Pronounced “R naught,” this isn’t just jargon made up in Hollywood. It represents an important concept in epidemiology and is a crucial part of public health planning during an outbreak, like the current coronavirus epidemic spreading outward from China. Scientists use R0—the reproduction number—to describe the intensity of an infectious disease outbreak. R0 estimates have been an important part of characterizing pandemics or large publicized outbreaks, including the 2003 SARS pandemic, the 2009 H1N1 Influenza pandemic and the 2014 Ebola epidemic in West Africa. How much will a disease spread? The formal definition of a disease’s R0 is the number of cases, on average, an infected person will cause during their infectious period. The term is used in two different ways. The basic reproduction number represents the maximum epidemic potential of a pathogen. It describes what would happen if an infectious person were to enter a fully susceptible community, and therefore is an estimate based on an idealized scenario. The effective reproduction number depends on the population’s current susceptibility. This measure of transmission potential is likely lower than the basic reproduction number, based on factors like whether some of the people are vaccinated against the disease, or whether some people have immunity due to prior exposure with the pathogen. Therefore, the effective R0 changes over time and is an estimate based on a more realistic situation within the population. It’s important to realize that both the basic and effective R0 are situation-dependent. It’s affected by the properties of the pathogen, such as how infectious it is. It’s affected by the host population – for instance, how susceptible people are due to nutritional status or other illnesses that may compromise one’s immune system. And it’s affected by the environment, including things like demographics, socioeconomic and climatic factors. For example, R0 for measles ranges from 12 to 18, depending on factors like population density and life expectancy. This is a large R0, mainly because the measles virus is highly infectious. On the other hand, the influenza virus is less infectious, with its R0 ranging from 2 to 3. Influenza, therefore, does not cause the same explosive outbreaks as measles, but it persists due to its ability to mutate and evade the human immune system. How contagious is the virus?It seems to spread very easily, making containment efforts difficult. The scale of an outbreak depends on how quickly and easily a virus is transmitted from person to person. The new coronavirus seems to spread very easily, especially in homes, hospitals, churches, cruise ships and other confined spaces. It is much more contagious than SARS, another coronavirus that circulated in China in 2003 and sickened about 8,000 people. The pathogen can travel through the air, enveloped in tiny respiratory droplets that are produced when a sick person breathes, talks, coughs or sneezes. These droplets fall to the ground within a few feet. That makes the virus harder to get than pathogens like measles, chickenpox and tuberculosis, which can travel 100 feet through the air. Research is still in its early stages, but some estimates suggest that each person with the new coronavirus could infect between two and four people without effective containment measures. That is enough to sustain and accelate an outbreak, if nothing is done to reduce it. Here’s how that works. In the animation below, a group of five infected people could spread the virus to about 368 people over just five cycles of infection. Compare that with a less contagious virus, like the seasonal flu, which can be slowed by vaccines and immunity from past epidemics. People with the flu tend to infect 1.3 other individuals, on average. The difference may seem small, but the result is a striking contrast: Only about 45 people might be infected in the same scenario. The transmission numbers of any disease aren’t set in stone. They can change depending on how much people interact at school, work or religious gatherings. When global health authorities methodically tracked and isolated people infected with SARS in 2003, they were able to bring the average number each sick person infected down to 0.4, enough to stop the outbreak. Health authorities around the world are expending enormous effort trying to repeat that. But the number of people infected globally is rising quickly, with large clusters of cases in Italy, Iran, Japan and South Korea. The virus’s high rate of transmission means containment measures — such as wearing masks, keeping a distance from infected people and implementing quarantines if people are exposed — must block more than 60 percent of transmissions in order to effectively control the outbreak, which is difficult. Faulty COVID-19 Test Kits Tilt Scales in Coronavirus' Favor Flawed coronavirus test kits from the Centers for Disease Control severely limited the number of U.S. patients screened in January and February. Instead of quickly testing every individual known to have been exposed to SARS-CoV-2, the coronavirus that causes COVID-19, just 3,600 were tested during January and February. A world-class bottleneck. The first misstep along the path to what could become an uncontained outbreak in this country belonged to the Centers for Disease Control. Instead of using testing kits already provided by the World Health Organization, the CDC decided to create its own. Unfortunately, there have been problems with the test kits sent out by the CDC that rendered most of them useless. The CDC didn't begin sending testing kits to hospitals until Feb. 4, which means thousands of blood samples were sent to one of just 12 approved testing facilities. Tightening the bottleneck even further, all results needed to be certified by the CDC's central lab in Atlanta due to reagents that failed initial quality checks. In the EU, a U.S. company called Co-Diagnostics (NASDAQ:CODX) has already launched a COVID-19 test available in every member state. Meanwhile, in the U.S., hospitals weren't able to use tests they already had access to until the beginning of March. Sleepless in New York and Seattle It wasn't until Tuesday that the public learned that a person who died in a Seattle hospital last week had a COVID-19 infection when they were admitted to that facility on Feb. 24. Several other coronavirus cases have been diagnosed among residents of the nursing home where that patient lived. Covid-19 May Have You Working At Home Preventing the virus’s spread will likely require ‘social distancing’ tactics like canceling events. By Luciana Borio and Scott Gottlieb March 3, 2020 6:49 pm ET The numbers don’t tell the whole story. Nine Americans have died of Covid-19, and the Centers for Disease Control and Prevention confirms more than 100 cases in 15 states. But there are probably already several thousand Americans who have been infected. For now, the average person’s risk is low in a nation of 330 million, but that may soon change. The highest risk is for those who live in regional hot zones such as Washington state or Northern California. Measures that limit social activity may soon be in place. China tried to curb transmission by shutting down entire cities and mandatory mass quarantines. That won’t fly in the U.S., socially or legally. Iran is taking the opposite approach amid what may be a wider epidemic. Tehran appears resigned to letting the virus rage through the country’s population. Iran’s official reports say there are 2,300 cases in the country with 77 deaths, but the true number is likely much higher. There’s still a chance to delay an American epidemic, though full containment is no longer possible. The key is to develop a strategy of mitigation, which will buy time to prepare hospitals, expand testing, and develop vaccines and therapies. This doesn’t mean the U.S. needs to ban people from moving freely in the country, which has happened in China and even Italy. Public-health authorities must develop an approach suited to American law, technology and social structures. The original priority was tightening travel to and from China. But travel restrictions and advisories will become obsolete as the number of affected countries grows. Any of the thousands of airplanes that land in the U.S. each day could be carrying people who are infected. Airports should remind incoming travelers to isolate themselves and seek medical care if they develop symptoms. Decisions to travel outside the U.S. will largely depend on factors such as individual’s risk tolerance, health status, and the ability to access medical care in a foreign country. As the number of cases expands, other time-worn public-health measures for containing disease—detecting infected individuals and tracing their contacts—will no longer be enough. Slowing down community spread may require aggressive steps aimed at “social distancing”—keeping people who are sick away from others. The CDC provides guidance to help states and cities put these measures into place—for example, canceling mass gatherings or closing schools and conducting classes online. But it’s up to local authorities to decide what measures to implement. A patchwork of policies may initially confuse the public. The usual flu-season advice applies: Wash your hands frequently, avoid handshakes, and try not to touch your face. Most important, stay home when you or someone in your household is sick. For home isolation and quarantine to work, employers need to be as understanding and flexible as possible. They can help by holding videoconferences instead of meetings and letting employees work from home. Some may consider alternative schedules or staggered shifts. These measures will be most effective if applied early and widely. Singapore suspended mass gatherings at schools and eldercare facilities immediately after detecting local disease transmission. This appears to have slowed the number of new infections. But it is important to acknowledge that these measures can cost workers money and even their jobs. The coming weeks will be hard as more Americans become infected and some die of the disease. Mitigation efforts will be disruptive. But these steps are the best defense until innovation can produce an effective treatment or vaccine that can arrest the virus’s spread. China is trying to get people back to work, risking a renewed spread of the coronavirus. Central and local governments are loosening the criteria for factories to resume operations as they walk a tightrope between containing a virus that has killed almost 2,600 people and preventing a slump in the world’s second-largest economy. The rush to restart has been propelled by China’s leader Xi Jinping and top leaders, who are urging companies to resume production so the country can continue to meet lofty goals for growth and economic development in 2020. At stake are the fates of millions of Chinese businesses facing collapse because of the shutdowns, and the ability of companies across the globe from Apple Inc. to Nissan Motor Co. to access crucial components. Officials in China’s provinces have taken up Xi’s call, with one region after another relaxing rules that had kept more than half the nation’s industrial base idle following the Lunar New Year holiday. After weeks of empty streets and shuttered shops, signs of life are emerging along the manufacturing belt in the country’s coastal regions. On Monday, at least six provinces lowered their emergency response levels from the highest rating. Conflicting Messages Chinese authorities, however, reiterated that Wuhan -- the epicenter of the outbreak -- will stay locked down, retracting a statement sent earlier in the day that had announced partial easing of travel curbs. The conflicting messages underscore the dilemma China faces on how to spur its ailing economy and curb the deadly virus. About 600 kilometers east of Wuhan, vendors and customers at the Yiwu wholesale market in Zhejiang province are having their body temperatures tested at the entrances after the vast complex that wholesales manufactured goods reopened last Tuesday, three days earlier than expected. Power demand has also started to pick up in China, with six major generators reporting that coal consumption -- while still below pre-holiday levels -- rose 7% on Feb. 20 from the previous day. But labor is still a big issue for many. “Our factory is still missing quite a lot of workers, so we can only resume limited production,“ said Dong Liu, vice president of a textile manufacturer in Fujian, southeastern China, that employs more than 400 workers. Dong said he applied to the government on Feb. 17 to restart and the inspector came the next day and gave permission. “More and more factories are allowed to reopen this week,” he said. The push to get production rolling again risks a renewed spread of the virus, about which much is still not yet known. While more than 79,000 people have been infected worldwide, the vast majority of those cases are in seven Chinese provinces, and mostly in the central province of Hubei, where restrictions on movement were imposed in a number of cities, but not before a lot of people had already left the region for the New Year break. “A peak may come at the end of this month for the whole country but it won’t necessarily indicate a turning point,” Zhong Nanshan, a respiratory disease expert who led research into a treatment for SARS, told reporters in Guangzhou. . . . . . . . . . . . . “The epidemic could have a new peak after people travel back to work.” Coronavirus Impact: This Chart Shows Chinese Nowhere Near Back To Work (Clickie for Charts) 3/2/20 One indicator that shows factories are indeed turning the lights back on is coal consumption data from six major power plants. Consumption is down by roughly a third, an improvement from two weeks ago before president Xi Jinping said he felt provinces not as impacted by the disease should prepare to get back to work. Many business offices have remained open as workers were able to work remotely. According to Zhang Kejian, a vice minister of the Ministry of Industry and Information Technology, 32.8% of small to mid-sized enterprises have resumed operation as of February 26. Small and mid-sized Chinese businesses account for around 60% of China’s GDP, 80% of urban employment and 90% of the number of firms. According to a media briefing on Saturday regarding the Manufacturing PMI data, which hit a 16-year low, the spokesman for the National Bureau of Statistics, Zhao Qinghe, said that 78.9% of large and medium-sized enterprises in the PMI survey have since returned to work. Over the weekend, China’s official manufacturing PMI came in at just 35.7 for the month of February compared to 50 in January, far below market consensus of 45. Anything under 50 is contraction. Non-manufacturing PMI fell even lower as people all stayed home since the extended Lunar New Year holiday. That one plummeted to 29.6 in February from 54.1 in January. The plunge in both manufacturing and non-manufacturing PMIs is all due to effects from the Covid-19 outbreak and slow business resumptions since the Lunar New Year holiday. China has moved out of “fire fighting mode”, thinks Winnie Chi-Man Yip, a professor of global health policy and economics at Harvard University’s T.H. School of Public Health. “I think China is now transitioning to a more targeted approach, dealing with Wuhan, the hot spot, so they can allow for the rest of the country to return to some normal life,” she says. To date, most of the outbreak is centered in Wuhan, the capital of Hubei province. The overall number of new cases have been declining over the last five days. All eyes will be on Beijing and Shanghai as more people return to work.

As spread of virus slows in China, it picks up speed in the U.S. Health officials across the United States were scrambling on Monday to trace all those who had come into contact with infected patients, even as they struggled to get a handle on how far the virus had spread in the country. To date, the American authorities have reported a total of 88 cases nationwide, with two fatalities, both of them older adults with underlying health problems. A genetic analysis of the virus in Washington State, where the deaths occurred, suggested that the illness could have been spreading within the community for as long as six weeks before the first case was detected. The coronavirus, now present on every continent except Antarctica, has infected nearly 90,000 people, killing more than 3,000. The United States recorded its first two deaths attributed to the coronavirus over the weekend, as states from coast to coast reported new infections leading to a significant jump in the total number of cases. On Friday, there were 65 cases and no known deaths in the United States. Less than 48 hours later, a single hospital in Washington State reported two deaths, the makings of a cluster, and the total number of cases nationwide jumped 35 percent, to 88. One state, Florida, declared a public health emergency, even as Vice President Mike Pence, tapped to lead the federal response to the crisis, sought to calm the public’s nerves. Officials in Washington State said on Sunday that a second person, a man in his 70s with underlying health conditions, had died at the EvergreenHealth hospital in Kirkland, a Seattle suburb. That is the same hospital where officials identified the United States’ first coronavirus death on Saturday — a man in his 50s. The man in his 70s had been a resident at a nursing facility in Kirkland, run by Life Care Centers of America. Twenty-three cases were announced on Saturday and Sunday in California, Florida, Illinois, New York, Oregon, Rhode Island and Washington. The new cases included a mix of people who had traveled to high-risk countries and those who were believed to have contracted the disease domestically.

Did a Woman Get Coronavirus Twice? Scientists Are Skeptical February 27, 2020 News reports claim that a Japanese patient was reinfected with Covid-19. But there are other possibilities, like a bad test or a flare-up of the original disease. What could be worse than getting the pneumonia-like illness now known as Covid-19? Getting it twice. That’s what Japanese government officials say may have happened to a female tour bus guide in Osaka. The woman was first diagnosed with Covid-19 in late January, according to a statement released by Osaka’s prefectural government Wednesday. She was discharged shortly after, once her symptoms had improved. A subsequent test came back negative for the virus. Three weeks later she returned with a sore throat and chest pain and tested again. For a second time, she tested positive for Covid-19. Read all of our coronavirus coverage here. News reports detailing the case raised the possibility that people may not be developing immunity to the new coronavirus, even after they’ve recovered. But several infectious disease specialists say there’s not enough data to support that conclusion. Another possibility is that the virus subsided and flared up again. (Some viruses tend to do that.) Or the test was simply wrong. “The question is really: How good is the proof?” says Donald Burke, an international health expert at the University of Pittsburgh. “And we’re a long way away from anything solid. What we need is the sequence of the virus.” The only way to differentiate between a relapse—meaning the same coronavirus seemed to go away and came back—and reinfection, in which a second strain of coronavirus swooped in just as the first cleared out, is a full viral sequence. The tests currently available for diagnosing Covid-19 are based on a method called RT-PCR, which picks up some pieces of the virus’s genetic code, but not everything. It’s designed to grab only the chunks that are unique to the new coronavirus, but stable enough that they won’t disappear if it mutates. Thanks to some specialized fluorescent dye, the more viral bits there are, the brighter the genetic material glows, creating a pattern of light that signals for the presence of the virus. In theory, if doctors completely sequenced the virus in a patient’s nose or mouth swab at the time of each positive diagnosis, they could compare each genetic letter using genome-reading software and determine if the person had the same strain of coronavirus or a new one. Without that kind of data, says Burke, there’s no way of knowing for sure. But others think the more likely scenario is that the virus can just linger in some people’s bodies longer than expected. “I suspect this is in fact a continuation of the original infection,” says Susan Kline, an infectious disease physician and epidemiologist at the University of Minnesota. For one thing, she says, just not enough time has elapsed for reinfection. In the case of the other coronaviruses people catch, like the ones that cause the common cold, people tend to develop immunity following an infection. But it doesn’t last forever. The body produces antibodies that are protective against subsequent exposures, and then over time that immune response wanes, says Kline. That process usually takes longer than three weeks. “It’s too soon,” says Kline. “This entire outbreak has only been going on for two months. I would be very surprised if people are getting reinfected in that time span.” So why then, did the Japanese woman test negative? Kline says there are a few potential explanations. One is that the test wasn’t sensitive enough to pick up traces of the virus. The RT-PCR tests currently in use for diagnosing Covid-19 require a fair amount of genetic material to work well. If the coronavirus is actively making more copies of itself, there will be lots of RNA for the test to detect. But if the viral load goes down, the test can give false negatives. That can happen because the patient’s immune system is mounting a strong response. Or it could be that a treatment is slowing down the coronavirus’s self-replication spree. Another possibility is that the sample was just bad—the swab didn’t pick up much virus in the first place. Other viruses are known to exhibit similar dynamics. HIV, for example, starts out with a burst of viral replication before the immune system is alerted to its presence. Once the body starts to mount a response, the amount of viral genetic material drops, sometimes below detectable levels. Then as HIV attacks the body’s immune cells and patients get sicker and sicker, it becomes detectable again. During the SARS epidemic 17 years ago, there were reports of people who were given steroids to clear up their pneumonia. BY SARA HARRISON The Osaka case, which was first reported by Reuters, is the first time anyone in Japan has been reported to have--maybe—caught the coronavirus more than once. But similar cases have also been reported in China, which is still the epicenter of an outbreak that has infected more than 83,000 globally and killed 2,858, according to the World Health Organization’s latest numbers. Last month, a Chinese physician warned that it was possible for recovered patients to contract the virus again. Speaking at a Chinese National Health Commission briefing on January 30, Zhan Quingyuan, director of pneumonia prevention and treatment at the China-Japan Friendship Hospital in Beijing, said that while Covid-19 patients all seem to generate antibodies against the coronavirus, in some individuals the antibodies don’t appear to last for very long. “For those patients who have been cured, there is a likelihood of relapse,” Zhan said. Burke says he’s keeping an eye on this, and any other data that emerges suggesting people are relapsing from Covid-19 or catching multiple strains. Both could have potentially huge consequences for the public health response to the outbreak. “If there were an appreciable number of genuine relapses, I would be concerned about how you decide when it’s safe to take a person out of containment,” says Burke. Health workers would need to adapt monitoring strategies to make sure people aren’t reactivating and becoming infectious again once they’ve recovered and are back in their communities. And the spectre of reinfection raises a host of other potential problems. “If the natural infection doesn’t give you solid immunity, then we would start to worry about the effectiveness of a vaccine,” says Burke. Of course, at this point, all of that is just speculation. For now, the best advice anyone has to offer is the same thing the Japanese government told its own citizens in Osaka: Wash your hands, and practice good cough etiquette. Brigita, a director at one of China’s largest car dealers, is running out of options. Her firm’s 100 outlets have been closed for about a month because of the coronavirus, cash reserves are dwindling and banks are reluctant to extend deadlines on billions of yuan in debt coming due over the next few months. There are also other creditors to think about. “If we can’t pay back the bonds, it will be very, very bad,” said Brigita, whose company has 10,000 employees and sells mid- to high-end car brands such as BMWs. She asked that only her first name be used and that her firm not be identified because she isn’t authorized to speak to the press. With much of China’s economy still idled as authorities try to contain an epidemic that has infected more than 75,000 people, millions of companies across the country are in a race against the clock to stay afloat. A survey of small- and medium-sized Chinese companies conducted this month showed that a third of respondents only had enough cash to cover fixed expenses for a month, with another third running out within two months. Only 30% of such firms have managed to resume operations due to a complicated local government approval procedure as well as a lack of employees and financing, a government official said at a press conference on Monday. While China’s government has cut interest rates, ordered banks to boost lending and loosened criteria for companies to restart operations, many of the nation’s private businesses say they’ve been unable to access the funding they need to meet upcoming deadlines for debt and salary payments. Without more financial support or a sudden rebound in China’s economy, some may have to shut for good. “If China fails to contain the virus in the first quarter, I expect a vast number of small businesses would go under,” said Lv Changshun, an analyst at Beijing Zhonghe Yingtai Management Consultant Co. Industrial & Commercial Bank of China Ltd., the nation’s largest lender, has offered relief to about 5% of its small business clients. In an emailed response to questions from Bloomberg News, ICBC said it has allocated 5.4 billion yuan ($770 million) to help companies fight the virus. “We approve qualified small businesses’ loan applications as soon as they arrive,” the bank said. As a group, Chinese banks had offered about 794 billion yuan in loans related to the containment effort as of Feb. 20, according to the banking industry association, with foreign lenders such as Citigroup Inc. also lowering rates. Yang, a property manager of a seven-story mall in Shanghai, says a tenant who runs a 150-room hotel that’s usually busy has called asking for a month’s rent waiver after business dried up. She expects the massage parlor that rents space in the mall is also struggling and is open to extending some help. A deputy financing director at a small developer in central Anhui province said his firm is even being denied loans under existing credit lines. A drop in sales has hurt the company’s credit profile and a dearth of new projects means there’s no collateral to put up. Without access to credit, the business can survive for about four months, or maybe longer if some payments can be delayed, he said. Banks are hardly any better off themselves. Many are under-capitalized and on the ropes after two years of record debt defaults. Rating firm S&P Global has estimated that a prolonged emergency could cause the banking system’s bad loan ratio to more than triple to about 6.3%, amounting to an increase of 5.6 trillion yuan. Wu Hai, owner of Mei KTV, a chain of 100 Karaoke bars across China, took to the nation’s premier outlet of discontent, social media platform WeChat, to voice his despair. KTV’s bars have been closed by the government because of the virus, choking off its cash flow. The special loans from the authorities will be of little help and no bank will provide a loan without enough collateral and cash flow, he said on his official WeChat account earlier this month. Eisenstein China could suffer the loss of a million vehicles worth of production as factories in its crucial automotive industry remain shuttered until at least next week — and likely longer in Wuhan, the “motor city” at the center of the coronavirus outbreak. With more than 24,000 people infected, the impact of the highly contagious disease is also beginning to be felt by automakers in other parts of the world. Hyundai is suspending production in its South Korean plants because of a shortage of Chinese-made parts, and even European car manufacturers could be hit: Volkswagen and BMW could see a dip of 5 percent in their earnings for the first half of 2020, according to research firm Bernstein. We’re predicting that these auto shutdowns are just beginning. All it takes is a single component to be unavailable and the entire line has to be shut down. Is China the sole source for many critical components in the auto industry? Absolutely. Here’s an inside view: On Monday, Steve Banker and I had the opportunity to speak with Razat Gaurav, CEO of Llamasoft. Razat had some interesting takes on the outbreak, especially as it relates to the automotive and pharmaceutical supply chains. On average it takes 30,000 parts to make a finished automobile. Due to the virus, production facilities have already indicated that they will have lower than normal parts volumes. This has left companies scrambling to make contingency plans. During my conversation with Razat, he mentioned that inventories for most of these automotive parts are managed on a lean just-in-time basis. This means that, on average, companies have anywhere between two and twelve weeks of buffer inventory on-hand for automotive parts. As production volumes are decreasing, this has the potential to cause quite the global shortage of parts. The buffer inventory will only last so long, and once the pre-holiday supply runs dry, the industry is going to be in serious trouble. According to Gaurav: “Most OEMs single source components for new vehicles and China is a large supplier of those.” (Source – Logistics Viewpoints) “Single sourcing” is exactly what it implies. There’s a single factory somewhere churning out a single component that is absolutely vital to make a motorized vehicle. If that factory goes away for any length of time, a new source has to be identified or – worse, from a time and cost standpoint – built from scratch. But this vulnerability to China-dependent supply chains is by no means unique to the auto industry: Last month, the U.S.-China Economic and Security Review Commission held a hearing on the United States’ growing reliance on China’s pharmaceutical products. The topic reminded me of a spirited discussion described in Bob Woodward’s book, Fear: Trump in the White House. In the discussion, Gary Cohn, then chief economic advisor to President Trump, argued against a trade war with China by invoking a Department of Commerce study that found that 97 percent of all antibiotics in the United States came from China. (Source – CFR) That’s as close to a ‘sole source’ as you can get. And to put the cherry on top: guess the name of the region in China responsible for producing all if these antibiotics? Yep, Hubei province. With Wuhan its most important production hub. Can we find another source for our generic drugs and antibiotics? India, possibly. But here again we run into the same global interdependency issue: Another industry that is feeling the impact of the coronavirus is the pharmaceutical industry. The average buffer inventory for the pharmaceutical industry is between three and six months. However, this does not tell the full story. Gaurav mentioned that China is responsible for producing 40 percent of the active pharmaceutical ingredients (APIs) for the pharmaceutical world. Additionally, China supplies 80 percent of key starting materials (KSM’s), which are the chemicals in APIs, to India. Put together, this represents 70 percent of all APIs across the world. (Source – Logistics Viewpoints) India’s production is directly tied to uninterrupted supply from China: Indian generic drugmakers may face supply shortages from China if coronavirus drags on Feb 13 (Reuters) – Shortages and potential price increases of generic drugs from India loom if the coronavirus outbreak disrupts suppliers of pharmaceutical ingredients in China past April, according to industry experts. An important supplier of generic drugs to the world, Indian companies procure almost 70% of the active pharmaceutical ingredients (APIs) for their medicines from China. India’s generic drugmakers say they currently have enough API supplies from China to cover their operations for up to about three months. “We are comfortably placed with eight to 10 weeks of key inventory in place,” said Debabrata Chakravorty, head of global sourcing and supply chain for Lupin Ltd, adding that the company does have some local suppliers for ingredients. Sun Pharmaceuticals Industries Ltd said it has sufficient inventory of API and raw materials for the short term and has not seen any major disruption in supplies at the moment. The Indian drugmaker, however, said supply has been impacted for a few API products and the company is closely monitoring the situation. It did not identify the products. The Deceptively Simple Number Sparking Coronavirus Fears Here’s what the oft-cited R0 number tells us about the new outbreak—and what it doesn’t. ED YONG JANUARY 28, 2020 Updated February 4, 2020, at 10:35 a.m. When a new disease emerges, health organizations turn to a seemingly simple number to gauge whether the outbreak will spread. It’s called the basic reproduction number—R0, pronounced R-nought—and though useful for decision makers, it’s a nightmare for public communication. In brief, R0 is the average number of people who will catch the disease from a single infected person, in a population that’s never seen the disease before. If R0 is 3, then on average every case will create three new cases. But even though it seems incredibly straightforward, it’s hard to calculate and tricky to interpret. R0 is important because if it’s greater than 1, the infection will probably keep spreading, and if it’s less than 1, the outbreak will likely peter out. So it offers vital information to organizations and nations as they consider how to respond to an outbreak—such as the one the world is currently experiencing. In December, a previously unknown coronavirus, now called 2019-nCoV, emerged in the Chinese city of Wuhan. There have been more than 4,500 confirmed cases, the majority of which have been in mainland China. But several dozen cases have been detected in more than 15 other countries, and as the outbreak has spread, so has fear. Public-health researchers have sped to estimate the R0 of the new disease, and as they have shared their findings, this number has fueled several alarmed missives on social media. In the past week, at least six teams of researchers, along with the World Health Organization, have published estimates of R0 for the new coronavirus. All these groups used different methods, but their results have been mostly consistent, with estimates hovering between 2 and 3. WHO was a little more conservative than the others, with estimates of 1.4 to 2.5. One Chinese team is a clear outlier, with estimates of 3.3 to 5.5. And a British-led group initially published a high average value of 3.8 last week before revising it downward to 2.5 as new data emerged. In the intervening time, however, some observers seized upon the 3.8 number, with one Harvard epidemiologist describing it as “thermonuclear pandemic level bad” in a tweet that was retweeted more than 16,000 times, before he took it down. That’s a dubious interpretation, and here are six reasons why. First, the R0 estimates for the new coronavirus are in line with those for many other diseases. They’re similar to those for SARS (2 to 5) and HIV (also 2 to 5), and considerably lower than those for measles (12 to 16). Second, a bigger R0 doesn’t necessarily mean a worse disease. Seasonal flu has an R0 that hovers around 1.3, and yet it infects millions of people every year. SARS had an R0 of 2 to 5 and infected just over 8,000 people. The number is a measure of potential transmissibility. It does not actually tell you how fast a disease will spread. Photos: Wuhan under quarantine “People make the mistake of thinking that a high R0 means that you’re inevitably going to end up with a pandemic, and that’s not what it means at all,” says Maia Majumder from Harvard Medical School, who published one of the seven estimates for the new virus. In her view, if the number is higher than 1, we should take the disease seriously. But exactly how high it is beyond that threshold isn’t very informative at this stage. Why? Because third, R0 is an average. Let’s say the virus has an R0 of 2. This could mean that every single infected person passes the virus to two other people. It could also mean that one infected person is a “super-spreader” who infects 100 people, while 49 infected people infect no one. These two scenarios have radically different implications for what will happen during an outbreak. Super-spreader events are dangerous for health-care workers, but counterintuitively, they can sometimes be a good sign. They suggest that most infected people won’t actually perpetuate the outbreak, while the most problematic cases “may be more likely to be recognized due to their dramatic nature,” writes David Fisman of the University of Toronto. This attention can mean that control measures are put in place more readily, he explained in a posting to the ProMED email list. Other coronaviral diseases, such as SARS and MERS, involved super-spreader events, although it’s still too early to say if 2019-nCoV will have the same. Fourth, R0 is not easy to calculate. That’s especially true in the early days of an epidemic, when it’s not even clear how many cases there have been. Some people might have been infected without showing symptoms. Others might not have reported their symptoms to health authorities. Absent clear data on who has the disease, let alone how they’re moving around and interacting with other people, scientists have to calculate R0 by doing complicated simulations using a variety of possible methods. That’s why early estimates can vary so wildly, and why they should be taken with a grain of salt. Photos: The 1918 flu pandemic Fifth, R0 is not some magical, immutable property of the virus itself. It depends on how likely someone is to be infected after contact with an infectious person, and how often such contact occurs—and these quantities are also affected by how societies deal with a virus. When SARS first emerged, transmission dynamics played out very differently in China and Canada, which is why the virus’s R0 values cover a wide range, from 2 to 5. “In places with good infection control, where you can isolate cases as soon as they happen, you’ll see a lower R0 than, say, in places where an outbreak initially took off,” Majumder says. The current R0 estimates for the new coronavirus are specific to Wuhan, and mostly to the era before people knew about the virus. New estimates will emerge as the virus spreads to places that are now aware of and watching for it. “Likely, these will all be significantly lower,” says Kristian Andersen, a virologist at the Scripps Research Translational Institute. Sixth, R0 is not destiny. It is a measure of a disease’s potential. And once nations realize that a new disease exists, they can actively screen for it, check that health-care workers are using proper protection, and instigate quarantines. Even simple steps such as hand-washing might make a difference. All these measures could potentially lower the chances that the virus will spread and ensure that its actual transmission rate—the quantity known simply as R—is less than R0, and ideally less than 1. There are a few reassuring signs: One study suggests that patients are now being isolated just one day after showing symptoms, as opposed to six days after at the start of the outbreak. The new virus is a serious threat, and the world should absolutely start considering what to do if containment measures fail. But at a time of great uncertainty, people grasp for solid answers, and numbers seem to offer them. This new virus has emerged at a time when scientists have more avenues than ever for publishing their data and comparing notes. That can be a good thing, since fast and open communication can help bring diseases to heel more quickly. The risk is that a complicated number is released without context into a world that doesn’t know how to think about it. “Getting these R0 values out very rapidly is super important,” Andersen says. “But the way that some people and news outlets have interpreted what they mean … that part is problematic.”
