The coronavirus is mutating — does it matter? Different SARS-CoV-2 strains haven’t yet had a major impact on the course of the pandemic, but they might in future. 08 SEPTEMBER 2020, CORRECTION 16 SEPTEMBER 2020 When COVID-19 spread around the globe this year, David Montefiori wondered how the deadly virus behind the pandemic might be changing as it passed from person to person. Montefiori is a virologist who has spent much of his career studying how chance mutations in HIV help it to evade the immune system. The same thing might happen with SARS-CoV-2, he thought. In March, Montefiori, who directs an AIDS-vaccine research laboratory at Duke University in Durham, North Carolina, contacted Bette Korber, an expert in HIV evolution and a long-time collaborator. Korber, a computational biologist at the Los Alamos National Laboratory (LANL) in New Mexico, had already started scouring thousands of coronavirus genetic sequences for mutations that might have changed the virus’s properties as it made its way around the world. Compared with HIV, SARS-CoV-2 is changing much more slowly as it spreads. But one mutation stood out to Korber. It was in the gene encoding the spike protein, which helps virus particles to penetrate cells. Korber saw the mutation appearing again and again in samples from people with COVID-19. At the 614th amino-acid position of the spike protein, the amino acid aspartate (D, in biochemical shorthand) was regularly being replaced by glycine (G) because of a copying fault that altered a single nucleotide in the virus’s 29,903-letter RNA code. Virologists were calling it the D614G mutation. In April, Korber, Montefiori and others warned in a preprint posted to the bioRxiv server that “D614G is increasing in frequency at an alarming rate”1. It had rapidly become the dominant SARS-CoV-2 lineage in Europe and had then taken hold in the United States, Canada and Australia. D614G represented a “more transmissible form of SARS-CoV-2”, the paper declared, one that had emerged as a product of natural selection. These assertions dismayed many scientists. It wasn’t clear that the D614G viral lineage was more transmissible, or that its rise indicated anything unusual, they said. But alarm spread fast across the media. Although many news stories included researchers’ caveats, some headlines declared that the virus was mutating to become more dangerous. In retrospect, Montefiori says he and his colleagues regret describing the variant’s rise as “alarming”. The word was scrubbed from the peer-reviewed version of the paper, published in Cell in July2. The work sparked a frenzy of interest in D614G. Even those who were sceptical that the mutation had changed the virus’s properties agreed that it was intriguing, because of its meteoric rise and ubiquity. For months, that lineage has been found in almost all sequenced samples of SARS-CoV-2 (see ‘Global spread’). “This variant now is the pandemic. As a result, its properties matter,” wrote Nathan Grubaugh, a viral epidemiologist at the Yale School of Public Health in New Haven, Connecticut, and two colleagues in a Cell essay on Korber and Montefiori’s findings3.  So far, the upshot of this work is less clear than Montefiori and Korber’s preprint suggested. Some experiments suggest that viruses carrying the variant infect cells more easily. Other work has revealed possible good news: the variant might mean that vaccines can target SARS-CoV-2 more easily. But many scientists say there remains no solid proof that D614G has a significant effect on the spread of the virus, or that a process of natural selection explains its rise. “The jury’s out,” says Timothy Sheahan, a coronavirologist at the University of North Carolina at Chapel Hill. “This mutation might mean something, or it might not.” Researchers still have more questions than answers about coronavirus mutations, and no one has yet found any change in SARS-CoV-2 that should raise public-health concerns, Sheahan, Grubaugh and others say. But studying mutations in detail could be important for controlling the pandemic. It might also help to pre-empt the most worrying of mutations: those that could help the virus to evade immune systems, vaccines or antibody therapies. Slow changeSoon after SARS-CoV-2 was detected in China, researchers began analysing viral samples and posting the genetic codes online. Mutations — most of them single-letter alterations between viruses from different people — allowed researchers to track the spread by linking closely related viruses, and to estimate when SARS-CoV-2 started infecting humans. Viruses that encode their genome in RNA, such as SARS-CoV-2, HIV and influenza, tend to pick up mutations quickly as they are copied inside their hosts, because enzymes that copy RNA are prone to making errors. After the severe acute respiratory syndrome (SARS) virus began circulating in humans, for instance, it developed a kind of mutation called a deletion that might have slowed its spread4. But sequencing data suggest that coronaviruses change more slowly than most other RNA viruses, probably because of a ‘proofreading’ enzyme that corrects potentially fatal copying mistakes. A typical SARS-CoV-2 virus accumulates only two single-letter mutations per month in its genome — a rate of change about half that of influenza and one-quarter that of HIV, says Emma Hodcroft, a molecular epidemiologist at the University of Basel, Switzerland. Other genome data have emphasized this stability — more than 90,000 isolates have been sequenced and made public (see www.gisaid.org). Two SARS-CoV-2 viruses collected from anywhere in the world differ by an average of just 10 RNA letters out of 29,903, says Lucy Van Dorp, a computational geneticist at University College London, who is tracking the differences for signs that they confer an evolutionary advantage. Despite the virus’s sluggish mutation rate, researchers have catalogued more than 12,000 mutations in SARS-CoV-2 genomes [AS OF SEPTEMBER 2020!]. But scientists can spot mutations faster than they can make sense of them. Many mutations will have no consequence for the virus’s ability to spread or cause disease, because they do not alter the shape of a protein, whereas those mutations that do change proteins are more likely to harm the virus than improve it (see ‘A catalogue of coronavirus mutations’). “It’s much easier to break something than it is to fix it,” says Hodcroft, who is part of Nextstrain, an effort to analyse SARS-CoV-2 genomes in real time.  Many researchers suspect that if a mutation did help the virus to spread faster, it probably happened earlier, when the virus first jumped into humans or acquired the ability to move efficiently from one person to another. At a time when nearly everyone on the planet is susceptible, there is likely to be little evolutionary pressure on the virus to spread better, so even potentially beneficial mutations might not flourish. “As far as the virus is concerned, every single person that it comes to is a good piece of meat,” says William Hanage, an epidemiologist at the Harvard T. H. Chan School of Public Health in Boston, Massachusetts. “There’s no selection to be doing it any better.” Faster spread?When Korber saw the rapid spread of D614G, she thought she might have found an example of meaningful natural selection. The mutation caught her eye because of its position in the spike protein, which is a major target for ‘neutralizing’ antibodies that bind to the virus and render it non-infectious. And viruses with the mutation were also rising in frequency in more than one part of the world. D614G was first spotted in viruses collected in China and Germany in late January; most scientists suspect the mutation arose in China. It’s now almost always accompanied by three mutations in other parts of the SARS-CoV-2 genome — possible evidence that most D614G viruses share a common ancestor. D614G’s rapid rise in Europe drew Korber’s attention. Before March — when much of the continent went into lockdown — both unmutated ‘D’ viruses and mutated ‘G’ viruses were present, with D viruses prevalent in most of the western European countries that geneticists sampled at the time. In March, G viruses rose in frequency across the continent, and by April they were dominant, reported Korber, Montefiori and their team1,2. But natural selection in favour of G viruses isn’t the only, or even the most likely, explanation for this pattern. The European dominance of G variants could be simply down to chance — if, for instance, the mutation happened to be slightly more common in the viruses that arrived in Europe. A small number of individuals seem to be responsible for most of the virus’s spread, and an early, chance tilt in favour of G viruses could explain the lineage’s apparent takeover now. Such ‘founder effects’ are common in viruses, especially when they spread unchecked, as SARS-CoV-2 did in much of Europe until mid- to late March. Korber and her colleagues tried to rule out a founder effect, by showing in their April preprint1 that D614G rose to dominance quickly in Canada, Australia and parts of the United States (an exception was Iceland, where G viruses present early in its outbreak were overtaken by D viruses). Analysing hospitalization data from Sheffield, UK, the team found no evidence that viruses carrying the mutation made people any sicker. But those infected with G viruses seemed to have slightly higher levels of viral RNA in their noses and mouths than did those with D viruses. Many scientists weren’t convinced that D614G’s rise was remarkable — or all that relevant to the pandemic. “I thought that preprint was incredibly premature,” says Sheahan. Montefiori says his and Korber’s perspective on D614G was shaped by their work on HIV, which has found that even seemingly insignificant mutations can have a profound effect on how the immune system recognizes that virus. “We were alarmed by it, and we need to see if it’s having an effect on vaccines,” he says. Rush of lab studiesTo examine further whether D614G made the virus more transmissible, Montefiori gauged its effects under laboratory conditions. He couldn’t study the natural SARS-CoV-2 virus in his lab, because of the biosafety containment required. So he studied a genetically modified form of HIV that used the SARS-CoV-2 spike protein to infect cells. Such ‘pseudovirus’ particles are a workhorse of virology labs: they enable the safe study of deadly pathogens such as the Ebola virus, and they make it easy to test the effects of mutations. The experiment uses a pseudovirus that doesn’t require high levels of biosafety containment.Credit: Jeremy Luban The first team to report pseudovirus experiments on D614G, in June, was led by Hyeryun Choe and Michael Farzan, virologists at the Scripps Research Institute in La Jolla, California5. Several other teams have posted similar studies on bioRxiv (Montefiori’s experiments, and those of another collaborator, appeared in the Cell paper2). The teams used different pseudovirus systems and tested them on various kinds of cell, but the experiments pointed to the same conclusion: viruses carrying the G mutation infected cells much more ably than did D viruses — up to ten times more efficiently, in some cases. In laboratory tests, “all of us agree that D to G is making the particles more infectious”, says Jeremy Luban, a virologist at the University of Massachusetts Medical School in Worcester. But these studies come with many caveats — and their relevance to human infections is unclear. “What’s irritating are people taking their results in very controlled settings, and saying this means something for the pandemic. That, we are so far away from knowing,” says Grubaugh. The pseudoviruses carry only the coronavirus spike protein, in most cases, and so the experiments measure only the ability of these particles to enter cells, not aspects of their effects inside cells, let alone on an organism. They also lack the other three mutations that almost all D614G viruses carry. “The bottom line is, they’re not the virus,” says Luban. Some labs are now working with infectious SARS-CoV-2 viruses that differ by only the single amino acid. These are tested in laboratory cultures of human lung and airway cells, and in lab animals such as ferrets and hamsters. For labs with the experience and the biosafety capabilities to manipulate viruses, “this is like bread-and-butter kind of work”, says Sheahan. The first of those studies, led by researchers at the University of Texas Medical Branch in Galveston, was reported in a 2 September preprint6. It found that viruses with the mutation were more infectious than were D viruses in a human lung cell line and in airway tissues, and that mutated viruses were present at greater levels in the upper airways of infected hamsters6. Coronavirus reinfections: three questions scientists are asking Even these experiments might not offer absolute clarity. Some studies show that certain mutations to the spike protein in the Middle East respiratory syndrome (MERS) virus can cause more-severe disease in mice — yet other mutations in the protein show very little effect in people or in camels, the likely reservoir for human MERS infections, says Stanley Perlman, a coronavirologist at the University of Iowa in Iowa City. The clearest sign that D614G has an effect on the spread of SARS-CoV-2 in humans comes from an ambitious UK effort called the COVID-19 Genomics UK Consortium, which has analysed genomes of around 25,000 viral samples. From these data, researchers have identified more than 1,300 instances in which a virus entered the United Kingdom and spread, including examples of D- and G-type viruses. A team led by Andrew Rambaut, an evolutionary biologist at the University of Edinburgh, UK, epidemiologist Erik Volz, at Imperial College London, and biologist Thomas Connor at Cardiff University, studied the UK spread of 62 COVID-19 clusters seeded by D viruses and 245 by G viruses7. The researchers found no clinical differences in people infected with either virus. However, G viruses tended to transmit slightly faster than lineages that didn’t carry the change, and formed larger clusters of infections. Their estimates of the difference in transmission rates hover around 20%, Volz says, but the true value could be a bit higher or lower. “There’s not a large effect in absolute terms,” says Rambaut. It’s possible that D614G is an adaptation that helps the virus to infect cells or compete with viruses that don’t carry the change, while altering little about how SARS-CoV-2 spreads between people or through a population, Rambaut says. “This might be a bona fide adaptation to humans or some human cells,” agrees Grubaugh, “but that doesn’t mean anything changes. An adaptation doesn’t have to make it more transmissible.” Grubaugh thinks that D614G has received too much attention from scientists, in part because of the high-profile papers it has garnered. “Scientists have this crazy fascination with these mutations,” he says. But he also sees D614G as a way to learn about a virus that doesn’t have much in the way of genetic diversity. “The virologist in me looks at these things and says it would be a lot of fun to study,” he says. “It creates this whole rabbit hole of different things you can go into.” He’ll have company. Intense study of D614G should help to explain how SARS-CoV-2 fuses with cells, says Luban — a process that might be blocked by drugs or targeted by a vaccine. In an updated version of their pseudovirus experiments posted on bioRxiv on 16 July8, Luban’s team used cryo-electron microscopy to analyse the structure of spike proteins bearing the D614G change. The spike protein is comprised of three identical peptides in an ‘open’ or ‘closed’ orientation. Previous research has suggested that at least two of the three peptides need to be open for the viral particle to fuse with the cell membrane9, and Luban’s team found that viruses carrying the G spike variant were much more likely to be in this state (see ‘The mutation that loosens the spike protein’). Computational modelling work by Montefiori and Korber, led by Korber’s LANL colleague Sandrasegaram Gnanakaran, came to the same conclusion10. “It looks like this molecular machine is primed to go in a way that D is not,” Luban says.  No escape from antibodies — yetMost available evidence suggests that D614G doesn’t stop the immune system’s neutralizing antibodies from recognizing SARS-CoV-2, as Montefiori had worried. That might be because the mutation is not in the spike protein’s receptor-binding domain (RBD), a region that many neutralizing antibodies target: the RBD binds to the cell-receptor protein ACE2, a key step in the virus’s entry to cells. But evidence is emerging that other mutations could help the virus to avoid some antibodies. A team led by virologists Theodora Hatziioannou and Paul Bieniasz, at Rockefeller University in New York City, genetically modified the vesicular stomatitis virus — a livestock pathogen — so that it used the SARS-CoV-2 spike protein to infect cells, and grew it in the presence of neutralizing antibodies. Their goal was to select for mutations that enabled the spike protein to evade antibody recognition. The experiment generated spike-protein mutants that were resistant to antibodies taken from the blood of people who had recovered from COVID-19, as well as to potent ‘monoclonal’ antibodies that are being developed into therapies. Every one of the spike mutations was found in virus sequences isolated from patients, report Hatziioannou, Bieniasz and their team — although at very low frequencies that suggest positive selection is not yet making the mutations more common11. Other scientists are trying to stay ahead of SARS-CoV-2’s evolution by predicting which mutations are likely to be important. Jesse Bloom, an evolutionary virologist at the Fred Hutchinson Cancer Research Center in Seattle, Washington, led a team that created nearly 4,000 mutated versions of the spike protein’s RBD, and measured how the alterations affected the expression of the spike protein and its ability to bind to ACE2. Most of the mutations had no effect on or hindered these properties, although a handful improved them12. Some of these mutations have been identified in people with COVID-19, but Bloom’s team found no signs of natural selection for any of the variants. “Probably the virus binds to ACE2 about as well as it needs to right now,” he says. How the pandemic might play out in 2021 and beyond The researchers didn’t test whether any of the mutations allow the virus to thwart the action of antibodies, but his team’s results suggest that such changes are possible. “It is a possibility, but by no means a certainty, that the virus will acquire mutations that change its susceptibility to antibodies and immunity,” says Bloom. Based on experience with other coronaviruses, that might take years. Studies of common-cold coronaviruses, sampled across multiple seasons, have identified some signs of evolution in response to immunity. But the pace of change is slow, says Volker Thiel, an RNA virologist at the Institute of Virology and Immunology in Bern. “These strains remain constant, more or less.” With most of the world still susceptible to SARS-CoV-2, it’s unlikely that immunity is currently a major factor in the virus’s evolution. But as population-wide immunity rises, whether through infection or vaccination, a steady trickle of immune-evading mutations could help SARS-CoV-2 to establish itself permanently, says Sheahan, potentially causing mostly mild symptoms when it infects individuals who have some residual immunity from a previous infection or vaccination. “I wouldn’t be surprised if this virus is maintained as a more common, cold-causing coronavirus.” But it’s also possible that our immune responses to coronavirus infections, including to SARS-CoV-2, aren’t strong or long-lived enough to generate selection pressure that leads to significantly altered virus strains. Worrisome mutations could also become more common if antibody therapies aren’t used wisely — if people with COVID-19 receive one antibody, which could be thwarted by a single viral mutation, for example. Cocktails of monoclonal antibodies, each of which can recognize multiple regions of the spike protein, might lessen the odds that such a mutation will be favoured through natural selection, researchers say. Vaccines arouse less concern on this score because, like the body’s natural immune response, they tend to elicit a range of antibodies. It’s even possible that the D614G change could make the virus an easier target for vaccines, Montefiori’s team found in a study posted to bioRxiv in July13. Mice, monkeys and humans that received one of a number of experimental RNA vaccines, including one being developed by drug maker Pfizer in New York City, produced antibodies that proved more potent at blocking G viruses than D viruses. With G viruses now ubiquitous, the finding is “good news”, says Montefiori. But as a scientist who has watched HIV mutate to elude many vaccines developed against it, he remains wary of the potential of SARS-CoV-2 to evade humanity’s responses. How South Korea’s Flu Vaccine Scare Offers Lessons for Other Nations Officials and scientists fought misinformation with data and clear communication — offering a game plan for the rollout of Covid-19 vaccines, experts say. By Choe Sang-Hun and Denise Grady

South Korea had vastly expanded its flu vaccine program to cover millions more people, to prevent a one-two punch to its health system as the coronavirus spread globally. But as the injections got underway, reports of deaths started popping up. South Korean scientists quickly determined that the deaths were unrelated to the flu shots. But they worried that if they didn’t stop the panic, the public might shun the vaccines altogether. So health officials doubled down — and, in the process, gave the world a game plan for when coronavirus vaccines become widely available. They ramped up efforts to communicate with the public. They disclosed data on what was found. And they quickly got the vaccination campaign back on track, at a time when scientists dealing with Covid-19 are increasingly worried about the rise of the anti-vaccine movement. “South Korea is doing everything right,” said Dr. Noel T. Brewer, a professor in the health behavior department at the Gillings School of Global Public Health at the University of North Carolina. “The government is gathering data, giving information to the public quickly and standing up for their vaccination program. That will ensure public trust, and help the program.” As pharmaceutical makers rush to get a coronavirus vaccine approved, countries around the world are grappling with how to safely and quickly distribute hundreds of millions of doses. Already, the pandemic has created confusion and distrust in an era when misinformation can spread rapidly — all of which could complicate the global effort toward immunity. If a long-proven, extensively tested flu shot can spark uncertainty and concern, it could portend challenges for mass inoculations of a brand-new coronavirus vaccine. The potential pitfalls played out on a small scale in South Korea. The country’s ambitious flu vaccine campaign began on Sept. 8, a month earlier than usual. Health officials announced plans to inoculate 30 million people, 10 million more than last year. But in their haste, problems arose. A company hired to transport millions of doses of the vaccine had never carried such large quantities of cargo before. The flu vaccines need to stay refrigerated, at between 2 and 8 degrees Celsius. On Sept. 21, photos began circulating online showing boxes of vaccines stacked outdoors, in a parking lot. South Korea’s flu vaccine campaign was up and running again on Oct. 13. But the public was still wary. The following week, a family reported that a 17-year-old high school student had died after receiving a flu vaccine. More reports of deaths came rushing in, most involving patients in their 70s and older. By Oct. 22, the reported death count had reached 28 and it was rising by the day. Singapore briefly suspended the use of a South Korean vaccine after the deaths were reported. As Dr. Jung Jae-hun, a professor of preventive medicine at Gachon University near Seoul, read the news, he felt an urgent need to push back. CORONAVIRUS BRIEFING: An informed guide to the global outbreak, with the latest developments and expert advice. Flu vaccines had been tested and safely used for decades in South Korea. If the credibility of the program could be so easily undermined by unsubstantiated claims, he wondered what would happen once millions of people started taking coronavirus vaccines. “I think this extreme situation was created because we are living through this unusual time of Covid-19 and people are overly sensitive about vaccines,” Dr. Jung wrote on Oct. 22, in the first of a series of Facebook posts criticizing the news reports. He warned that reporting on “post-vaccination deaths” when there was no scientific evidence establishing a connection was like counting how many people had died after eating breakfast. “If people don’t understand this, it could lead to the proliferation of anti-vaxxers here, like the ones in the West,” he said. While the government stood by the safety of the vaccines, it also launched an investigation into the deaths, hoping to use science to counter misinformation. If, for instance, all the cases were linked to a particular vaccine or clinic, or if all the deaths were similar, it would raise red flags. Multiple deaths from anaphylaxis, a severe allergic reaction, would also implicate the vaccine. But the government’s forensic investigations, which would eventually rule out such connections, didn’t move as quickly as the panic spread. “Elderly people die every day, of a stroke, of heart attack, but the media reported these deaths as if none of them died on a normal day,” said Dr. Ki Mo-ran, an epidemiologist at the National Cancer Center who sat on the government’s vaccination supervisory group. “As the people waited for the results, anxiety grew, trust fell and the vaccination program suffered.” To help quell some of the public’s concerns, Dr. Jung published an opinion piece in the Journal of Korean Medical Science pointing out that it’s not unusual for some people to die coincidentally from unrelated causes, after receiving a vaccine. He cited a study published in 2013 that showed 23 out of every 100,000 Americans aged between 75 and 84 had died from a variety of causes within a week of taking their vaccines. Only 19 million South Koreans have gotten flu shots this year, far short of the government’s target of 30 million. Days after Dr. Jung’s essay was published, the South Korean government released corresponding national figures. Last year, it said, 1,500 South Koreans 65 or older had died within a week of taking a flu vaccine. None of the deaths were linked to the vaccines. As influenza usually kills 3,000 people each year in South Korea, health officials insisted that the benefits of vaccination far outweighed any risks. To Dr. Kim Woo-joo, a professor of medicine at Korea University, the government’s initial response could have been faster and more aggressive. “It was a failure in risk communication,” he said. “What was needed was fast, transparent and science-based communication to dispel misinformation and conspiracy theories and to show that these deaths were likely just coincidences.” But Dr. Brewer, the professor at the University of North Carolina, held up South Korea as an example of how to respond to vaccine scares in the future. He noted that the main threat to vaccination programs around the world is misinformation, often related to an unproven safety scare. He pointed to Japan and Denmark, where misinformation spread about vaccines against the human papillomavirus, or HPV. The vaccines help prevent cervical cancer in women, among other diseases. Both countries failed to address inaccurate reports about the safety of the vaccines. Viruses constantly mutate, so it’s not surprising that the coronavirus that emerged in China in late 2019 has gone through multiple minor variations. But it has also undergone several major mutations, and it’s likely that more, significant variations will emerge. Most recently, strains have emerged in South Africa and the U.K. that have prompted some concerns about the efficacy of coronavirus vaccines. There is also a suspected new strain in the U.S., with the White House coronavirus task force warning early in the new year that there could be a new, more transmissible variant of the virus that evolved in the U.S. and is driving spread, according to a document obtained by NBC News. And on Sunday, Japan’s National Institute of Infectious Diseases said it had detected a new variant of the coronavirus in four travelers arriving from Brazil. Essentially, scientists worry about any major changes to the virus’ so-called spike protein. This contains the receptor-binding domain, and is used by the virus to gain entry into cells within the body. As such, mutations can not only make the virus more transmissible, they can mean that vaccines are rendered less powerful and require them to be updated. The ‘UK variant’ A new variant was reported by U.K. health officials to the World Health Organization on Dec. 14 that is now known formally as “VOC 202012/01” (which stands for “variant of concern, year 2020, month 12, variant 01″). The variant was first detected in a patient in Kent, southeast England, in September. It then quickly spread to London. With infections surging in both regions, early analysis suggested it may be up to 70% more transmissible than the old variant that was circulating in the country. News of the new variant prompted many countries to ban flights from the U.K. in a bid to keep the new strain out, and led the country’s government to scrap a planned relaxation of social restrictions over Christmas. Nonetheless, the mutation provoked a huge surge in infections, with the number of new daily cases coming in above 50,000 since December 28. The WHO notes that “how and where SARS-CoV-2 VOC 202012/01 originated is unclear,” although scientists are looking into whether the mutation emerged in patients with weaker immune systems who had long-running coronavirus infections, giving the virus the opportunity and time to evolve in a way that enables it to spread faster. The ‘South Africa variant’ Hot on the heels of the news from the U.K., South Africa’s authorities announced on Dec. 18 the detection of a mutation that was rapidly spreading in Eastern Cape, Western Cape, and KwaZulu-Natal. It has now become the country’s dominant strain of the coronavirus. South Africa named the variant “501Y.V2” because of the N501Y mutation found in the spike protein. This mutation, among others, was also found in the new U.K. strain and, as such, it is thought to be similarly more transmissible. The South Africa strain does contain other mutations, and these have prompted some concerns that it could prove more resistant to coronavirus vaccines. However, most scientists expect the vaccines to work despite the mutation and vaccines are regularly adapted to new strains of viruses, like the common flu. The ‘Denmark mink variant’ Another coronavirus variant that emerged in Denmark last summer was linked to the country’s large mink farming sector. Since June, 214 human cases of Covid-19 have been identified in Denmark with variants associated with farmed minks. Twelve of those cases were identified as having a unique variant that was reported to the WHO on Nov. 5. The new strain was found in North Jutland in Denmark and was linked to infections among farmed mink that had subsequently been transmitted to humans. “The variant, referred to as the ‘Cluster 5’ variant by Danish authorities, had a combination of mutations not previously observed,” the WHO noted. It added that these prompted concern that they could “result in reduced virus neutralization in humans, which could potentially decrease the extend and duration of immune protection following natural infection or vaccination.” Studies are ongoing to assess the effectiveness of treatment among humans with this variant. Luckily, it does not appear to be more transmissible, according to the European Centre for Disease Prevention and Control, and the WHO. Denmark took drastic action, culling 17 million farmed mink. Early mutation in China The WHO notes that a variant of the coronavirus appeared early on in the pandemic (indeed, before it was even declared a global pandemic in March 2020), noting that a new strain with a mutation known as “D614G” emerged in late Jan. or early Feb. 2020. After several months, this became the dominant strain of the virus we know today, the WHO said. “Over a period of several months, the D614G mutation replaced the initial SARS-CoV-2 strain identified in China and by June 2020 became the dominant form of the virus circulating globally.” Studies in human respiratory cells and animal models demonstrated that compared to the initial virus strain, the newer strain had increased in infectivity and transmission. However, the new variant was not seen as causing “more severe illness or alter the effectiveness of existing laboratory diagnostics, therapeutics, vaccines, or public health preventive measures.” No blame-game While variants are sometimes dubbed “the U.K. strain” or “Denmark mutation,” experts say it’s important to note that the origin of these viruses are ultimately difficult to prove and countries shouldn’t be “blamed” for different mutations. A handful of new strains of the coronavirus have emerged overseas that have given scientists some cause for concern. Some variants that have been identified in the United Kingdom, South Africa and Brazil appear to be more transmissible than previous strains but not necessarily more deadly. While it’s no surprise the virus is mutating, researchers are quickly trying to determine what the changes might mean for recently developed lifesaving vaccines and therapeutics against the disease. Some early findings that were published in the preprint server bioRxiv, which have yet to be peer reviewed, indicate that the variant identified in South Africa, known as 501Y.V2, can evade the antibodies provided by some coronavirus treatments and may reduce the efficacy of the current line of available vaccines. Both vaccines from Pfizer and Moderna have proven to be highly effective, providing about 95% protection from infection, which creates a “cushion effect” that would allow for some dip in their effectiveness. “We’re following very carefully the one in South Africa, which is a little bit more concerning, but nonetheless not something that we don’t think we can handle,” Fauci said. A dip in the vaccines’ effectiveness would be “all the more reason why we should be vaccinating as many people as you possibly can.” Mutations occur when the virus spreads and replicates itself, which can be suppressed if enough people are inoculated against the disease to build so-called herd immunity, he explained. “Bottom line: We’re paying very close attention to it. There are alternative plans if we ever have to modify the vaccine. That’s not something that is a very onerous thing, we can do that given the platforms we have,” Fauci said. The World Health Organization’s immunization director, Kate O’Brien, said on Friday that it’s too early to have clear information on whether the emerging variants will have an impact on any of the current vaccines. O’Brien added that not every variant will act the same way, and the answer will depend on the type of mutation and the vaccine. “This is really evolving information and there are a number of ways evaluations can be done to understand whether or not any ... of the vaccines are less effective against the variants,” O’Brien said during a press briefing. Dr. Rochelle Walensky, the new director for the Centers for Disease Control and Prevention, said on Tuesday that while she believes the vaccines will workagainst the mutated variants, they may not end up as effective as they were in clinical trials. “I’m really optimistic about how these variants are going to go,” Walensky said in an interview with the JAMA Network. “I could be wrong. It could be that we’ll find variants and variants may emerge ... where the vaccine is less potent, but I’m still currently optimistic.” Early dataSo far, the United States has not detected any cases of Covid-19 with the strain identified in South Africa, Fauci said, though he added that the level of surveillance for the strains “is not at the level that we would have liked.” The research on the new variants is preliminary and limited. The National Institute for Communicable Diseases used the blood samplesfrom 44 people who previously had Covid-19 to determine if their antibodies worked against the 501Y.V2 variant. The study found that in nearly half of the cases, the neutralizing antibodies were not effective against the new variant, suggesting people may be susceptible to reinfection. The researchers noted that more large-scale clinical trials will be needed to determine the outcome of the vaccines. They added that “nevertheless, the speed and scope of 501Y.V2 mediated immune escape from pre-existing neutralizing antibodies highlight the urgent requirement for rapidly adaptable vaccine design platforms.” Another study, which has not been peer-reviewed, from Rockefeller University used blood samples from 20 people who had been vaccinated against Covid-19 with either Moderna’s or Pfizer’s regimen and tested their antibodies against the various mutations. Researchers noticed that with some of the mutations, the antibodies didn’t work as well, adding that the mRNA vaccines “may need to be updated periodically to avoid potential loss of clinical efficacy.” “It’s a small difference but it is definitely a difference,” Rockefeller’s Dr. Michel Nussenzweig, who led the study, told the Associated Press. However, a study printed in bioRxiv by Pfizer and BioNTech’s scientists found that their Covid-19 vaccine was likely to be just as effective against the mutated, infectious strain that was discovered in the U.K. The U.S. has identified at least 144 cases with that variant so far, according to recent data from the CDC. Pandemic aftershocks overwhelm global supply lines As shopping habits change, ports and cargo carriers struggle to keep pace One year after the coronavirus pandemic first disrupted global supply chains by closing Chinese factories, fresh shipping headaches are delaying U.S. farm exports, crimping domestic manufacturing and threatening higher prices for American consumers. LYNCH JANUARY 24 AT 3:02 PM The cost of shipping a container of goods has risen by 80 percent since early November and has nearly tripled over the past year, according to the Freightos Baltic Index. The increase reflects dramatic shifts in consumption during the pandemic, as consumers redirect money they once spent at restaurants or movie theaters to the purchase of record amounts of imported clothing, computers, furniture and other goods. That abrupt and unprecedented spending shift has upended long-standing trade patterns, causing bottlenecks from the gates of Chinese factories to the doorsteps of U.S. homes. The commercial disorder is just the latest blow to globalization’s finely tuned engine, capping more than a decade of financial crisis, trade wars, contagion and recession. Each shock has triggered swings in the flow of cash and goods through the $91 trillion global economy. But reverberations from the pandemic are exposing vulnerabilities in the physical plumbing of cross-border commerce that may linger, according to exporters, port officials and trade specialists. “It’s crazy. Prices are at record highs. Multiple things are happening all at once,” said Phil Levy, an economist with Flexport, a San Francisco-based freight forwarder. “People work off of expectations. But now there’s just so much uncertainty.” At the Port of Los Angeles one day last week, 42 ships were anchored offshore, waiting to unload their cargoes, even as every warehouse within 60 miles was already full. A shortage of dock workers amid California’s worsening coronavirus outbreak is further complicating operations; inbound cargo volumes in December were more than 23 percent higher than one year earlier. “Some areas of the supply chain need to be sharpened,” Gene Seroka, the port’s executive director, said. “People are a little bit on edge.” It’s a global problem, and it may get worse before it gets better. More than one-third of the containers transiting the world’s 20 largest ports last month failed to ship when scheduled, according to Ocean Insights, a data provider. Glimmers of sticker shock are starting to vex corporate planners. The cost of imported industrial supplies jumped 4.2 percent in December and is up 27 percent since April’s pandemic low, with manufacturers complaining of shortages of materials such as steel. Shipping issues are affecting familiar brand names such as the Gap, where an executive recently told investors that “port issues” were hamstringing operations. At WD-40, higher freight and warehousing costs dented profit margins last quarter, Jay Rembolt, the chief financial officer, told investors this month. Bang & Olufsen, a maker of music systems and televisions, said it had resorted to more expensive airfreight to compensate for a lack of seaborne options. “These challenges have put inflationary cost pressures on our and many businesses and, as the market is anticipating, will put further inflationary pressure on transportation rates in 2021,” said Shelley Simpson, chief commercial officer for J.B. Hunt Transport Services, on a recent earnings call. Consumers already have seen an impact with spot shortages of household appliances and some clothing items in recent months. The price of goods arriving from China posted its largest one-month gain in more than three years last month, rising by 0.3 percent. Overall, prices of imported goods rose 0.9 percent, their largest rise since August. By themselves, shipping cost spikes are likely to have only a modest effect on inflation, according to Neil Shearing, chief economist for Capital Economics in London. But they will reinforce the effects of other factors, such as oil prices and ample fiscal and monetary stimulus, that are expected to drive the current 1.4 percent inflation rate higher, at least for a while. “All of these temporary factors come together at the same time the market narrative is primed for a post-covid inflation surge,” Shearing said. As the pandemic rippled across the globe last year, it interfered with typical seasonal patterns of global production and distribution. Factories closed, first in China and then elsewhere, as the world slipped into recession. Shipping carriers initially idled vessels to match reduced demand. But as consumers stuck at home began buying desks, computers, backyard fire pits and entertainment systems — and Chinese factories resumed normal operations — Asian exporters clamored for space aboard cargo ships. The sudden changes played havoc with supply chains that were designed to operate on “just in time” principles, bringing goods to ports when vessels were waiting to whisk them to distant customers. But the surge in demand overwhelmed the system. Fewer ships arriving in U.S. ports meant fewer shipping containers available for the return trip to Asia. With department stores and other retailers closed by shutdowns, goods piled up at port terminals and stateside warehouses. Last year’s stop-and-go global economy effectively shifted 5 million shipping containers from the first half of the year to the second half — on top of customary trade flows, said Lars Jensen, chief executive of SeaIntelligence, a Copenhagen-based consultancy. “It’s multiple different bottlenecks all at the same time,” Jensen said. “It’s like a train wreck in slow motion.” At the nation’s busiest container port, officials in Los Angeles have seen cargo volumes soar and plunge in dizzying cycles. Inbound shipments fell in the first months of the pandemic before roaring back to life in August. Average monthly import volumes in the second half of 2020 were more than 50 percent greater than during the first six months of the year, according to Seroka, the port’s executive director. Since there is more demand to send goods from China to the United States than to ship in the other direction, ocean carriers — after delivering their cargoes to Los Angeles — are refusing to wait for their containers to be reloaded with U.S. exports before returning them to China. In December, the port processed about 2.5 times as many empty containers headed to China as full ones. “It is simply a matter of supply and demand,” Seroka said.
