2020 is 8% to 12% higher than it would have been if the coronavirus pandemic had never happened. That’s at least 164,937 deaths above the number expected for the first seven months of the year — 16,183 more than the number attributed to COVID-19 thus far for that period — and it could be as high as 204,691.

Florida fired its coronavirus data scientist. Now she’s publishing the statistics on her own.

Tension built for days between Florida Department of Health supervisors and the department’s geographic information systems manager before officials showed her the door, she said, permanently pulling her off the coronavirus dashboard that she had operated for weeks.

Managers had wanted Rebekah Jones to make certain changes to the public-facing portal, she said. Jones had objected to — and sometimes refused to comply with — what she considered unethical requests. She said the department offered to let her resign. Jones declined.

White House coronavirus response coordinator Deborah Birx praised Florida’s official coronavirus dashboard in April as a beacon of transparency.

But Jones has said that the site undercounts the state’s infection total and   the number of people tested — with the official numbers bolstering the decision to start loosening restrictions on the economy in early May, when the state had not met federal guidelines for reopening.

Gov. Ron DeSantis severed all state ties to Quest Diagnostics after learning of a massive data dump of nearly 75,000 unreported coronavirus tests dating to April.

The state reported that without the Quest results, the positivity rate for new cases totaling 3,773 for Aug. 31 is 5.9%. With the Quest results, that total rises to 7,643 cases with a positivity rate of 6.8%. The state said the majority of the Quest results were for tests more than two weeks old, and some as old as five months.

DeSantis stressed that “the individuals who tested positive at that time were notified by the lab, which is obviously very important. The lab just did not submit those results to the state. ... That’s not acceptable.”

He said the biggest impact of the mistake was that testing numbers play a huge role in determining public strategy.

“These labs know that their results are being used by people to determine the course of certain policies,” he said. “Someone will say, ’the positive rate has gone up, maybe we can’t do in-person schooling.’ ... That actually has an effect. And so I think it’s really disquieting to see that happen.”
