Don't hold your breath for a coronavirus vaccine. Here are the 7 biggest challenges we still need to overcome. That's why scientists and governments are working at unprecedented speeds to create one. Since the start of the outbreak, top health officials have said it'll take about 12-18 months for a vaccine to be ready. Anthony Fauci, director of the National Institute of Allergy and Infectious Diseases, has said he's "optimistic" that we'll have at least one vaccine ready by the end of this year or the start of 2021. But even that timeline is unknown. "When you're dealing with vaccines, you can't guarantee things," Fauci told PBS NewsHour in a July 17 interview. Plus, even once we create a promising vaccine, big challenges to humanity's exit strategy come after that: in testing, mass-manufacturing, and distributing these vaccines fairly and affordably. More insidious political and cultural barriers may also arise, since even if a vaccine is produced and distributed in record time, many Americans say they won't opt in. Where things stand now Progress to date looks good: More than 150 coronavirus vaccine programs are underway. While the majority are still being developed on laboratory benchtops, about two dozen are already being testing in people. The frontrunners, from Moderna, Pfizer, and AstraZeneca, are on the verge of kicking off clinical trials that will enroll tens of thousands of volunteers to help determine whether the shots prevent coronavirus infections or enable people to fight off the virus if they do get sick. Pharmaceutical giant Merck is also working on a coronavirus shot but hasn't provided details on when it might be ready. Early coronavirus vaccine safety tests, done in small groups of young, healthy people, so far show that the majority of vaccinated volunteers experience some mild side effects, such as fevers, fatigue, and pain at the injection site. That raises questions about whether people who are older or who have preexisting health conditions — those who need protection from COVID-19 the most — will tolerate these shots as well as the test groups. Additional trials are underway in larger groups of people, including the elderly and people with comorbidities. When vaccine candidates get tested in tens of thousands of people, it's possible we'll learn about additional problems, including ultra-rare but serious side effects that have hindered some previous vaccine programs. There's also a theoretical risk that a vaccine could make the disease worse in some people, a phenomenon called antibody-dependent enhancement. Does the vaccine work? The answer to this question is probably more complicated than a simple yes or no. A perfect vaccine would be 100% effective at preventing viral infections. But no vaccine developer expects this to be the case with COVID-19. The US Food and Drug Administration has said it wants to see at least 50% effectiveness before it approves a shot. A coronavirus vaccine could wind up being partially effective, like the flu shot, meaning it reduces the likelihood that people who get the shot develop serious symptoms. Moderna, for instance, has designed its big human trial to detect whether its coronavirus vaccine can achieve at least a 60% reduction in symptomatic disease. So even with a vaccine, it's likely that there will be some coronavirus that continues to circulate in society. Will protection from a vaccine fade over time? A test tube containing coronavirus antibodies. Vaccines are created to prompt your body to generate antibodies that protect you from future infection. Companies like Moderna, Pfizer, and AstraZeneca have compared their vaccines to a sampling of recovered COVID-19 patients in early studies. The results have shown that the shots generate an antibody response that's at least as strong as what a typical COVID-19 survivor's body naturally develops. But some research suggests coronavirus antibodies don't last very long — one study suggested antibodies might last only three to five weeks in some patients. If antibodies are really that fleeting, a vaccine's protection could be short-lived, too. Still, experts say that's no cause for panic, since our body's immunity isn't just tied to antibodies. White blood cells have an impressive immunological memory that can help the body identify and attack the invading virus should it ever return. T cells can destroy infected cells, and B cells work to produce new antibodies. A vaccine could help create both. "There's no evidence that immunity is fleeting and no rigorous estimates yet of how long it will last," Marm Kilpatrick, a disease ecologist at University of California Santa Cruz, told Business Insider. And even if antibodies wane over time, that's not a deal breaker for a vaccine, according to Florian Krammer, a vaccinologist at the Icahn School of Medicine at Mount Sinai. "This happens for a lot of vaccines," he previously told Business Insider. "It's not a problem. You can get revaccinated." Many vaccines have to be given repeatedly, or boosted, at some point in a person's life. "To be quite honest, the least of our problems is, 'Do we have a vaccine that is going to last us for years?'" Maria Elena Bottazzi, a vaccine developer at Baylor College of Medicine, told Business Insider. Can we mass-produce it? Say a vaccine is found to be safe and effective for widespread use. Then comes another huge challenge: developers will have to make massive quantities of it. The leading vaccines use several different technologies — such as mRNA, recombinant protein, and adenoviruses —each of which has its own complex manufacturing process. While all the leading vaccine developers have been scaling up their production capabilities, none will be able to meet worldwide demand anytime soon. What's more, when vaccine vials come off the factory line, the last mile of their journey may be the hardest. Some of the candidates, such as Moderna's and Pfizer's, need to be kept in freezers or refrigerators to work. Moderna's vaccine, for instance, needs to be stored at minus 4 degrees Fahrenheit. That will pose a formidable infrastructure issue for the developing world, particularly in places where electricity or established health systems are not widespread. Still, world governments have already committed billions to scale up vaccine production. When will we really have a vaccine?Having a vaccine is a mushy concept: Do we "have a vaccine" when one succeeds in clinical trials? Or when it's approved by regulators? Or only once there are enough doses available to inoculate the world? It's possible that a vaccine could secure emergency use authorization from the FDA in the US as soon as this fall, but that doesn't mean it would be widely available. According to David Heymann, an epidemiologist and advisor to the World Health Organization, it's likely that those first limited doses will go to the swaths of the population at greatest risk of infection. "This would likely be health workers and carers, persons with comorbidities and the elderly," he said. "After that other populations would likely be considered provided there is sufficient vaccine." The Trump administration has set a goal of having 300 million doses of a vaccine by January. There's no detailed timetable for how to inoculate the world, but getting any vaccine to the entire planet will take at least several years, if not longer. Groups like Gavi and the World Health Organization are creating a framework to ensure the developing world is not left behind. Once we have a vaccine, will people want to get it?Even if the US winds up ready to start vaccinating lots of people in early 2021, it's not clear how many people would trust the shots. Scientists and some politicians have struggled for months to convince chunks of the American public that masks are worth wearing. Winning over vaccine skeptics will likely be even more challenging. Doing so requires the public to trust the drug companies that developed the shot, the research system that vetted it in trials, and the Trump administration regulators that approved it — all of which would have happened in record time. In a recent poll, about one in five Americans said they don't plan to get a coronavirus vaccine, while half said they would. The rest were unsure. Another barrier could be the cost. An early coronavirus bailout bill requires US health insurers to eventually cover a coronavirus vaccine, though that might not happen immediately. The US has also vowed to make a coronavirus vaccine free to "vulnerable" Americans, but the details aren't clear. The government announced this month, however, that it would make one potential coronavirus vaccine, from Pfizer, available at no cost to individuals. What about booster shots?Given that people will likely need to get two shots of a coronavirus vaccine a few weeks apart for it to be effective, healthcare systems in all countries will have to ensure that people come back for the second dose — which is sure to be a challenge. "The more complicated the schedule, the more difficult it is to get people to come in," Walt Orenstein, a vaccinologist and former director of the US National Immunization Program, told Business Insider. Two required shots also means we'll need twice as many vials, syringes, clinic visits, and so on. Those challenges would only snowball if it turns out that people need to get revaccinated regularly. Different SARS-CoV-2 strains haven’t yet had a major impact on the course of the pandemic, but they might in future. 08 SEPTEMBER 2020, CORRECTION 16 SEPTEMBER 2020 When COVID-19 spread around the globe this year, David Montefiori wondered how the deadly virus behind the pandemic might be changing as it passed from person to person. Montefiori is a virologist who has spent much of his career studying how chance mutations in HIV help it to evade the immune system. The same thing might happen with SARS-CoV-2, he thought. In March, Montefiori, who directs an AIDS-vaccine research laboratory at Duke University in Durham, North Carolina, contacted Bette Korber, an expert in HIV evolution and a long-time collaborator. Korber, a computational biologist at the Los Alamos National Laboratory (LANL) in New Mexico, had already started scouring thousands of coronavirus genetic sequences for mutations that might have changed the virus’s properties as it made its way around the world. Compared with HIV, SARS-CoV-2 is changing much more slowly as it spreads. But one mutation stood out to Korber. It was in the gene encoding the spike protein, which helps virus particles to penetrate cells. Korber saw the mutation appearing again and again in samples from people with COVID-19. At the 614th amino-acid position of the spike protein, the amino acid aspartate (D, in biochemical shorthand) was regularly being replaced by glycine (G) because of a copying fault that altered a single nucleotide in the virus’s 29,903-letter RNA code. Virologists were calling it the D614G mutation. How many people has the coronavirus killed? In April, Korber, Montefiori and others warned in a preprint posted to the bioRxiv server that “D614G is increasing in frequency at an alarming rate”1. It had rapidly become the dominant SARS-CoV-2 lineage in Europe and had then taken hold in the United States, Canada and Australia. D614G represented a “more transmissible form of SARS-CoV-2”, the paper declared, one that had emerged as a product of natural selection. These assertions dismayed many scientists. It wasn’t clear that the D614G viral lineage was more transmissible, or that its rise indicated anything unusual, they said. But alarm spread fast across the media. Although many news stories included researchers’ caveats, some headlines declared that the virus was mutating to become more dangerous. In retrospect, Montefiori says he and his colleagues regret describing the variant’s rise as “alarming”. The word was scrubbed from the peer-reviewed version of the paper, published in Cell in July2. The work sparked a frenzy of interest in D614G. Even those who were sceptical that the mutation had changed the virus’s properties agreed that it was intriguing, because of its meteoric rise and ubiquity. For months, that lineage has been found in almost all sequenced samples of SARS-CoV-2 (see ‘Global spread’). “This variant now is the pandemic. As a result, its properties matter,” wrote Nathan Grubaugh, a viral epidemiologist at the Yale School of Public Health in New Haven, Connecticut, and two colleagues in a Cell essay on Korber and Montefiori’s findings3.  So far, the upshot of this work is less clear than Montefiori and Korber’s preprint suggested. Some experiments suggest that viruses carrying the variant infect cells more easily. Other work has revealed possible good news: the variant might mean that vaccines can target SARS-CoV-2 more easily. But many scientists say there remains no solid proof that D614G has a significant effect on the spread of the virus, or that a process of natural selection explains its rise. “The jury’s out,” says Timothy Sheahan, a coronavirologist at the University of North Carolina at Chapel Hill. “This mutation might mean something, or it might not.” Researchers still have more questions than answers about coronavirus mutations, and no one has yet found any change in SARS-CoV-2 that should raise public-health concerns, Sheahan, Grubaugh and others say. But studying mutations in detail could be important for controlling the pandemic. It might also help to pre-empt the most worrying of mutations: those that could help the virus to evade immune systems, vaccines or antibody therapies. Slow changeSoon after SARS-CoV-2 was detected in China, researchers began analysing viral samples and posting the genetic codes online. Mutations — most of them single-letter alterations between viruses from different people — allowed researchers to track the spread by linking closely related viruses, and to estimate when SARS-CoV-2 started infecting humans. Viruses that encode their genome in RNA, such as SARS-CoV-2, HIV and influenza, tend to pick up mutations quickly as they are copied inside their hosts, because enzymes that copy RNA are prone to making errors. After the severe acute respiratory syndrome (SARS) virus began circulating in humans, for instance, it developed a kind of mutation called a deletion that might have slowed its spread4. But sequencing data suggest that coronaviruses change more slowly than most other RNA viruses, probably because of a ‘proofreading’ enzyme that corrects potentially fatal copying mistakes. A typical SARS-CoV-2 virus accumulates only two single-letter mutations per month in its genome — a rate of change about half that of influenza and one-quarter that of HIV, says Emma Hodcroft, a molecular epidemiologist at the University of Basel, Switzerland. Other genome data have emphasized this stability — more than 90,000 isolates have been sequenced and made public (see www.gisaid.org). Two SARS-CoV-2 viruses collected from anywhere in the world differ by an average of just 10 RNA letters out of 29,903, says Lucy Van Dorp, a computational geneticist at University College London, who is tracking the differences for signs that they confer an evolutionary advantage. Despite the virus’s sluggish mutation rate, researchers have catalogued more than 12,000 mutations in SARS-CoV-2 genomes. But scientists can spot mutations faster than they can make sense of them. Many mutations will have no consequence for the virus’s ability to spread or cause disease, because they do not alter the shape of a protein, whereas those mutations that do change proteins are more likely to harm the virus than improve it (see ‘A catalogue of coronavirus mutations’). “It’s much easier to break something than it is to fix it,” says Hodcroft, who is part of Nextstrain, an effort to analyse SARS-CoV-2 genomes in real time. Lancet 396, 603–611 (2020) Many researchers suspect that if a mutation did help the virus to spread faster, it probably happened earlier, when the virus first jumped into humans or acquired the ability to move efficiently from one person to another. At a time when nearly everyone on the planet is susceptible, there is likely to be little evolutionary pressure on the virus to spread better, so even potentially beneficial mutations might not flourish. “As far as the virus is concerned, every single person that it comes to is a good piece of meat,” says William Hanage, an epidemiologist at the Harvard T. H. Chan School of Public Health in Boston, Massachusetts. “There’s no selection to be doing it any better.” Faster spread? When Korber saw the rapid spread of D614G, she thought she might have found an example of meaningful natural selection. The mutation caught her eye because of its position in the spike protein, which is a major target for ‘neutralizing’ antibodies that bind to the virus and render it non-infectious. And viruses with the mutation were also rising in frequency in more than one part of the world. D614G was first spotted in viruses collected in China and Germany in late January; most scientists suspect the mutation arose in China. It’s now almost always accompanied by three mutations in other parts of the SARS-CoV-2 genome — possible evidence that most D614G viruses share a common ancestor. D614G’s rapid rise in Europe drew Korber’s attention. Before March — when much of the continent went into lockdown — both unmutated ‘D’ viruses and mutated ‘G’ viruses were present, with D viruses prevalent in most of the western European countries that geneticists sampled at the time. In March, G viruses rose in frequency across the continent, and by April they were dominant, reported Korber, Montefiori and their team1,2. At this time, viruses with the D614G mutation were spreading across Europe. Credit: Roberto Silvino/NurPhoto/Getty But natural selection in favour of G viruses isn’t the only, or even the most likely, explanation for this pattern. The European dominance of G variants could be simply down to chance — if, for instance, the mutation happened to be slightly more common in the viruses that arrived in Europe. A small number of individuals seem to be responsible for most of the virus’s spread, and an early, chance tilt in favour of G viruses could explain the lineage’s apparent takeover now. Such ‘founder effects’ are common in viruses, especially when they spread unchecked, as SARS-CoV-2 did in much of Europe until mid- to late March. Korber and her colleagues tried to rule out a founder effect, by showing in their April preprint1 that D614G rose to dominance quickly in Canada, Australia and parts of the United States (an exception was Iceland, where G viruses present early in its outbreak were overtaken by D viruses). Analysing hospitalization data from Sheffield, UK, the team found no evidence that viruses carrying the mutation made people any sicker. But those infected with G viruses seemed to have slightly higher levels of viral RNA in their noses and mouths than did those with D viruses. Many scientists weren’t convinced that D614G’s rise was remarkable — or all that relevant to the pandemic. “I thought that preprint was incredibly premature,” says Sheahan. Montefiori says his and Korber’s perspective on D614G was shaped by their work on HIV, which has found that even seemingly insignificant mutations can have a profound effect on how the immune system recognizes that virus. “We were alarmed by it, and we need to see if it’s having an effect on vaccines,” he says. Rush of lab studies To examine further whether D614G made the virus more transmissible, Montefiori gauged its effects under laboratory conditions. He couldn’t study the natural SARS-CoV-2 virus in his lab, because of the biosafety containment required. So he studied a genetically modified form of HIV that used the SARS-CoV-2 spike protein to infect cells. Such ‘pseudovirus’ particles are a workhorse of virology labs: they enable the safe study of deadly pathogens such as the Ebola virus, and they make it easy to test the effects of mutations. The experiment uses a pseudovirus that doesn’t require high levels of biosafety containment.Credit: Jeremy Luban The first team to report pseudovirus experiments on D614G, in June, was led by Hyeryun Choe and Michael Farzan, virologists at the Scripps Research Institute in La Jolla, California5. Several other teams have posted similar studies on bioRxiv (Montefiori’s experiments, and those of another collaborator, appeared in the Cell paper2). The teams used different pseudovirus systems and tested them on various kinds of cell, but the experiments pointed to the same conclusion: viruses carrying the G mutation infected cells much more ably than did D viruses — up to ten times more efficiently, in some cases. In laboratory tests, “all of us agree that D to G is making the particles more infectious”, says Jeremy Luban, a virologist at the University of Massachusetts Medical School in Worcester. But these studies come with many caveats — and their relevance to human infections is unclear. “What’s irritating are people taking their results in very controlled settings, and saying this means something for the pandemic. That, we are so far away from knowing,” says Grubaugh. The pseudoviruses carry only the coronavirus spike protein, in most cases, and so the experiments measure only the ability of these particles to enter cells, not aspects of their effects inside cells, let alone on an organism. They also lack the other three mutations that almost all D614G viruses carry. “The bottom line is, they’re not the virus,” says Luban. Some labs are now working with infectious SARS-CoV-2 viruses that differ by only the single amino acid. These are tested in laboratory cultures of human lung and airway cells, and in lab animals such as ferrets and hamsters. For labs with the experience and the biosafety capabilities to manipulate viruses, “this is like bread-and-butter kind of work”, says Sheahan. The first of those studies, led by researchers at the University of Texas Medical Branch in Galveston, was reported in a 2 September preprint6. It found that viruses with the mutation were more infectious than were D viruses in a human lung cell line and in airway tissues, and that mutated viruses were present at greater levels in the upper airways of infected hamsters6. Coronavirus reinfections: three questions scientists are asking Even these experiments might not offer absolute clarity. Some studies show that certain mutations to the spike protein in the Middle East respiratory syndrome (MERS) virus can cause more-severe disease in mice — yet other mutations in the protein show very little effect in people or in camels, the likely reservoir for human MERS infections, says Stanley Perlman, a coronavirologist at the University of Iowa in Iowa City. The clearest sign that D614G has an effect on the spread of SARS-CoV-2 in humans comes from an ambitious UK effort called the COVID-19 Genomics UK Consortium, which has analysed genomes of around 25,000 viral samples. From these data, researchers have identified more than 1,300 instances in which a virus entered the United Kingdom and spread, including examples of D- and G-type viruses. A team led by Andrew Rambaut, an evolutionary biologist at the University of Edinburgh, UK, epidemiologist Erik Volz, at Imperial College London, and biologist Thomas Connor at Cardiff University, studied the UK spread of 62 COVID-19 clusters seeded by D viruses and 245 by G viruses7. The researchers found no clinical differences in people infected with either virus. However, G viruses tended to transmit slightly faster than lineages that didn’t carry the change, and formed larger clusters of infections. Their estimates of the difference in transmission rates hover around 20%, Volz says, but the true value could be a bit higher or lower. “There’s not a large effect in absolute terms,” says Rambaut. It’s possible that D614G is an adaptation that helps the virus to infect cells or compete with viruses that don’t carry the change, while altering little about how SARS-CoV-2 spreads between people or through a population, Rambaut says. “This might be a bona fide adaptation to humans or some human cells,” agrees Grubaugh, “but that doesn’t mean anything changes. An adaptation doesn’t have to make it more transmissible.” Grubaugh thinks that D614G has received too much attention from scientists, in part because of the high-profile papers it has garnered. “Scientists have this crazy fascination with these mutations,” he says. But he also sees D614G as a way to learn about a virus that doesn’t have much in the way of genetic diversity. “The virologist in me looks at these things and says it would be a lot of fun to study,” he says. “It creates this whole rabbit hole of different things you can go into.” He’ll have company. Intense study of D614G should help to explain how SARS-CoV-2 fuses with cells, says Luban — a process that might be blocked by drugs or targeted by a vaccine. In an updated version of their pseudovirus experiments posted on bioRxiv on 16 July8, Luban’s team used cryo-electron microscopy to analyse the structure of spike proteins bearing the D614G change. The spike protein is comprised of three identical peptides in an ‘open’ or ‘closed’ orientation. Previous research has suggested that at least two of the three peptides need to be open for the viral particle to fuse with the cell membrane9, and Luban’s team found that viruses carrying the G spike variant were much more likely to be in this state (see ‘The mutation that loosens the spike protein’). Computational modelling work by Montefiori and Korber, led by Korber’s LANL colleague Sandrasegaram Gnanakaran, came to the same conclusion10. “It looks like this molecular machine is primed to go in a way that D is not,” Luban says. Luban No escape from antibodies — yet Most available evidence suggests that D614G doesn’t stop the immune system’s neutralizing antibodies from recognizing SARS-CoV-2, as Montefiori had worried. That might be because the mutation is not in the spike protein’s receptor-binding domain (RBD), a region that many neutralizing antibodies target: the RBD binds to the cell-receptor protein ACE2, a key step in the virus’s entry to cells. But evidence is emerging that other mutations could help the virus to avoid some antibodies. A team led by virologists Theodora Hatziioannou and Paul Bieniasz, at Rockefeller University in New York City, genetically modified the vesicular stomatitis virus — a livestock pathogen — so that it used the SARS-CoV-2 spike protein to infect cells, and grew it in the presence of neutralizing antibodies. Their goal was to select for mutations that enabled the spike protein to evade antibody recognition. The experiment generated spike-protein mutants that were resistant to antibodies taken from the blood of people who had recovered from COVID-19, as well as to potent ‘monoclonal’ antibodies that are being developed into therapies. Every one of the spike mutations was found in virus sequences isolated from patients, report Hatziioannou, Bieniasz and their team — although at very low frequencies that suggest positive selection is not yet making the mutations more common11. Other scientists are trying to stay ahead of SARS-CoV-2’s evolution by predicting which mutations are likely to be important. Jesse Bloom, an evolutionary virologist at the Fred Hutchinson Cancer Research Center in Seattle, Washington, led a team that created nearly 4,000 mutated versions of the spike protein’s RBD, and measured how the alterations affected the expression of the spike protein and its ability to bind to ACE2. Most of the mutations had no effect on or hindered these properties, although a handful improved them12. Some of these mutations have been identified in people with COVID-19, but Bloom’s team found no signs of natural selection for any of the variants. “Probably the virus binds to ACE2 about as well as it needs to right now,” he says. How the pandemic might play out in 2021 and beyond The researchers didn’t test whether any of the mutations allow the virus to thwart the action of antibodies, but his team’s results suggest that such changes are possible. “It is a possibility, but by no means a certainty, that the virus will acquire mutations that change its susceptibility to antibodies and immunity,” says Bloom. Based on experience with other coronaviruses, that might take years. Studies of common-cold coronaviruses, sampled across multiple seasons, have identified some signs of evolution in response to immunity. But the pace of change is slow, says Volker Thiel, an RNA virologist at the Institute of Virology and Immunology in Bern. “These strains remain constant, more or less.” With most of the world still susceptible to SARS-CoV-2, it’s unlikely that immunity is currently a major factor in the virus’s evolution. But as population-wide immunity rises, whether through infection or vaccination, a steady trickle of immune-evading mutations could help SARS-CoV-2 to establish itself permanently, says Sheahan, potentially causing mostly mild symptoms when it infects individuals who have some residual immunity from a previous infection or vaccination. “I wouldn’t be surprised if this virus is maintained as a more common, cold-causing coronavirus.” But it’s also possible that our immune responses to coronavirus infections, including to SARS-CoV-2, aren’t strong or long-lived enough to generate selection pressure that leads to significantly altered virus strains. Worrisome mutations could also become more common if antibody therapies aren’t used wisely — if people with COVID-19 receive one antibody, which could be thwarted by a single viral mutation, for example. Cocktails of monoclonal antibodies, each of which can recognize multiple regions of the spike protein, might lessen the odds that such a mutation will be favoured through natural selection, researchers say. Vaccines arouse less concern on this score because, like the body’s natural immune response, they tend to elicit a range of antibodies. It’s even possible that the D614G change could make the virus an easier target for vaccines, Montefiori’s team found in a study posted to bioRxiv in July13. Mice, monkeys and humans that received one of a number of experimental RNA vaccines, including one being developed by drug maker Pfizer in New York City, produced antibodies that proved more potent at blocking G viruses than D viruses. With G viruses now ubiquitous, the finding is “good news”, says Montefiori. But as a scientist who has watched HIV mutate to elude many vaccines developed against it, he remains wary of the potential of SARS-CoV-2 to evade humanity’s responses. Luban agrees: “We need to keep our eyes open for additional changes.”

Researchers at Virginia Commonwealth University in Richmond found nearly 75,000 more people may have died from the pandemic than what was recorded in March to July, according to the report published Monday in the peer-reviewed journal JAMA. By examining death certificates, the study found more than 150,000 deaths were officially attributed to COVID-19 during that period. But researchers determined that nearly 75,000 additional deaths were indirectly caused by the pandemic, bringing the total number of deaths for those four months to more than 225,000. Delayed care, fear of seeking care or emotional crises stemming from the pandemic could have also contributed to these deaths, he says, as well as inaccurate death certificates that may have misidentified a COVID-19 death. Woolf saw a similar pattern in a previous study by researchers at Virginia Commonwealth and Yale universities that looked at excess deaths early in the pandemic, from March to April. In that study, researchers found deaths from these other diseases spiked in states like New York, New Jersey and Massachusetts, where coronavirus cases surged during the beginning of the pandemic. In June and July, Woolf said similar deaths spiked in Southern states that experienced a summer surge in coronavirus cases. He could face a relapse, experts warn, as president downplays coronavirus threat despite massive death toll. Another notable finding is how long the surges lasted in various states. According to the study, the level of excess deaths in New York, New Jersey and Massachusetts were immense but short-lived, resulting in a “A-shaped” model. In Sunbelt states, however, excess deaths began to gradually increase at the beginning of the pandemic then skyrocket in June, rising until Woolf’s team ended its research in July. “This suggests it has some policy implications in terms of the consequences of the decision of some states to ease restrictions early in the pandemic,” he said. “It’s sort of a warning call going forward.” Dr. Amesh Adalja, an infectious disease physician and senior scholar at the Johns Hopkins Center for Health Security, said the study confirms what doctors see every day in the hospital and underscored how badly federal officials have performed during the pandemic. While the study may be only a snapshot of March through July, he says the country is still seeing an excess of deaths from the coronavirus. “The fact is even now in the fall with all the knowledge and the new tools ... this is still killing people at a rate that’s much too high,” Adalja said. The study contributes to research from the University of Washington that suggests nearly 400,000 people will die this year from COVID-19 or consequences of the pandemic. JAMA editor-in-chief Dr. Howard Bauchner said in a journal editorial published Monday the number of excess of deaths “cannot be overstated.” “These deaths reflect a true measure of the human cost of the Great Pandemic of 2020,” he wrote.
Americans Are Dying In The Pandemic At Rates Far Higher Than In Other Countries JASON BEAUBIEN · OCTOBER 13, 2020 During this pandemic, people in the United States are currently dying at rates unparalleled elsewhere in the world. A new report in the Journal of the American Medical Association finds that over the last 5 months per capita deaths in the U.S., both from COVID-19 and other causes have been far greater than in 18 other high-income countries. "It's shocking. It's horrible," says Dr. Ezekiel J. Emanuel, a professor of health policy and medical ethics at the University of Pennsylvania and one of the authors of the study. "The United States really has done remarkably badly compared to other countries," he says and then adds, "I mean, remarkably badly." The study looks at per capita death rates in 2020 in 18 countries with populations larger than 5 million people and per capita gross domestic product levels above $25,000 per year. It breaks out deaths attributed to COVID-19, but also examines how total deaths in the U.S. are higher than normal this year. This so-called "all-cause" mortality takes in to account fatalities that may have been due to the coronavirus but were never confirmed or were due to other factors such as people not seeking medical care during the crisis. Overall deaths in the United States this year are more than 85% higher than in places such as Germany, Israel and Denmark after adjusting for population size. Deaths in the U.S. are 29% higher than even in Sweden, "which ignored everything for so long," Emanuel says. Sweden made a point of refusing to order strict social restrictions and never went in to a full lockdown. "We have 29% more mortality than we should have if we'd followed Sweden's path and Sweden virtually did nothing." Even looking just at confirmed COVID-19 deaths, the number of people dying since May 10th — again after adjusting for population size — is on average 50% higher than every other country in the study. In addition the rate people are dying in the U.S. has stayed far above everywhere else. Emanuel says the current elevated mortality rates are important because they eliminate the chaotic early months of the pandemic when testing, treatment and reporting varied dramatically around the globe. The rate of COVID-19 deaths in the U.S. since June 7th is 27.2 per 100,000 people. In contrast in Italy the death rate is down to 3.1 per 100,000. "It's not like Italy has some secret medicine that we don't," Emmanuel says. "They've got the same public health measures we've got. They just implemented them effectively and we implemented them poorly." If the U.S. had managed to keep its per capita death rate at the level of Italy's, 79,120 fewer Americans would have died. This study is important for illustrating just how bad the death rate from COVID-19 has been in the U.S. compared to other countries, says Theo Vos, a professor of health metrics and evaluation at the Institute for Health Metrics and Evaluation at the University of Washington in Seattle. "The U.S. is in the company of a few other countries with very high rates of deaths assigned to COVID," says Vos who was not involved in the study. "Only Belgium looks worse." But he points out that once you look past the initial phase of the outbreak, the U.S. ends up looking worse than Belgium. "Early on in the epidemic, many countries had a lack of testing possibilities. And a lot of deaths occurred early on where there was no formal diagnosis," he says. "So Belgium decided we're going to count all of them. While in other countries like the Netherlands or the U.K. and the U.S. quite a few deaths have not been recognized as COVID." He says this is why looking at overall deaths since the start of the pandemic, not just official COVID-19 deaths is important. On that measure the U.S. is now consistently reporting roughly 25% more deaths than these other wealthy nations. Vos is from the Netherlands and even though he works for the University of Washington, he recently moved back to Holland. He says looking at how death rates are changing over time is important because this pandemic is far from over. "I now live in a country that's seeing the biggest increase in the number of cases of almost any place in the world currently," he says. Second waves of infections are hitting many places in Europe right now including France, Spain, Belgium and the U.K. Vos expects this to get even worse as more people spend more time indoors in the colder months. He says there's also been a lot of complacency in Europe. "Here in the Netherlands, there is an appalling low acceptance of masks," he says. "Only in the last week I've started seeing other people in the supermarket wearing a mask. I used to be the only one." So while the U.S. death rates so far have been much higher than most other wealthy countries, Vos says, that could change as this pandemic continues to ebb and surge in various parts of the world.

Eight months ago, this movie wasn’t even a notion; now it’s one of three non-fiction projects from the Oscar-winning documentarian (“Taxi to the Dark Side”) on multiple platforms this fall. “Totally Under Control” is available On Demand October 13 and hits Hulu October 20. The pandemic has done little to slow down Gibney and his prolific Jigsaw Prods. His HBO documentary “Crazy, Not Insane” was supposed to debut at SXSW; instead, his intimate profile of forensic psychiatrist Dorothy Otnow Lewis who diagnosed high-profile killers with multiple personality disorders debuted at Venice and will finally reach HBO in November. Gibney also completed “Agents of Chaos,” his two-part, four-hour documentary series about Russian interference in the 2016 U.S. presidential election that revealed running for president was Trump’s backup plan to closing Moscow Trump Tower. Gibney collaborated with his “Wikileaks” and “Zero Days” cyber expert Javier Botero on the series, along with famed 60 Minutes producer Lowell Bergman (“Inside Man”). It premiered on HBO September 23. Gibney likes to explore horrifying corporate and government corruption; he first tackled Trump with Netflix series “Dirty Money,” which investigated his business acumen. Quinn, who released Gibney’s breakout documentary at Magnolia Films (“Enron: The Smartest Man in the Room”), liked the idea, as well as rushing it for release ahead of the November election. Keep in mind that in addition to his other 2020 docs, Gibney was also working on a deep dive into the American opioid crisis; was in post-production on Rick Rowley’s “Kingdom of Silence” (October 2, Showtime) about the murder of Washington Post journalist Jamal Khashoggi; and was the executive producer of Matt Heineman’s vérité documentary about New York hospitals fighting coronavirus, which has Heineman furiously editing to meet the Sundance 2021 deadline. “I had no business doing it,” Gibney said. “I had no time or space left. But it seemed important, I felt compelled. I wanted to do it on a timeline to get it out before the election, another crazy decision. That said, I knew I couldn’t do it by myself.” Gibney reached out to two women to come on as his co-directors, “Crazy, Not Insane” producer Ophelia Harutyunyan and New York Times Op-Docs journalist Suzanne Hillinger. “It seemed like a good idea,” he said. The trio pushed forward, knowing that they had to lock picture in August and do post-production in September to meet the theatrical October 9 release date. “We started the editors two weeks after we started,” said Gibney, who set them to work trolling archives. “We had logistical problems managing the data and getting it to our coeditors, who were working remotely.” It took three, then four, experienced editors working overtime to meet the deadline. They finished the movie October 1. At 2 AM on October 2, Quinn called Gibney, telling him that Trump had tested positive. (They noted this with a card at the end.) While Gibney has developed deep sources in Washington, D.C., they weren’t inside the White House or the public health establishment. “It did take hard work,” he said. “The whistleblowers Rick Bright and Max Kennedy helped us to get inside, along with New York Times White House reporter Michael Shear.” While the team rigged up, masked and gowned, to film on location at labs and hospitals, the filmmakers didn’t do in-person interviews. Instead, they delivered COVID-cam packages — complete with tray, computer, and DSLR mic — to their subjects’ houses. “Everything had to be done remotely,” said Gibney. “They would pick up the tray and go into their house and turn it on and connect to the internet.” Once they were set up, DP Ben Bloodwell could adjust the focus and the stop, or suggest a slightly different location. He also set up Interrotrons with lights and sound at local Airbnbs in which the interviewee would enter the space and sit down in front of the camera, which was surrounded by a plastic shower-curtain barrier between the subject and the camera operator. “Over the barrel of the lens was an eye-direct with the image of whichever one of us was conducting the interview,” said Gibney. “We got high marks from doctors and public health officials.” Accustomed to tackling tough, upsetting subjects, even Gibney was shocked by what he learned. “We discovered things that we didn’t expect to discover,” he said. “Also, it was both inspiring and more upsetting that we start the movie one month before COVID hits, where you see people without masks walking around doing their daily business. You see how how this disaster could have been prevented and wasn’t.” Even with its stresses, making “Totally Under Control” was “hugely therapeutic,” said Gibney. “A couple of days I thought my head would explode, but I felt I had some control over something you had no control over. It gave us all a sense of purpose that helped us to avoid an otherwise brutal reliving and gnashing of teeth: ‘This is what happened; it was highly preventable.’ And to hold this administration to account. They intentionally slow-walked testing.” Former FBI deputy director Andrew McCabe in “Agents of Chaos.” When Gibney realized that Trump’s avoidance of testing led to a month-long delay that cost countless lives, “I was angry,” he said. “I still am. This is one that has a real body count. The other one [“Agents of Chaos”] was corruption, it was perfidy, it was how he got elected. With this one, 210,000 people are dead and almost all of that could have been prevented.” By the people who enabled Trump? “100 percent,” said Gibney. “It was hard to watch [U.S. Secretary of Health and Human Services Alex] Azar and [CDC director Robert] Redfield and others complicit in the corruption of science. Donald Trump is a human wrecking ball who was sent by the people to tear it all down, and that’s what he’s done: torn down the reputation of the CDC, which was regarded as the gold standard…The federal government was the ultimate piggy bank for private enterprise to make money in the middle of a pandemic. It’s shocking.” The movie compares America’s fight against the pandemic with South Korea, which discovered the pandemic at the same starting point. “The film is centered around learning and not learning,” said Gibney. “Korea learned lessons and changed things as a result. And we did just the opposite.”
8 Million Have Slipped Into Poverty Since May as Federal Aid Has Dried Up Two new studies show the effect of the emergency $2 trillion package known as the Cares Act and what happened when the money ran out. By Jason DeParle, Oct. 15, 202 WASHINGTON — After an ambitious expansion of the safety netin the spring saved millions of people from poverty, the aid is now largely exhausted and poverty has returned to levels higher than before the coronavirus crisis, two new studies have found. The number of poor people has grown by eight million since May, according to researchers at Columbia University, after falling by four million at the pandemic’s start as a result of a $2 trillion emergency package known as the Cares Act. Using a different definition of poverty, researchers from the University of Chicago and Notre Dame found that poverty has grown by six million people in the past three months, with circumstances worsening most for Black people and children. “These numbers are very concerning,” said Bruce D. Meyer, an economist at the University of Chicago and an author of the study. “They tell us people are having a lot more trouble paying their bills, paying their rent, putting food on the table.” Underscoring those concerns, the Labor Department reported on Thursday that about 886,000 people filed new claims for unemployment benefits last week, an increase of nearly 77,000, or 9.5 percent, from the previous week. Adjusted for seasonal variations, the total was 898,000. The recent rise in poverty has occurred despite an improving job market since May, an indication that the economy had been rebounding too slowly to offset the lost benefits. And now the economy is showing new signs of deceleration, amid layoffs, a surge in coronavirus cases and deadlocked talks in Washington over new stimulus. The Democratic House has twice passed multitrillion-dollar packages to provide more help and to stimulate the economy, but members of a divided Republican Senate, questioning the cost and necessity, have proposed smaller plans. President Trump has alternately demanded that Congress “go big”before the elections and canceled negotiations. On Thursday, he signaled he was ready to increase the size of previous White House offers, to accommodate Democrats, only to be rebuffed hours later by Senator Mitch McConnell, Republican of Kentucky and the majority leader. The Cares Act included one-time payments for most households — $1,200 per adult and $500 per child — and a huge expansion of unemployment insurance. The Tenants Who Evicted Their Landlord That expansion at least doubled the share of jobless workers who receive checks, the researchers estimated, by including gig workers and the self-employed through December. In addition, it added $600 to weekly aid through July — nearly tripling the average benefit. For about two-thirds of the beneficiaries, the bolstered checks more than replaced their lost wages. Latest Updates: The Coronavirus Outbreak Jobless workers built up savings with stimulus relief. But then the checks stopped. At its peak in May, the aid kept more than 18 million people from poverty, the Columbia researchers found. But by September, that number had fallen to about four million. “The Cares Act was unusually successful, but now it’s gone, and a lot more people are poor,” said Zachary Parolin, an author of the Columbia analysis. While the Columbia model showed an improvement in September, the Chicago and Notre Dame analysts found poverty continued to grow. The evidence of increased need comes as Congress is still deadlocked on proposals for providing more aid. Among those experiencing new hardships is Kristin Jeffcoat, 24, who is raising three children in Camptonville, Calif., a hamlet about 80 miles north of Sacramento. When schools closed last spring, Ms. Jeffcoat, an Instacart shopper, stayed home to watch them. Then her husband got laid off from landscaping work. The expanded safety net initially caught them: Together, they received more than $1,500 a week in jobless benefits, which exceeded their lost wages. They also received a $3,900 stimulus check, which they used to prepay three months of rent. But since the unemployment bonus ended in July, their cash income has fallen nearly 80 percent. Now living on $350 a week plus food stamps, Ms. Jeffcoat and her husband have gone without electricity because they cannot afford generator fuel (their house is off the power grid) and have spent weeks without propane for cooking and hot showers. “We stick with cold meals — cereals,” she said. CORONAVIRUS SCHOOLS BRIEFING: It’s back to school — or is it? To feed the children, Ms. Jeffcoat said she sometimes skips meals, especially at the end of the month when the food stamps have run out. Her husband sold his tools to buy diapers, and Ms. Jeffcoat tried to sell her eggs to a fertility clinic, but she did not medically qualify. Worse than the physical hardships is the worry. “I’ve definitely found myself feeling a little more anxious — snappier with the children,” Ms. Jeffcoat said. Income volatility is especially hard on low-income families, who lack the savings or credit to keep essential bills paid. It acts as a kind of invisible tax, measured in units as varied as late fees, toxic stress and worse school outcomes for children. “The lack of predictability has all kinds of negative consequences,” said Bradley L. Hardy, an economist at American University, who notes the recent benefit fluctuations amplify the economic gyrations. The aid expansion did not reach everyone. About a third of the unemployed still do not receive unemployment checks, the Columbia analysts estimated. Some jobless people are unaware they can apply, and many encounter red tape. Undocumented workers are disqualified from unemployment aid, and no one in their households can get stimulus checks, including spouses and millions of American children. Among individuals eligible for stimulus checks, about 30 percent failed to receive them, the Columbia researchers estimate. While most families received them automatically, those too poor to have filed tax returns had to apply. Hazel Musa, left, and her brother Tino outside their home in Camptonville. Still, admirers of the Cares Act say its success in reducing poverty, amid an economic collapse, shows the benefits of a strong safety net. “It wasn’t perfect, but hands down it’s the most successful thing we’ve ever done in negating hardship,” said H. Luke Shaefer, a poverty researcher at the University of Michigan. Members of both research groups said the rising poverty showed a need for a new round of help. “It’s really important that we reinstate some of the lost benefits,” said Mr. Meyer, who is also affiliated with the conservative American Enterprise Institute. His conclusion that poverty is rising may draw special attention because he is a critic of government poverty statistics, saying they exaggerate the number of poor people by failing to fully measure the resources that lower-income households receive. But some opponents of further assistance argue it has discouraged people from working. “There’s just lots of opportunity that’s not being accessed — we’ve got to get people back to work,” said Jason Turner, who runs the Secretaries’ Innovation Group, which advises conservative state officials on aid policies. “I’m not as alarmed about poverty as I am about unemployment. Poverty is an arbitrary income threshold, and people who dip below it, they make adjustments. If you’re not working at all, that’s a huge deal. Physical and mental health declines, substance abuse goes up.” Given the magnitude of the crisis, the increase in poverty since January — about 8 percent by the Columbia count — was a “modest amount,” Mr. Turner said. By the government’s fullest measure, a family of four in a typical city is considered poor if its annual income falls below $28,170. The crisis is hitting minorities especially hard, preserving or even deepening the large poverty gaps that predated the pandemic. The analysts at Chicago and Notre Dame (including James X. Sullivan and Jeehoon Han) found poverty among Black people rising at an especially fast pace, at a time of widespread protests over racial inequality. Black people and Latinos are more than twice as likely as white people to be poor, the new data shows. Both minority groups disproportionately work in industries hard-hit by the recession and may face barriers to aid. Black people disproportionately live in Southern states with low benefits, and some Latinos are disqualified because they lack legal status. Both studies also found child poverty rising at a rapid rate, with an additional 2.5 million children falling below the poverty line since May. Research shows that even short stays in poverty can cause children lasting harm. Jenny Santiago, a single mother in Pontiac, Mich., fears her household’s worsening finances creates new peril for her four children, ages 8 to 13. A driver for takeout services, Ms. Santiago quit work when schools closed in March to to watch her children. The stimulus check and $600 unemployment bonus provided “a nice chunk” of help, she said, “but it didn’t last forever.” Now that her income has dwindled, she trims her meals to feed the children, and her landlord is trying to evict her. But she cannot work without child care, and her children feel her anxiety. “It’s scary,” Ms. Santiago said. “I’ve got to keep a roof over their head. They know when I’m stressed out.” Both studies showed poverty started to rise before the unemployment bonus expired in July, suggesting the stimulus checks, which arrived earlier, played an important role. Optimists might note that the Columbia study showed poverty fell in September. That could be a sign that hardship is easing. But Mr. Parolin, the Columbia researcher, said that he “wouldn’t make too much of a one-month trend” when levels remain elevated. And the Chicago-Notre Dame study found poverty in September continued to grow. Officially, the government measures poverty on an annual basis and publishes its estimates in arrears — this year’s rate will not be released until next fall. To provide policymakers more timely information, both teams use monthly census data to project more up-to-date trends. The Chicago-Notre Dame approach counts the most recent 12 months of income, preserving the annualized time frame. The Columbia researchers consider each month’s income separately, which makes it more timely but ignores earlier paychecks and aid. (The researchers include Megan A. Researchers found that more than 40 percent of patients did not experience a cough, a fever or difficulty breathing. Additionally, about one in five cases did not have symptoms of COVID-19 until after they tested positive for the virus. The team, from Brown University School of Public Health, says the findings not only provide evidence for increased testing and isolation but also that nursing home location could be an important predictor of future. A new study from Brown University School of Public Health found that 40.6% of more than 5,400 COVID-19 cases across nursing homes were asymptomatic. Nursing homes in the top 5% of counties with COVID-19 prevalence had higher counts of asymptomatic and presymptomatic patients (above) Early on in the pandemic, Dr Robert Redfield, the director of the Centers for Disease Control and Prevention, estimated that one in four patients are asymptomatic. In an interview with NPR in March, he said this probably explains how cases continued to spread across the country even as strict social distancing measures were enacted. Redfield added that asymptomatic people can shed the virus up to 48 hours before any symptoms appear. However, new estimates suggests that between 40 percent and 45 percent of all people infected with SARS-CoV-2 are asymptomatic when they are tested. For the study, published in JAMA Internal Medicine, the team looked at data from Genesis HealthCare, a company with about 500 nursing homes across 34 states. Researchers combined electronic medical records with daily infection logs to identity all positive cases recorded between March 16, 2020 and July 15, 2020. At the end of the study period, 182 nursing homes in 20 states had at least one infection with a total of 5,403 positive cases. Results showed found that 2,176 patients were symptomatic with signs such as a dry cough, fever, fatigue and shortness of breath. However, 2,194 patients - or 40.6 percent - were asymptomatic, meaning they did not exhibit any of those signs. What's more, the remaining 10.1 percent of cases were presymptomatic, which means they did not develop symptoms until after testing positive. Rates were slightly higher among nursing homes where facility-wide testing was conducted rather than unit-based testing, which occurs when all residents in specific units are tested without testing the whole house. Among facility-wide testing sites, 40.9 percent of cases were asymptomatic and 19.3 percent of cases were presymptomatic. Nursing homes in the top five percent of counties with coronavirus prevalence had higher combined counts of asymptomatic and presymptomatic cases than those in counties with a lower prevalence. 'We observed high asymptomatic and presymptomatic SARS-CoV-2 infection rates in a large multistate sample of [nursing homes], demonstrating the importance of universal testing for identifying and isolating cases,' the authors wrote. 'The [nursing homes] located in areas with high SARS-CoV-2 prevalence detected higher numbers of asymptomatic and presymptomatic cases during initial point prevalence surveys, building on emerging evidence that [nursing home] location is an important predictor of outbreaks.'
