    5/22/20 - Antimalarial drug touted by President Trump is linked to increased risk of death in coronavirus patients, study says. An analysis of 96,000 patients shows those treated with hydroxychloroquine were also more likely to suffer irregular heart rhythms. By Ariana Eunjung Cha and Laurie McGinley May 22, 2020 at 1:41 p.m. PDT A study of 96,000 hospitalized coronavirus patients on six continents found that those who received an antimalarial drug promoted by President Trump as a “game changer” in the fight against the virus had a significantly higher risk of death compared with those who did not. People treated with hydroxychloroquine, or the closely related drug chloroquine, were also more likely to develop a type of irregular heart rhythm, or arrhythmia, that can lead to sudden cardiac death, it concluded. The study, published Friday in the medical journal Lancet is the largest analysis to date of the risks and benefits of treating covid-19 patients with antimalarial drugs. Like earlier smaller studies, it delivered disappointing news to a world eager for promising treatments for the novel coronavirus as the global death toll grows to more than 300,000. While doctors have refined how they treat the disease, they have yet to discover a magic bullet against a pathogen for which humans have no known immunity. “It’s one thing not to have benefit, but this shows distinct harm,” said Eric Topol, a cardiologist and director of the Scripps Research Translational Institute. “If there was ever hope for this drug, this is the death of it.” David Maron, director of preventive cardiology at the Stanford University School of Medicine, said that “these findings provide absolutely no reason for optimism that these drugs might be useful in the prevention or treatment of covid-19.” President Trump stunned many doctors earlier this week when he said he was taking hydroxychloroquine “every day” — despite FDA warnings that the use of the drug should be limited to those in a hospital setting or in clinical trials. He has since said he is close to finishing his course of treatment and would stop taking the medication in “a day or two.” At a White House press briefing Friday, Deborah Birx, who is coordinating the government’s coronavirus response, deflected a question about the president’s use of the drug, and added the Food and Drug Administration “has been very clear on their website about their concerns about hydroxychloroquine." The Lancet analysis — by Mandeep Mehra, a Harvard Medical School professor and physician at Brigham and Women’s Hospital, and colleagues at other institutions — included patients with a positive laboratory test for covid-19 who were hospitalized between Dec. 20, 2019, and April 14, 2020, at 671 medical centers worldwide. The mean age was 54 years, and 53 percent were men. Those who were on mechanical ventilators or who received remdesivir, an antiviral drug made by Gilead Sciences that has shown promise in decreasing recovery times, were excluded. Mehra said in an interview that the widespread use of antimalarials for covid-19 patients was based on the idea of “a desperate disease demands desperate measures,” but that we have learned a hard lesson from the experience about the importance of first doing no harm. In retrospect, Mehra said, using the drugs without systematic testing was “unwise.” “I wish we had had this information at the outset, as there has potentially been harm to patients,” he said. Nearly 15,000 of the 96,000 patients in the analysis were treated with hydroxychloroquine or chloroquine alone or in combination with a type of antibiotics known as a macrolide, such as azithromycin or clarithromycin, within 48 hours of their diagnosis. The study is a retrospective analysis of their medical records, rather than a controlled study in which patients are divided randomly into treatment groups — the method considered the gold standard of medicine. But the sheer size of the study was convincing to some scientists. And the difference between those who received the antimalarials and those who did not was striking. For those given hydroxychloroquine, there was a 34 percent increase in risk of mortality and a 137 percent increased risk of a serious heart arrhythmias. For those receiving hydroxychloroquine and an antibiotic — the cocktail endorsed by Trump — there was a 45 percent increased risk of death and a 411 percent increased risk of serious heart arrhythmias. Those given chloroquine had a 37 percent increased risk of death and a 256 percent increased risk of serious heart arrhythmias. For those taking chloroquine and an antibiotic, there was a 37 percent increased risk of death and a 301 percent increased risk of serious heart arrhythmias. Cardiologist Steven Nissen of the Cleveland Clinic said the new data, combined with data from smaller previous studies, suggests that the drug “is maybe harmful and that no one should be taking it outside of a clinical trial.” Jesse Goodman, a former FDA chief scientist who is now a Georgetown University professor, called the report “very concerning.” He noted, however, that it is an observational study, rather than a randomized controlled trial, so it shows correlation between the drugs and certain outcomes, rather than a clear cause and effect. Peter Lurie, a former top FDA official who now heads the Center for Science in the Public Interest, called the report “another nail in the coffin for hydroxychloroquine — this time from the largest study ever.” He said it was time to revoke the emergency use authorization issued by the FDA, which approved the drug for seriously ill patients who were hospitalized or for whom a clinical trial was not available. FDA spokesman Michael Felberbaum said Friday the agency generally “does not comment on third-party research” but that an emergency use authorization may be revised or revoked under certain circumstances, such as when there are linked or suspected adverse events, new data about effectiveness, or changes in the risk-benefit assessment of the drug. Last month, the agency warned against the use of the drug outside hospital settings or clinical trials citing reports of “serious heart rhythm problems.” There have been at least 13 studies in recent months on hydroxychloroquine or chloroquine as a treatment for covid-19 patients. They have included randomized controlled studies and observational analyses encompassing patients on the continuum from mild illness to those near death. Evidence of any benefit, such as viral clearance or improved symptoms, has been almost nonexistent. But many found an increased risk in adverse cardiac reactions — especially when combined with the antibiotic azithromycin. Earlier this month, backers of the president’s views on antimalarials seized on a study out of NYU Langone Health that gave hospitalized coronavirus patients zinc combined with hydroxychloroquine and azithromycin. Those in the treated group had a higher rate of survival, but researchers emphasized the study was focused on zinc and whether hydroxychloroquine would enhance the supplement’s anti-viral effects. They emphasized their findings were tentative and could have been due to factors other than the drugs, especially since zinc was added to patients’ regimens later in the pandemic when hospital treatments and procedures had been refined. Last week, the National Institutes of Health announced a randomized, placebo-controlled clinical trial of hydroxychloroquine and azithromycin involving 2,000 adults dealing with their illness at home. In April, NIH launched a different randomized trial of the drug cocktail in hospitalized patients. Topol, of the Scripps Research Translational Institute, suggested that the researchers should reconsider the ethics of those trials, given the increasing evidence of potential harm. “It’s very hard to ignore that signal, and it’s worrisome to continue giving it,” he said. Geoffrey Barnes, a cardiovascular specialist at the University of Michigan, said the study’s approach and its findings were “striking” in making the case that “the risk with these drugs is real.” However, he said that due to the enthusiasm some Americans have for the drug and the Lancet study’s findings, randomized trials are even more important. “There has been so much discussion about this drug that I think the scientific and medical community has an obligation to define what the potential benefit or risk is in the best way possible,” Barnes said. Some scientists have expressed alarm at the politicization of science in the debate over the use of antimalarial drugs. The Lancet, one of the world’s oldest and best-known journals, also published an unsigned editorial last week criticizing Trump’s “inconsistent and incoherent national response” to the pandemic, and urging Americans to “put a president in the White House come January, 2021, who will understand that public health should not be guided by partisan politics.” When the first large wave of sick patients began showing up at hospitals in March, doctors had very little to offer them. As a result, many took a gamble on hydroxychloroquine. The drug had been shown to have strong antiviral properties in cell cultures, was widely available and was thought to be rather benign in terms of side effects. For years, hydroxychloroquine has been considered a generally safe and effective treatment for malaria, lupus and rheumatoid arthritis. But those findings of safety were at lower doses than were being used at hospitals during the early days of the surge in patients in the United States and mostly in patients who were healthy. The population infected with covid-19 in hospitals, it turned out, was already at higher risk of cardiovascular complications because many suffer from high blood pressure or other heart issues. Doctors also discovered that, to their surprise, the novel coronavirus appeared to directly or indirectly attack the heart, including by reducing its ability to pump, creating an imbalance in its electrical rhythms, and attacking blood vessels. The N95 masks have been in high demand since the pandemic hit the United States and as health care workers scramble to protect themselves while caring for thousands of patients flooding local hospitals. On Jan. 22, Prestige Ameritech, a medical supply company in Fort Worth, Texas, offered to ramp up production to make an additional 1.7 million N95 masks, noting that the federal government’s stockpile was diminishing. The government turned down the offer. “I don’t believe we as a government are anywhere near answering those questions for you yet,” Laura Wolf, director of the agency’s Division of Critical Infrastructure Protection, responded the same day Prestige inquired about its stockpile, according to the Post. Michael Bowen, who owns the company, told HHS his company was receiving various requests from private entities but would be willing to set masks aside for the government. “We are the last major domestic mask company,” he wrote on Jan. 23, according to the Post. “My phones are ringing now, so I don’t ‘need’ government business. Who wrote the Gospels? The story of Jesus of Nazareth is told in the New Testament by four different authors (see Fig. 4(c)). The first four books of the New Testament are called ‘gospels’ because they contain the ‘good news’ about Jesus’s life, death and resurrection. The four authors – traditionally identified as Matthew, Mark, Luke and John – had different audiences in mind when they wrote, so the accounts differ quite markedly in approach. Matthew, Mark and Luke set out to present a description of Jesus’s life and death roughly in chronological order. As, together, they see Jesus’s life in broadly the same way, they are known as the ‘synoptic’ (meaning ‘seeing together’) gospels. John was more concerned with explaining Jesus’s teachings and theology, so he has a more thematic approach and makes little attempt to present his gospel in chronological order. Matthew’s Gospel is traditionally believed to have been written by Levi, a Jew from Galilee who collected taxes for the Romans, and whose Greek name was Matthew (see Matthew 9:9-13 & Mark 2:13-17). He was one of Jesus’s first ‘apostles’ – his close circle of twelve followers or ‘disciples’ – who were ‘sent out’ with the Good News (see Matthew 10:2-5). Matthew’s account of Jesus’s life was written between 70 and 80AD primarily for Jewish readers. Its particular emphasis was to persuade its readers that Jesus really was the Messiah or Christ – the ‘anointed one’ promised by the Old Testament prophets. As a result, many passages in Matthew’s gospel set out to demonstrate that Jesus fulfilled the expectations that Jews had of the Messiah in New Testament times (see the feature on Who was the Messiah? in Section 2). Matthew therefore begins his narrative with a lengthy genealogy to establish that Jesus was a descendent of King David (see Isaiah 9:7 & Matthew 1:1-17). He tells about the ‘magi’ (astrologers or ‘wise men’ from Mesopotamia) who came to Bethlehem after Jesus’s birth seeking the new ‘King of the Jews’ (see Micah 5:2 & Matthew 2:1-12). He then endeavours to show how Jesus was both a ‘prophet like Moses’ (see Deuteronomy 18:15-18 & Matthew 21:11) and the promised ‘Son of Man’ (see Daniel 7:13-14 & Matthew 11:18-19). Matthew’s overriding theme is that Jesus was the ‘Christ’ or ‘Messiah’, foretold by the Jewish prophets, who fulfilled the Law of Moses (see Matthew 5:17-20). Mark’s Gospel, the shortest of the four gospels, is widely thought to be the first one to have been written. Over a third of his account concentrates on the events surrounding Jesus’s death and resurrection. It was probably written as early as 62AD (a little over thirty years after the death and resurrection of Jesus) while Mark was staying with Paul in Rome. It is considered to be the earliest gospel because Luke and Matthew appear to borrow a considerable amount of their information from Mark’s narrative. About ninety per cent of Mark’s narrative is repeated in Matthew’s gospel, while Luke includes over half of Mark’s content. Mark’s early account of Jesus’s life is widely believed to have been written by John Mark, a Jewish member of the early Christian community in Jerusalem, at whose home the first Christian believers met (see Acts 12:12). John Mark, as a teenager, may have known Jesus well. The Upper Room where Jesus’s ‘Last Supper’ was held may have been at Mark’s house. Many scholars think that the young man whom Mark says escaped from the Garden of Gethsemane when Jesus was arrested (see Mark 14:51-52) was John Mark himself. After Jesus’s death, Mark would have heard first-hand accounts of the life and teachings of Jesus from Peter and the other apostles. As a young man, he travelled across Cyprus with Paul and Barnabas (who was his uncle) in 46AD, before returning to Jerusalem (see Acts 12:25 & 15:37-39). He later spent two years in Rome (60-62AD) when Paul was under house arrest awaiting his trial before Emperor Nero. It is likely that Mark’s Gospel was written around this time, relying, for accuracy, on the accounts that the author had heard from Peter and the other apostles. Mark was later sent by Paul to the church at Colossae, but Paul asked Timothy to bring Mark back from Colossae to Rome just before his death in c.67AD (see Colossians 4:10 & 2 Timothy 4:11). We know that John Mark arrived back in Rome about this time as Peter sends greetings from Mark in his First Letter to the believers in Asia Minor, written in c.67AD shortly before Peter was executed (see 1 Peter 5:13). As Mark goes out of his way to explain Jewish customs (see Mark 7:3-4 & 15:42), he was probably addressing an audience that included Gentiles, and may well have written his ‘Good News’ for the believers in Rome. Luke’s Gospel appears to rely heavily on information from Mark’s gospel, as well as containing some additional material from other sources (sometimes referred to by Biblical scholars as ‘Q’ or ‘Quelle’, German for ‘source’). Luke was a Gentile (non-Jewish) doctor, who was a close companion of Paul. He is the only non-Jewish writer whose work is found in the Bible. He first joined Paul in 51AD at Troas on his second missionary journey (hence the ‘we’ passages that Luke wrote following Acts 16:11) and may well have lived in Philippi where he stayed after Paul and Silas were forced to leave (see Acts 16:40). Some scholars believe that Luke was the ‘Man from Macedonia’ who appeared in Paul’s dream and begged Paul to visit his homeland (see Acts 16:9). Luke wrote his gospel for a Gentile audience, having been asked for an account of Jesus’s life and teachings by a Roman friend he calls ‘Theophilus’ (‘lover of God’) (see Luke 1:1-4). Luke stresses that Jesus was the saviour of all mankind, whatever their background, their gender or their nationality. He also wrote a second book for ‘Theophilus’ about the Acts of the Apostles (see Acts 1:1-2). Luke, a Roman citizen, later accompanied Paul from Philippi to Jerusalem in 57AD after his third missionary journey (see the ‘we’ passages following Acts 20:6). He stayed for two years with the believers in Jerusalem and Caesarea, during which time he probably met on many occasions with the Jewish Christian community at the home of John Mark in Jerusalem. Together with Aristarchus, he then travelled with Paul to Rome in 60AD. The next two years were spent with Paul in Rome while Paul was under house arrest awaiting his trial before Nero (see 2 Timothy 4:11). It was probably during this two years in Rome (60-62AD) that Luke wrote his gospel. He appears to have completed his gospel and the Acts of the Apostles before 64AD, as his account of the apostles’ activities ends before the Great Fire of Rome in 64AD and the outbreak of the Jewish War in 66AD. As the Acts of the Apostles makes no mention of the outcome of Paul’s appeal to Nero that was heard in c.62AD, it was probably written before the result was known. It follows that Luke probably wrote his gospel and the Acts of the Apostles while staying with Paul in Rome between 60 and 62AD. As it’s likely that John Mark was also staying with Paul around the same time, this may help to explain why large parts of the gospels of Luke and Mark are very similar. Most of the difference in emphasis is accounted for by the fact that Mark’s gospel is written from the perspective of a Jewish Christian, while Luke wrote as a Gentile believer. John’s Gospel is quite different from the other three gospels. It is traditionally believed to have been written by John, one of Jesus’s close circle of twelve apostles, in c.85AD when John, a leader of the Christian community in Ephesus, was an old man. It was written to denounce and disprove a heretical (false) teaching known by scholars as ‘Gnosticism’. Gnostics taught that Jesus was human, but was not divine. John set out to show that Jesus was both human and divine – still a fundamental belief of Christians today. John wrote three letters to Christian believers between 85 and 88AD in order to denounce the teachings of Gnostics (see 1 John 1:1-4, 2 John 1:7-11 & 3 John 1:3-4), and he probably wrote his gospel very shortly before this in c.85AD. The emphasis of John’s gospel is that Jesus – the ‘Word of God’ who has always co-existed with God – was an amazing revelation of God himself in human form (see John 1:1-14). John’s gospel is therefore unique, and is fascinating to read alongside the other more conventional gospel accounts of Jesus’s life and teaching. After completing his gospel and his three letters to the daughter churches in the region around Ephesus, John was exiled by the Roman emperor Domitian to the island of Patmos. Here, in c.90AD, he wrote the Book of Revelation – a revelation he received from Jesus Christ about the ‘last days’ (see Revelation 1:1-3). He returned to Ephesus in c.96AD, where he died and was buried. Bible scholar Bart Ehrman began his studies at the Moody Bible Institute in Chicago. Originally an evangelical Christian, Ehrman believed that the Bible was the inerrant word of God. But later, as a student at Princeton Theological Seminary, Ehrman started reading the Bible with a more historical approach and analyzing contradictions in the Gospels.
Ehrman, the author of Jesus, Interrupted: Revealing the Hidden Contradictions in the Bible (and Why We Don't Know About Them), tells Terry Gross that he discourages readers from "smash[ing] the four Gospels into one big Gospel and think[ing] that [they] get the true understanding." "When Matthew was writing, he didn't intend for somebody ... to interpret his Gospel in light of what some other author said. He had his own message," Ehrman says. To illustrate the differences between the Gospels, Ehrman offers opposing depictions of Jesus talking about himself. In the book of John, Jesus talks about himself and proclaims who he is, saying "I am the bread of life." Whereas in Mark, Jesus teaches principally about the coming kingdom and hardly ever mentions himself directly. These differences offer clues into the perspectives of the authors, and the eras in which they wrote their respective Gospels, according to Ehrman. "In Mark's Gospel, Jesus is not interested in teaching about himself. But when you read John's Gospel, that's virtually the only thing Jesus talks about is who he is, what his identity is, where he came from," Ehrman says. "This is completely unlike anything that you find in Mark or in Matthew and Luke. And historically it creates all sorts of problems, because if the historical Jesus actually went around saying that he was God, it's very hard to believe that Matthew, Mark and Luke left out that part — you know, as if that part wasn't important to mention. But in fact, they don't mention it. And so this view of the divinity of Jesus on his own lips is found only in our latest Gospel, the Gospel of John." How could that be? Don't these books all have the authors' names attached to them? Matthew, Mark, Luke, John, the letters of Paul, 1 and 2 Peter, and 1, 2 and 3 John? How could the wrong names be attached to books of Scripture? Isn't this the Word of God? If someone wrote a book claiming to be Paul while knowing full well that he wasn't Paul — isn't that lying? Can Scripture contain lies? When I arrived at seminary I was fully armed and ready for the onslaught on my faith by liberal biblical scholars who were going to insist on such crazy ideas. Having been trained in conservative circles, I knew that these views were standard fare at places like Princeton Theological Seminary. But what did they know? Bunch of liberals. What came as a shock to me over time was just how little actual evidence there is for the traditional ascriptions of authorship that I had always taken for granted, and how much real evidence there was that many of these ascriptions are wrong. It turned out the liberals actually had something to say and had evidence to back it up; they weren't simply involved in destructive wishful thinking. There were some books, such as the Gospels, that had been written anonymously, only later to be ascribed to certain authors who probably did not write them (apostles and friends of the apostles). Other books were written by authors who flat out claimed to be someone they weren't. In this chapter I'd like to explain what that evidence is. Who Wrote The Gospels? Though it is evidently not the sort of thing pastors normally tell their congregations, for over a century there has been a broad consensus among scholars that many of the books of the New Testament were not written by the people whose names are attached to them. So if that is the case, who did write them? Preliminary Observations: The Gospels as Eyewitness Accounts As we have just seen, the Gospels are filled with discrepancies large and small. Why are there so many differences among the four Gospels? These books are called Matthew, Mark, Luke, and John because they were traditionally thought to have been written by Matthew, a disciple who was a tax collector; John, the "Beloved Disciple" mentioned in the Fourth Gospel; Mark, the secretary of the disciple Peter; and Luke, the traveling companion of Paul. These traditions can be traced back to about a century after the books were written. But if Matthew and John were both written by earthly disciples of Jesus, why are they so very different, on all sorts of levels? Why do they contain so many contradictions? Why do they have such fundamentally different views of who Jesus was? In Matthew, Jesus comes into being when he is conceived, or born, of a virgin; in John, Jesus is the incarnate Word of God who was with God in the beginning and through whom the universe was made. In Matthew, there is not a word about Jesus being God; in John, that's precisely who he is. In Matthew, Jesus teaches about the coming kingdom of God and almost never about himself (and never that he is divine); in John, Jesus teaches almost exclusively about himself, especially his divinity. In Matthew, Jesus refuses to perform miracles in order to prove his identity; in John, that is practically the only reason he does miracles. Did two of the earthly followers of Jesus really have such radically different understandings of who he was? It is possible. Two people who served in the administration of George W. Bush may well have radically different views about him (although I doubt anyone would call him divine). This raises an important methodological point that I want to stress before discussing the evidence for the authorship of the Gospels. Why did the tradition eventually arise that these books were written by apostles and companions of the apostles? In part it was in order to assure readers that they were written by eyewitnesses and companions of eyewitnesses. An eyewitness could be trusted to relate the truth of what actually happened in Jesus' life. But the reality is that eyewitnesses cannot be trusted to give historically accurate accounts. They never could be trusted and can't be trusted still. If eyewitnesses always gave historically accurate accounts, we would have no need for law courts. If we needed to find out what actually happened when a crime was committed, we could just ask someone. Real-life legal cases require multiple eyewitnesses, because eyewitnesses' testimonies differ. If two eyewitnesses in a court of law were to differ as much as Matthew and John, imagine how hard it would be to reach a judgment. A further reality is that all the Gospels were written anonymously, and none of the writers claims to be an eyewitness. Names are attached to the titles of the Gospels ("the Gospel according to Matthew"), but these titles are later additions to the Gospels, provided by editors and scribes to inform readers who the editors thought were the authorities behind the different versions. That the titles are not original to the Gospels themselves should be clear upon some simple reflection. Whoever wrote Matthew did not call it "The Gospel according to Matthew." The persons who gave it that title are telling you who, in their opinion, wrote it. Authors never title their books "according to." Moreover, Matthew's Gospel is written completely in the third person, about what "they" — Jesus and the disciples — were doing, never about what "we" — Jesus and the rest of us — were doing. Even when this Gospel narrates the event of Matthew being called to become a disciple, it talks about "him," not about "me." Read the account for yourself (Matthew 9:9). There's not a thing in it that would make you suspect the author is talking about himself. With John it is even more clear. At the end of the Gospel the author says of the "Beloved Disciple": "This is the disciple who is testifying to these things and has written them, and we know that his testimony is true" (John 21:24). Note how the author differentiates between his source of information, "the disciple who testifies," and himself: "we know that his testimony is true." He/we: this author is not the disciple. He claims to have gotten some of his information from the disciple. As for the other Gospels, Mark was said to be not a disciple but a companion of Peter, and Luke was a companion of Paul, who also was not a disciple. Even if they had been disciples, it would not guarantee the objectivity or truthfulness of their stories. But in fact none of the writers was an eyewitness, and none of them claims to be.
