The Medicaid Redesign Team overwhelmingly backed the slate of proposals at its Thursday meeting, which aim to slash spending on NYC Health & Hospitals — the Big Apple’s public hospital system — by $186 million in the fiscal year beginning April 1. The Medicaid Redesign Team overwhelmingly backed the slate of proposals at its Thursday meeting, which aim to slash spending on NYC Health & Hospitals — the Big Apple’s public hospital system — by $186 million in the fiscal year beginning April 1. Denmark’s Idea Could Help the World Avoid a Great Depression “We are freezing the economy.” MARCH 21, 2020 Derek Thompson While the White House and lawmakers haggle over the terms of an emergency economic-stabilization package, Denmark has gone big—very, very big—to defeat the unprecedented challenge of the coronavirus. This week, the Danish government told private companies hit by the effects of the pandemic that it would pay 75 percent of their employees’ salaries to avoid mass layoffs. The plan could require the government to spend as much as 13 percent of the national economy in three months. That is roughly the equivalent of a $2.5 trillion stimulus in the United States spread out over just 13 weeks. Like I said: very, very big. This response might strike some as a catastrophically ruinous overreaction. Perhaps for Denmark, it will be. But we are at a fragile moment in American history. The U.S. faces the sharpest economic downturn in a century, and statistics that seem impossibly pessimistic one moment look positively optimistic hours later. In weeks—even days—Denmark’s aggressive response could be a blueprint for how the world can avoid another Great Depression. To find out more, I corresponded with Flemming Larsen, a professor at the Center for Labor Market Research at Denmark’s Aalborg University, over two days of emails and an hour-long Skype call. The following interview blends those conversations, which have been edited for length and clarity. Derek Thompson: Could you first give me a sense of what’s happening on the ground in Denmark? What do the streets look like? Flemming Larsen: Denmark has nearly entirely closed down universities, schools, public institutions, restaurants, museums, cinemas. No assembly of more than 10 persons is allowed. The borders have been closed too. Thompson: Denmark’s government has announced a very aggressive plan to help workers in the next few months. Tell me what it’s doing. Larsen: Denmark’s government agreed to cover the cost of employees’ salaries at private companies as long as those companies do not fire people. If a company makes a notice saying that it has to either lay off 30 percent of their workers or fire at least 50 people, the state has agreed to take on 75 percent of workers’ salaries, up to $3,288 per month. (This would preserve the income for all employees earning up to $52,400 per year.) The philosophy here is that the government wants companies to preserve their relationship with their workers. It’s going to be harder to have a strong recovery if companies have to spend time hiring back workers that have been fired. The plan will last for three months, after which point they hope things come back to normal. Thompson: So the government is offering to pick up the tab for workers whose employment is threatened by the downturn. Couldn’t companies easily defraud the government and collect the money anyway? Larsen: Maybe, but the workers compensated are not allowed to work in the period. Workers staying with the company do not receive the 75 percent compensation. Thompson: Some American economists say the U.S. should copy Germany’s work-sharing plan, Kurzarbeit, in which workers’ hours are reduced and then the government takes on part of workers’ salaries. Is Denmark’s plan like that? Larsen: Not exactly. In the German plan, the government and the employer share the cost of paying for work. Here, the government is paying companies for employees who are going home and not working. These workers are being paid a wage to do nothing. The government is saying: Lots of people are suddenly in danger of being fired. But if we have firing rounds, it will be very difficult to adapt later. This way, the company maintains their workforce under the crisis and people maintain their salaries. You are compensating people even though they have to go home. Derek Thompson: The coronavirus will be a catastrophe for the poor Thompson: I think I understand you, and I’m going to try to summarize, but tell me if this summary is wrong: Denmark is putting the economy into the freezer for three months. You’re saying: We know that all these people won’t be able to work for the next few months. It’s inevitable. Rather than do rounds of firing followed by rounds of hiring, which will delay the recovery, let’s throw the whole economy into a deep freezer, and when the virus winds down we can thaw it out and almost everybody will still be with the company they worked for in January. Larsen: That’s exactly it. We are freezing the economy. Because otherwise the government is afraid of the long-term damage that this will do to the entire system. The hope is that this will be over in three or four months, and then we can start up society again. Thompson: What else is Denmark’s government proposing? Larsen: There are a few things. To prevent the financial sector from shutting down, the state will guarantee 70 percent of new bank loans to companies. This will encourage more lending even in the case of more bankruptcies. Also, people on unemployment benefits are put on pause. Typically, people have to go to meetings at job centers and make a certain number of job applications to receive jobless benefits. There are a lot of rules. But those rules are suspended for now. There are no requirements. The other part of the pause is that, while you can only be on unemployment benefits for two years in Denmark, people who pass that threshold will still receive benefits. Again, we are freezing everything. Also, the state agreed to compensate companies for their fixed expenses, like rent and contract obligations, depending on their level of income loss. If they typically sell $1 million in a period, but now they can only sell $100,000, they lose 90 percent of their income. That will qualify them to receive large government help to cover fixed expenses. Also, the spring payment of taxes for companies have been postponed until autumn, and all public employees will keep their salaries when sent home. Thompson: This sounds incredibly bold and incredibly expensive. How much does the government expect this is going to cost? Larsen: The cost is 287 billon DKK. [Over email, we worked out that this is the equivalent of approximately 13 percent of the country’s GDP. In the U.S., that would be about $2.5 trillion.] Thompson: How does this response compare with what Denmark did during the global financial crisis in 2008? Larsen: Back then, there was nothing at all at this scale. There was no huge amount of spending. The government was worried about public debt. There was a huge, long debate about whether Denmark should spend a lot of money at all. And Denmark had one of the highest increases in unemployment during the last crisis. But today, the Danish economy is extremely strong. We have a huge surplus. We have a negative interest rate. There is a lot of public savings. So there is a lot of room to do this now. Also, the political environment has changed. We’ve tried to make higher investments in welfare spending in the last few years. Read: How the coronavirus became an American catastrophe Thompson: It sounds like 10 years ago, there was a debate about stimulus. But today, everybody agrees that you just have to save the economy. Larsen: Yes. They just want to save the economy. The philosophy is, if we don’t do it now, it will be more expensive to save the economy later. We’ve seen what the virus can do in Italy, in Spain. So I think people are very concerned. We are facing a huge, huge crisis. Thompson: I have to admit that it’s rather jarring to hear about a country agreeing in a matter of days to do something this big. Your government could spend more than 10 percent of your GDP before July. Meanwhile, in the U.S., we’re still debating the size of government-signed checks that citizens might not receive for several months. There is a profound difference in both the speed and size of the government response. Larsen: I have to say that the decision-making process in Denmark has been very extraordinary. We have 10 parties in Parliament. From the very left-wing to the really, really right-wing. And they all agree. There is nearly 100 percent consensus about this. And that’s really amazing. People are convinced that it’s wise to do this now. Many of these policies are made as tripartite agreements between unions, employers’ associations, and the state. That’s because, in Denmark, most labor-market regulation is done by the unions and the employers’ associations. They regulate the labor market mainly through their own collective agreements. To make all this possible, you need the unions and employers’ associations to be a part of these agreements. That is very difficult. But they succeeded rapidly. In a matter of days, this was a signed agreement. Juliette Kayyem: The crisis could last 18 months. Be prepared. Thompson: Do you think it’s a good idea? Larsen: I don’t know. Nobody knows for sure. This is unknown territory. I think it’s a good attempt. If you ruin people’s private lives and companies go bankrupt, it will take years to build this up again. So I think it’s a wise decision. As the coronavirus pandemic unfolds, people are dying around the world. But the trajectories of cases and deaths differ by country. The accompanying chart, which will update every morning, allows you to follow the disease’s progression by country. It uses what’s called a logarithmic scale — exponential growth at different rates will appear as straight lines of different steepness. The steeper the line, the faster the total number of coronavirus deaths is doubling. The number of deaths from the illness known as Covid-19 provides one of the most reliable measurements of the pandemic’s impact around the world. Testing rates for the virus differ so much that the number of positive results in a given country is not a precise barometer of how many people are afflicted. But deaths also lag infections substantially; evidence from China suggests that most patients who died from Covid-19 were infected for a month before their death. See the spread of coronavirus on a map. [Click for Map, updated daily] The data are drawn from The New York Times’s aggregation of global coronavirus statistics. To make it easier to compare the trajectories of the epidemic by nation, the counts in the chart all begin with a country’s 25th death. The disease has reached different countries at different times, but comparing them all in this way can show whether the disease is progressing faster or slower in various places once it arrives. Deaths in U.S. States Similar tracking shows how the disease is progressing in different U.S. states. Deaths so far have been highest in the states of Washington and New York, the sites of the country’s first major outbreaks. But the numbers are growing in other parts of the country as well. As death totals in any state reach 10, they will be added to this chart. (Since many parts of the U.S. are in an earlier stage of the epidemic, we’re using a lower threshold for this chart — we expect to adjust this in the days ahead.) Deaths are not a perfect proxy for total infections, since the fatality rate from the disease depends on the underlying age and health of populations; the availability of various treatments; and the capacity of different health systems themselves. The Coronavirus Will Be a Catastrophe for the Poor This pandemic will be especially punishing for low-income workers, just as they were starting to reverse a generation of widening inequality. MARCH 20, 2020, Derek Thompson When the 1918 influenza pandemic ripped through the United States, inequality took its revenge. In the decade just before the outbreak, Progressives had worked tirelessly to narrow the gap between rich and poor that had blown wide open in the Gilded Age. They took on monopoly power with the Federal Trade Commission, ratified a federal income-tax amendment, and gave federal workers a form of disability insurance. But in a rebuke to Progressive victories, the influenza caused a regressive pandemic. Its first wave disproportionately punished the poor, who worked in closer quarters and lived in smaller apartments than their wealthier counterparts. One century later, history is rhyming. The pandemic now bulldozing through the United States will, in its own way, be especially punishing for low-income workers, just as they were starting to reverse a generation of widening inequality. Nobody should confuse 2020 for the height of the 20th-century Progressive era, but the past few years saw precious gains for poorer workers. In late 2019, wages in low-income industries were growing faster than at any time in the previous 20 years. This achievement was the result of minimum-wage hikes across the country (a huge win for labor groups, whose goals once seemed impossible) and historically low unemployment. The U.S. economy had added jobs for more than 100 consecutive months, the longest streak on record, bringing the unemployment rate for black and Latino workers down to its lowest in U.S. history. The federal safety net was, arguably, as strong as it had been in half a century, thanks to the passage of the Affordable Care Act. According to the Congressional Budget Office’s December 2019 report on household income, federal tax-and-transfer policy was doing more to reduce income inequality than at any other time on record, going back to at least 1979. The coronavirus pandemic is poised to halt this progressive momentum. According to the White House, unemployment could hit 20 percent in the next few months. That means the virus may swing the economy from the lowest unemployment rate since the 1950s to the highest rate since the 1930s. According to JP Morgan analysts, GDP could decline in the second quarter by 14 percent. If they’re right, the economy will lurch from the longest expansion on record to the worst quarterly GDP decline on record. Like 102 years ago, this wave of the pandemic will almost certainly disproportionately punish the poor—not only by arresting the long recovery since the Great Recession, but also by specifically targeting industries where workers are most vulnerable and have the least protection. “One month could wipe out 10 years of progress,” in the fight against inequality, Mark Muro, a senior fellow at the Brookings Institution, said. “A huge service-sector recession is coming, and we’re talking about more than 10 million jobs at risk that are often low-wage, low-benefit, or tip-based.” Ground zero for the pandemic’s threat to the labor force are the face-to-face services and leisure economy, much of which have been forcibly shut down by governments to prevent the spread of the virus. Online reservations for restaurants in Los Angeles, San Francisco, and Washington, D.C., have declined to zero. The largest hotel in New York City, the Hilton Midtown on Sixth Avenue, is closing its 1,878 rooms indefinitely, for the first time ever. Disneyland is empty, and the casinos that had always lit the Las Vegas strip have gone dark. The workers in these sectors—salespeople, waiters, hotel desk clerks, groundskeepers, maids, and entertainment attendants—have a few things in common: First, their average annual wages are less than $30,000, including tips, according to the Bureau of Labor Statistics. (The typical annual wage for a full-time worker in the U.S. is about $47,000.) Second, they have the fewest labor protections, such as paid sick leave, and in many cases their tips don’t count as income when they apply for unemployment insurance. Third, they can’t do their work from home. Remote work is the labor market’s only remedy against the virus while Americans are in mass lockdown. There is still so much we don’t know, including exactly how many people will lose their jobs in the next few months. Mark Zandi, the chief economist at Moody’s Analytics, has said that 18 percent of the labor force is at “high-risk”—more than 25 million jobs. That might sound extreme, but a recent NPR/PBS poll found that by March 14, exactly 18 percent of respondents said they had already been let go or had their hours reduced. For Americans making less than $50,000, the number spiked to 25 percent. What seems more certain is that the effects of the pandemic will remain with the U.S. for generations. It may supercharge inequality in the short term. Eventually, however, the pandemic may give birth to a new kind of socialized thinking in America that demands universal insurance and sick leave for all, not just the white-collar remote-work class. For example, Muro believes that national paid leave, if it becomes law, will likely be expanded, because offering paid leave to coronavirus patients in 2020 only to yank it away from cancer patients in 2021 would be too incongruous. When the 1918 influenza pandemic returned for its second and third waves, it didn’t target low-income workers anymore. It simply killed everyone—laborers and bosses, men and women. In the long run, a virus does not discriminate between the rich and the poor. At some point, then, the question must be asked of America’s political leaders: Why do we?
The math behind why we need social distancing, starting right now How just one case of coronavirus could lead to thousands more if we all don’t limit social contact. By Umair Irfan Mar 15, 2020, 1:20pm EDT (click for charts) Health officials and citizens in the United States and Europe are desperately trying to slow the spread of the Covid-19 coronavirus outbreak and are pleading with each other to practice social distancing and good hygiene. Canceling events and family gatherings, closing schools, and reducing visits to public spaces and businesses are simple, powerful, and effective tactics to control disease transmission alongside testing to identify sick patients and isolate them from others. And since the novel coronavirus is already spreading locally in several communities, epidemiologists say measures to limit the spread of Covid-19, the disease caused by the virus, are most effective right now in the early stage of the outbreak, when few people are infected and the most lives can be saved. Conversely, the disease becomes vastly more dangerous the longer the country waits to implement such measures, particularly since it can spread even before an infected person shows symptoms. Countries like China and Italy found this out the hard way when they, seemingly overnight, experienced massive increases in the number of Covid-19 infections and deaths. The math behind the outbreak explains why. Right now, many places seeing Covid-19 transmission are following an exponential growth trajectory. That is, the rate of the spread of the infection is proportional to the number of people infected. Each infected person is expected to infect a certain number of people — around 2.5 right now — who each in turn go on to infect 2.5 more, and on and on, unless drastic measures are taken to reduce social contact and isolate the infected from others. Think about counting doubles — 1, 2, 4, 8, 16 and so on. How many times would you have to double to get to more than 1 million? 20. How many doubles to get to more than 16 million? 24. So that’s why Covid-19 appears to spread very slowly, and then all at once. As epidemiologist Adam Kucharski pointed out on Twitter, this trend also shows how important it is to reduce the rate of infection early. Adam Kucharski@AdamJKucharski Early in outbreak, each COVID-19 case infects ~2.5 others on average. There's ~5 days between one infection and next, so we'd expect one case to lead to 2.5^6 = 244 more cases in a month. If we can halve transmission, so each infects 1.25 others instead, we'd expect 4 more cases. And individual actions can make a big difference. Reuters showed how one Covid-19 patient in South Korea, so-called Patient 31, had direct contact with 1,160 people and created new clusters of the infection. The lesson: Don’t be Patient 31. Of course, Covid-19 can’t spread exponentially forever. There are upper limits to the population and over time, people start to build immunity to the disease and the infected start to recover, so there’s a natural ceiling. As with many other disease outbreaks, there will likely be a peak and decline in the number of cases. Nonetheless, it’s important to lower that peak and spread out the cases as much as possible. That buys valuable time for health providers, allowing them to acquire the tools they need like beds and ventilators while avoiding being overwhelmed by patients. Flattening the curve of cases over time can therefore save lives, and in places where the health care system is already close to capacity, it’s critical. Taking drastic measures to limit the transmission rate of Covid-19, like closing schools, canceling public events, and sending people home from work, may seem like an overreaction when few people in a city or state are infected. However, early in the outbreak is exactly when such measures are most effective. Using data from Hubei province, China, where the virus originated and where the vast majority of the Chinese cases were seen, designer Tomas Pueyo posted on Medium a conceptual model he created showing the difference even one day of social distancing can make in reducing the burden of cases. Vox has recreated the chart here (note that this is based on a theoretical model and not actual reported figures): According to an epidemiological model visualized by the New York Times, one scenario is with no interventions, the number of infections in the United States reach 9.4 million. But with aggressive public health interventions starting today, that peak could fall to 3 million, according to the model. So the sooner such measures start, the bigger impact they will have. Coronavirus math shows the importance of social distancing, and the horrible consequences of not doing it by Marie McCullough, Updated: March 24, 2020 If you still doubt the crucial importance of avoiding other people, or if you think Philadelphia’s “stay at home” order is excessive, consider this: Without the lockdown in Wuhan, China, where the pandemic started, there would have been 44,214 cases in other Chinese cities through February — instead of the actual number of 27,956, according to a new study led by University of Pennsylvania economist Hanming Fang. “Social distancing and, if an epicenter can be identified, as was the case for the city of Wuhan in China, a lockdown can play crucial roles in ‘flattening’ the daily infection cases curve, giving the stressed medical system a chance to regroup and deal with the onslaught of new infection cases,” wrote Fang and his coauthors. “Flatten the curve” has become a rallying cry, but in New Jersey and New York, it appears to be too late. In both states, the average daily rate of increase is more than 50%, according to an Inquirer analysis of data from Sunday. As a trickle of patients has turned into a deluge, New York City hospitals are scrambling to find more ventilators and protective gear, and New Jersey is moving to reopen the former Inspira Medical Center Woodbury in Gloucester County as an intensive care facility. In Pennsylvania, which so far has 644 reported cases, the tally is increasing at an average daily rate of 34%. Though lower, that’s still a growth trajectory that will lead to an overwhelming surge in patients needing hospitalization. And because testing in Pennsylvania has ramped up only in the past week, the growth rate may actually be higher. An analysis by Politico and the COVID19 Tracking Project found that New York has done about 61,400 tests, more than any other state. Pennsylvania has done about 5,400. New Jersey, meanwhile, has done relatively few tests — 2,300 — but more than half have been positive, suggesting that undiagnosed cases are widespread. No one yet knows how readily the virus can be transmitted before it causes symptoms, and asymptomatic transmission has been well documented. Drawing on emerging worldwide data, University of California, San Diego biologist Robert A.J. Signer and art director Gary Warshaw created a graphic that clearly and simply conveys the effect of reducing exposure to the virus through social distancing. Here’s their math: Symptoms of the respiratory illness develop on an average of five days after infection. During those five days, called the incubation period, the virus can spread. If the rate of spread is the same for those with and without symptoms, then one infected person transmits the virus to 2.5 other people on average, and those 2.5 people each transmit to 2.5 more people, and so on. Within 30 days, 406 people would be infected. Social distancing that reduces interaction by 50% would halve the chances of spreading the virus, so one person would only infect 1.25 other people on average, and there would only be 15 infected people in 30 days, Signer calculated. But if people stay home and transmission is cut by 75% — so that an infected person spreads the disease to less than one other person on average — new infections will eventually subside and stop. That’s what has happened in Wuhan, where the 50-day lockdown is now being eased. The Math Behind Social Distancing Symptoms of COVID-19 develop an average of five days after infection. Maybe You’re in the Wrong Country Decisions and blunders made months ago have caused testing disparities worldwide. The science, it turns out, was the easy part. By Matt Apuzzo and Selam Gebrekidan March 20, 2020 Scientists around the world were waiting at their computers in early January when China released the coronavirus genetic code, the blueprint for creating tests and vaccines. Within days, labs from Hong Kong to Berlin had designed tests and shared their research with others. Within about two weeks, Australia had its own tests, and even citizens in the most far-flung regions of the country could be tested. Laboratories in Singapore and South Korea ramped up test kit production and ordered extra supplies. That quick work allowed them to test hundreds of thousands of people, isolate the sick and — so far, at least — contain the spread of the disease. By contrast, anxious citizens in the United States and many parts of Western Europe have endured byzantine delays, or have been denied testing altogether. As the coronavirus pandemic shuts down world capitals and paralyzes entire economies, political leaders are rushing to make testing more widely available. But experts say that the decisive moment, when aggressive testing might have allowed officials to stay ahead of the disease, passed more than a month ago. It was not a question of science. Researchers say a viral test is relatively easy to develop. Rather, scientists say, the chasm between the testing haves and have-nots reflects politics, public health strategies and, in some cases, blunders. The world may be paying for those missteps right now. Testing is central to the effort to fight the spread of the virus. Countries that test widely can isolate infected people and prevent or slow new infections. Without early and widespread testing, health officials and policymakers will be flying blind, epidemiologists say. “You cannot fight a fire blindfolded,” said Tedros Adhanom Ghebreyesus, W.H.O.’s director general. “And we cannot stop this pandemic if we don’t know who is infected.” But testing has been inconsistent in what has been a patchwork response to the epidemic worldwide. Some countries, like France, did not have a strategy that centered on testing to map the advance of the virus. Testing in Italy has been plagued by political squabbles. The United Kingdom developed tests but decided not to use them widely, as Singapore and South Korea had done. Other countries were caught off guard by shortages of testing chemicals. As the virus reached into the United States in late January, President Trump and his administration spent weeks downplaying the potential for an outbreak. The Centers for Disease Control opted to develop its own test rather than rely on private laboratories or the World Health Organization. The outbreak quickly outpaced Mr. Trump’s predictions, and the C.D.C.’s test kits turned out to be flawed, leaving the United States far behind other parts of the world — both technically and politically. In that same period, Singapore was setting up health screenings at airports, issuing work-from-home guidelines and releasing plans to monitor travelers returning from abroad. Independent labs in Korea were rushing their tests out the door. “They were ready, and they just churned out the kits,” said Dr. Jerome Kim, of the International Vaccine Institute in Seoul. Today, the epicenter of the outbreak is Europe and experts say the wave is only starting to hit the United States. Faced with a growing number of cases and limited test kits, many countries have tightened restrictions on who gets tested. In Germany, where the first approved test was developed, only doctors can prescribe one. In France and Belgium, only severely sick patients get tested. In Britain, as in many other countries, the virus is circulating so quickly that it is no longer possible to test people and investigate whom they may have infected, said David McCoy, a public health professor at Queen Mary University in London. Nearly 100 people have died from the virus there. Testing is still valuable in helping scientists understand the epidemiology of the disease, he said. “The window of opportunity to contain the epidemic has now shut,” Mr. McCoy said. ‘This Could Be a Problem’From the beginning, some countries showed greater urgency than others and were more nimble in their response. Australia, Korea and Singapore turned to networks of public and private laboratories to develop tests. On Feb. 4, the South Korean government granted fast-track approval for a company’s coronavirus test and began shipping kits. A second company was approved a week later. Two more soon followed. Australian labs designed a generic test in early January, then refined it after receiving the genome. “We were anticipating early on that we could see cases, that this could be a problem,” said Dr. Jen Kok, a government virologist in New South Wales, Australia, a region where more than 33,000 people have been tested so far. The United States and Britain favored a centralized approach. Britain initially assigned a single lab in north London to perform the tests but, a month later, began allowing other labs to do the same. The C.D.C. had to reverse course, too. After its homegrown test proved faulty, it cost the country valuable time. The Trump administration then had to change tactics, urging outside labs and manufacturers to help make a million tests available. Labs that moved quickly had an advantage. They purchased extra testing products, known as reagents, that extract viral RNA from nose or throat swab samples. Those reagents are now in short supply. “It’s the way we do things here,” said Dr. David Speers, the top microbiologist at PathWest Laboratory Medicine, the government laboratory in Western Australia. “We always try to plan ahead.” Technical speed and laboratory organization, though, do not explain everything. The availability of testing — at least in some countries — also reflects policy. When Australia identified its first coronavirus patient in late January, political leaders made clear that testing would be widespread. “We’re testing people,” Dr. Kerry Chant, the top public health official in New South Wales, said on Jan. 30. “We’re asking people to come forward, and I want to acknowledge the fact that we have had so many people come forward for testing.” Even before the virus began spreading in Singapore, the prime minister, Lee Hsien Loong reminded the public about the 2003 SARS outbreak and said he planned to overreact to the coronavirus. “We have built up our institutions, our plans, our facilities, our stockpiles, our people, our training,” he said on Jan. 31. “Because we knew that one day something like that would happen again.” South Korea opened nearly 600 testing clinics, including dozens of drive-through stations. More than 250,000 people have been tested — far more than any other country that has released data. The country has largely contained its outbreak to the southeast city of Daegu. Most cases are linked to a cluster around the Shincheonji Church of Jesus. “Korea’s approach was: Test everybody,” Dr. Kim said. “Anybody who needs a test should get tested.” The United States, along with countries in Western Europe, chose a different strategy and tone. While Singapore warned that infections were certain to increase, Mr. Trump predicted it would disappear within weeks. In late January, French officials were hesitant to activate emergency protocols because they did not believe that the epidemic was as serious as the 2009 H1N1 outbreak. “We have three cases in France and they are not that severe,” Dr. Patrick Pelloux, of the country’s emergency medical services, said Jan. 25. “It’s an epidemic that is under control.” France says that it is able to test 2,500 cases daily, and health officials said earlier this week that more than 40,000 people had been tested. The United States has run about 25,000 tests. Neither country has contained the virus or tested aggressively for it. Korea and Singapore have so far been able to do both. “We were not just looking at having a very good diagnostics test. That’s kind of a given. You can’t do anything without that,” said Dr. Sidney Yee, the chief executive of Singapore’s Diagnostics Development Hub. “We were also looking at getting people prepared and getting accurate messages out.” Nicolas Locker, a professor of virology at the University of Surrey in Britain, said national leaders set the tone. “What you’re seeing today is the impact of those earlier comments, and that earlier attitude,” Dr. Locker said. Incorrect Assumptions Testing can be as much a political issue as a scientific one. Italy, the site of the biggest outbreak outside China, is a prime example. At first, regional authorities in the north tested widely and tried to trace contacts with sick people. But the national government in Rome objected, saying there was no need to test people who did not exhibit symptoms. “Someone said we’re testing too many people and this is why we have such a huge number. That is not true,” Giovanni Rezza, director of the department of infectious diseases at the Italian National Institute of Health. Under pressure, the regional governments began testing only patients who exhibited symptoms. Politicians and scientists continue to debate those protocols, Dr. Rezza said. Still, the country has managed to test more than 182,000 people. Britain was one of the first to develop coronavirus diagnostic kits but made a decision not to test widely. The government’s strategy initially focused on slowing the contagion rather than stopping it. The government, though, severely underestimated the potential scope of the epidemic, according to a study published on Monday. Prime Minister Boris Johnson’s government recently reversed its strategy and decided to widen testing. Mr. Johnson told Parliament on Wednesday that his government will have the ability to conduct 25,000 tests a day. But raw numbers ignore the effect of timing. South Korea deployed its tests early and alongside other approaches, including some that European populations might resist. A government app monitored people to ensure they remained quarantined. Police officers used surveillance camera footage, phone data and credit card records to recreate the movements of new patients and identify potential contacts. Kim Gang-lip, a South Korean vice health minister, said the contagiousness of the disease and its rapid spread demanded a new approach. “Such characteristics of the virus render the traditional response, which emphasizes lockdown and isolation, ineffective,” he said. A New Testing Reality Lockdown and isolation are a reality today for tens of millions of people. Italy is at a standstill. Europe has all but shut its borders. President Emmanuel Macron of France told people to stay at home for 15 days and ordered the army to transport the sick to hospitals. Mr. Trump recommended against all but the smallest gatherings. With no treatment for the disease, many countries are telling sick people to stay home unless they become seriously ill. Hospitals cannot afford to be overwhelmed by nervous people asking for tests. But patients who self-quarantine likely won’t ever be tested, making it difficult to know the true scope of the disease. And as the disease spreads, the practicality of testing declines, as does its value. “Testing of contacts, I believe, will be totally out of control very soon,” said Manfred Green, an epidemiologist with the University of Haifa in Israel. Australian officials say they, too, worry about wasting tests on the merely worried. They recently adjusted testing protocols, but remain aggressive. Anyone who has recently been out of the country and so much as spikes a fever will likely be tested. “We are still in the containment phase,” said Dr. Kok. “We’re testing really widely.” Matt Apuzzo reported from Brussels, Selam Gebrekidan from London.
